Chronisch entzündliche Darmerkrankungen stellen klinisch ein heterogenes Krankheitsbild dar. In etwa 10% der Fälle ist eine Unterscheidung zwischen Morbus Crohn und Colitis ulcerosa nicht möglich. Neue serologische Marker zur Unterscheidung dieser Entitäten wären hilfreich. Die Diagnose einer CED fußt regelhaft auf klinischen, radiologischen, endoskopischen und histopathologischen Kriterien.
Wir haben daher neue Anti-Glykan-Serumantikörper – darunter die bisher unbekannten Marker Anti-L und Anti-C – zur Diagnose und Differentialdiagnose von CED untersucht. Zur Durchführung der Studie wurden insgesamt 824 Probanden analysiert, davon 363 Patienten mit Morbus Crohn und weitere 130 Patienten mit Colitis ulcerosa. Weitere 257 Proben entstammen Probanden ohne gastrointestinale Erkrankung (KO). Des Weiteren wurden Serumproben von 74 Patienten mit anderer gastrointestinaler Erkrankung (GI) als CED getestet.
Die Seren dieser 824 Patienten des „Kompetenznetz CED“ wurden geblindet auf das Vorhandensein von Anti-Chitobiosid-IgA-Carbohydrat-Antikörper (ACCA), Anti-Laminaribiosid-IgG-Carbohydrat-Antikörper (ALCA), Anti-Mannobiosid-IgG-Carbohydrat-Antikörper (AMCA), Anti-Saccharomyces-cerevisiae-Antikörper (gASCA), sowie Anti-Laminarin-IgA-Carbohydrat-Antikörper (Anti-L) und Anti-Chitin-IgA-Carbohydrat-Antikörper (Anti-C) mittels ELISA (Glycominds Ltd., Lod, Israel) sowie auf pANCA mittels Immunfluoreszenz (Glycominds Ltd., Lod, Israel) untersucht. Vollständige klinische Daten aller CED-Patienten – darunter klinischer Untersuchungsbefund, radiologische, endoskopische und histopathologische Ergebnisse – waren zur Auswertung verfügbar. Die Ergebnisse wurden mittels SAS Software Version 9.1 (The SAS Institute, Cary, NC, USA) und die Software R Version 2.4.1 (The R Institute for Statistical Computing, Wien, Österreich) analysiert.
Das Kollektiv der MC-Patienten war zu 77% positiv für wenigstens einen der getesteten Anti-Glykan-Serumantikörper. Die gemessenen Antikörpertiter waren bei MC signifikant erhöht gegenüber den Kontrollgruppen (p<0,001). gASCA wies mit AUC-Werten von 0,75 - 0,82 je nach Kontrollgruppe die höchste diagnostische Genauigkeit als Einzelmarker auf. In der Differentialdiagnose MC vs. CU zeigte sich die Kombination aus gASCA+/pANCA- für MC und gASCA-/pANCA+ für CU weiterhin als bestes diagnostisches Mittel, wie auch von anderen Autoren gefunden. Das Hinzuziehen von Anti-L und Anti-C zu gASCA/pANCA verbesserte die Diskrimination von MC zu CU (AUC 0,83 vs. 0,85; p=0,006). Außerdem verbesserte die Kombination aus Anti-L, Anti-C und gASCA (AUC 0,81) die Differenzierbarkeit von MC zu Kein MC (p=0,002), verglichen mit gASCA alleine (AUC 0,79). Das Entfernen von gASCA aus dem Panel reduzierte die Diskriminationsfähigkeit der Anti-Glykan-Serumantikörper (AUC 0,79 vs. 0,67; p<0,0001) signifikant.
Abschließend kann festgestellt werden, dass die getesteten Marker eine hohe Diskriminationsfähigkeit für MC im Vergleich zu CU, KO oder GI zeigen. Die getesteten Anti-Glykan-Serumantikörper AMCA, ALCA, ACCA und gASCA in Kombination mit pANCA, sowie mit Anti-L und Anti-C können zukünftig zur Diagnosestellung eines MC, sowie zur Unterscheidung eines MC vs. CU oder MC vs. Kein MC beitragen und damit in diagnostisch unklaren Fällen eine Zuordnung zur zugrundeliegenden Erkrankung ermöglichen.

Inflammatory bowel diseases (IBD) present clinically with a heterogenous picture. In about 10% of cases a discrimination between Crohn`s disease (CD) and Ulcerative Colitis (UC) is not possible. New serologic markers may be useful for differentiating these two entities. Usually, diagnosis is based on clinical, endoscopic, histologic and radiologic criteria.
We therefore tested a new panel of anti-glycan serum antibodies – containing the previously unpublished marker Anti-L und Anti-C – in diagnosis and differential diagnosis in IBD. For the study sera samples of 824 patients have been analyzed. These samples were taken from 363 Crohn´s disease patients and from 130 patients with ulcerative colitis. Further 257 samples derived from test persons without gastrointestinal diseases (KO) and served as controls. In addition 74 sera samples of patients with other gastrointestinal disease (GI) than IBD were also used as controls.
Sera of these 824 patients of the German „Kompetenznetz CED“ were tested for the presence of Anti-Chitobioside IgA Carbohydrate Antibodies (ACCA), Anti-Laminaribioside IgG Carbohydrate Antibodies (ALCA), Anti-Mannobioside IgG Carbohydrate Antibodies (AMCA), Anti-Saccharomyces cerevisiae Antibodies (gASCA), as well as Anti-Laminarin IgA Carbohydrate Antibodies (Anti-L) and Anti-Chitin IgA Carbohydrate Antibodies (Anti-C) with ELISA (Glycominds Ltd., Lod, Israel) and for pANCA with immunofluorescence (Glycominds Ltd., Lod, Israel) in a blinded fashion. Full clinical data on diagnosis of all IBD-patients based on clinical, endoscopic, histologic and radiologic criteria was available. Results were analyzed using SAS Software Version 9.1 (The SAS Institute, Cary, NC, USA) and Software R Version 2.4.1 (The R Institute for Statistical Computing, Vienna, Austria).
77% of the Crohn´s disease patients were positive for at least one of the tested serum markers. Levels of antibodies were significantly higher in Crohn´s patients than in the other groups (p<0,001). gASCA was found to be the best single marker with AUC-values of 0,75 - 0,82. In differential diagnosis CD vs. UC the combination of gASCA+/pANCA- for CD and gASCA-/pANCA+ for UC was the best diagnostic tool, as reported before.
Combining Anti-L und Anti-C to gASCA/pANCA increased differentiation between CD and UC (AUC 0,83 vs. 0,85; p=0,006). In addition the combination of Anti-L, Anti-C und gASCA (AUC 0,81) increased the differentiation of CD vs. Non-CD (p=0,002), compared to gASCA alone (AUC 0,79). Eliminating gASCA from the panel significantly reduced the discriminatory value of the serum anti-glycan antibodies (AUC 0,79 vs. 0,67; p<0,0001).
In conclusion a high discriminatory value of the serum anti-glycan antibodies can be shown for CD vs. UC, KO or GI. The tested markers AMCA, ALCA, ACCA and gASCA in conjunction with pANCA, as well as Anti-L and Anti-C may contribute to diagnosis of CD and to differentiation between CD vs. UC or CD vs. Non-CD. In the future the tested panel may help to correctly diagnose indeterminate cases of patients with IBD.
