Abstract (German)

Die Vielzahl der in der Literatur beschriebenen und derzeit zur Verfügung stehenden Kiefergelenkendoprothesen und Rekonstruktionsplatten mit Gelenkkopf steht im Widerspruch zu ihrer relativ seltenen Anwendung und verdeutlicht, dass die Entwicklung der alloplastischen Rekonstruktion des Kiefergelenkes auch heute noch nicht als abgeschlossen betrachtet werden kann.
Aktuell wird zwischen Prothesen zum singulären Ersatz der Fossa, Prothesen und Rekonstruktionsplatten zum alleinigen Ersatz des Kondylus und Prothesen zum kombinierten Fossa-Kondylus-Ersatz unterschieden. Prothesen zum singulären Ersatz der Fossa und Prothesen zum kombinierten Fossa-Kondylus-Ersatz werden bei besonderer Belastung der Gelenkpfanne (chronisch-degenerative Erkrankungen, Arthritiden, idiopathische Kondylusresorptionen, Ankylosen, mehrfach voroperierte Schmerzpatienten) empfohlen. Der reine Kondylusersatz wird als temporärer Kiefergelenkersatz in der ablativen Chirurgie bevorzugt und sollte nur in Ausnahmefällen - möglichst unter Erhalt des Discus articularis - als langfristiger Ersatz dienen, um Penetrationen in die mittlere Schädelgrube vorzubeugen. Dies wird durch den Terminus �Rekonstruktionsplatte mit Gelenkkopf� besser zum Ausdruck gebracht als durch den eine permanente Rekonstruktion implizierenden Begriff �Kiefergelenkhalbendoprothese�.
Da vergleichende Langzeitstudien zur funktionellen und ästhetischen Wertigkeit der einzelnen Prothesentypen und Rekonstruktionsplatten mit Gelenkkopf fehlen, fällt es schwer, bei gleicher Indikationsstellung einzelne Modelle innerhalb der drei Gruppen zu favorisieren. Hier obliegt die Wahl individuellen Präferenzen.

Translation of the abstract (English)

Whereas in the early 1990�s particle shedding of the Vitek-Kent prosthesis brought discredit on each and every alloplastic TMJ replacement, novel condylar reconstruction plates and prostheses for total TMJ replacement that have been introduced since the turn of the millenium seem to prove an increased demand in alloplastic TMJ replacement.
Alloplastic TMJ replacement can be divided into prostheses only replacing the fossa, prostheses and reconstruction plates for replacement of the condyle and prostheses for the combined replacement of the fossa and condyle.
The surgeon should decide which anatomic structures he wants to reconstruct based on the specific indications. Any strongly loaded glenoid fossa (degenerative disorders, arthritis, ankylosis, multiply operated chronic pain patients) is to be treated by fossa or total joint prostheses.
Patients undergoing ablative surgery benefit most from only a temporary condylar replacement. If the latter is used for the long term in selected cases, the articular disc should be retained in order to prevent penetrations into the middle cranial fossa. The term �condylar reconstruction plate� makes this temporary character more obvious than the expression �ramus prosthesis� which implies a permanent reconstruction.
With long-term studies comparing the single prostheses and condylar reconstruction plates missing, it is finally the surgeon�s choice, which model to favour within the three different groups.
