Hintergrund:

Die Studienlage zur Wertigkeit einer diagnostischen TURP ist sehr spärlich. Wir verglichen Prostatastanzbiopsien und Gewebe von transurethralen Resektionen der Prostata bei 184 Patienten mit bekannten lokoregionärem Prostatakarzinom. Ziel der Studie war es, Faktoren zu ermitteln, bei denen im Rahmen der TURP es zu einer signifikant häufigeren Karzinomdetektionsrate kommt.

Patienten und Methoden:

184 Patienten mit stanzbioptisch gesicherten lokoregionärem Prostatakarzinom unterliefen einer TURP zur Vorbereitung auf eine HIFU Therapie. Das histologische Material der Stanzbiopsie und der TURP wurde bzgl. PSA-Wert, Gleason-Score (GS), Anzahl der betroffenen Stanzen und Lokalisation der positiven Stanzen verglichen.

Ergebnisse:

Die Detektionsrate bei der TURP betrug insgesamt 53.8% (99 von 184). Es konnten signifikant häufiger Karzinome nachgewiesen werden, wenn der PSA-Ausgangswert > 10 ng/ml war (p=0.004). Die Höhe des GS der Stanzzylinder hatte keinen Einfluss, jedoch war der GS der TURP Resektate signifikant höher als der, der entsprechenden Stanzzylinder (p=0.0417). Die Anzahl der entnommenen Stanzzylinder erbrachte keine signifikant erhöhte Tumordetektionsrate. War eine der mittleren Stanzen tumorpenetriert, konnten signifikant vermehrt Karzinome bei der TURP nachgewiesen werden (p=0.012).

Schlussfolgerung:

Unsere Daten zeigen, dass für ein bestimmtes Patientenkollektiv eine diagnostische TURP sinnvoll sein kann. Bei den Patienten sollte klinisch ein PC vermutet werden, ein kontinuierlich steigender PSA-Wert vorliegen und mind. zwei im Intervall entnommene negative Stanzbiopsien vorliegen.

Introduction:

The aim of our study was to evaluate the significance of transurethral resection of the Prostate (TUR-P) to detect prostate cancer. A transurethral resection is performed before high intensity focussed ultrasound (HIFU) to reduce the volume of the gland. A comparison of the TUR specimen of patients undergoing HIFU with the core biopsies was performed.

Methods:

Between April 2001 and April 2006 184 patients with localized prostate cancer were treated with TUR-P before undergoing HIFU therapy at our department. The resected tissue was histopathologicaly evaluated and compared to the histological results of the transrectal prostate biopsy.

Results:

Overall, prostatic cancer was detected in the resected prostatic tissue of 99 patients (53.8%). It was not possible to demonstrate a statistic significant correlation between positive TUR-P and prostate size or number of positive cores. There war a positive correlation detecting prostate cancer if the PSA level of the prostatic punch biopsy was higher than 10 ng/ml (p=0.004). There was no positive correlation between the Gleasonsum of the punch biopsy and detecting prostate cancer. But the Gleasonsum of the TURP tissue was significant higher than the Gleasonsum aof the punch biopsy (p=0.0417). If there was a positive core biopsy in the middle of the prostate gland we could detect significant more prostate cancer in the TURP tissue (p=0.012).

Conclusions:

With a high rate of the high amount of 53.8% prostate cancer detected by TURP our date gives evidence that in patients with continuously rising PSA levels and at least two negative biopsies a TURP might be indicated to detect prostate cancer.
