Abstract (German)

Die vorliegende Arbeit untersuchte retrospektiv die Sicherheit von Tirofiban in der Behandlung des akuten ischämischen Schlaganfalls in Monotherapie oder in Kombination mit verschiedenen Methoden der Gefäßrekanalisation. Zusätzlich wurden das Outcome mindestens drei Monate nach Schlaganfall sowie, falls entsprechende Daten vorhanden waren, die Rekanalisationsrate untersucht.
Alle Patienten die zwischen September 2004 und Januar 2008 in der Neurologischen Klinik der Universität Regensburg wegen eines ischämischen Schlaganfalls im Rahmen eines individuellen Heilversuches mit Tirofiban behandelt wurden, wurden identifiziert und ihre Daten hinsichtlich Risikoprofil, Schlaganfallschwere, initialer Bildgebung, Behandlungsregime, und Komplikationen ausgewertet. Bezüglich des Langzeitergebnisses erfolgte eine Befragung per Telefon und per Post.
Die Mehrheit der Patienten erhielt Tirofiban als Monotherapie (n= 34). Eine weitere Gruppe erhielt die Behandlung in Anschluß an eine i. v. oder i. a. Thrombolysetherapie (n=13) und eine dritte Gruppe (n=13) schließlich wurde im Rahmen eines �Bridging-Konzepts� behandlt. Insgesamt wurden nur 4 zerebrale Einblutungen festgestellt, wobei keine symptomatisch war. Bei 34 von 56 untersuchten Patienten zeigte die initiale Gefäßdarstellung einen kompletten oder nahezu kompletten Gefäßverschluß. Dokumentiert wurde eine Rekanalisation bei 12 Patienten, wobei allerdings nur 20 Patienten einer Kontroll-Gefäßdarstellung unterzogen worden waren. Ein vorteilhaftes 3-Monats-Outcome (mRS 0-2) konnte bei 43,3% der Patienten festgehalten werden.
In der vorliegenden Untersuchung ist die Schlaganfallbehandlung mit Tirofiban als sicher einzustufen. Im Gegensatz zur mit Abciximab durchgeführten AbESST II Studie traten hier mit Tirofiban nur geringfügige Blutungskomplikationen auf, was dazu ermutigt, die Substanz in größeren, randomisierten, kontrollierten Studien weiter zu untersuchen.

This thesis examined the safety of tirofiban in acute ischemic stroke as a monotherapie as well as in combination with other methods of vessel recanalization in a retrospective manner. In addition the outcome at least three months after stroke and - if data were available � the recanalization rate were determined.
All patients treated with tirofiban between September 2004 and January 2008 for acute ischemic stroke in the neurological department of the University Hospital in Regensburg were identified and their data were evaluated concerning individual risk profile, severity of stroke, initial brain imaging, treatment regime and complications. To collect information concerning long-term outcome, patients or their relatives were contacted via phone or mail.
Most patients received tirofiban as a monotherapy (n= 34). A second group was treated after IV or IA thrombolysis (n=13) and a third group was treated with tirofiban as a bridging therapy (n=13).
In all patients only four intracerebral haemorrhages occurred, none of them symptomatic. In 34 of 56 examined subjects the initial vessel-imagining showed a complete or almost complete vessel occlusion. Recanalization was documented in 12 patients. However it is to mention that only 20 out of these 34 subjects received control of their vessel-status after treatment.
A favourable three-months-outcome (mRS 0-2) could be documented in 43.3% of all patients.
Summing up it can be said that treatment of acute ischemic stroke with tirofiban seems to be safe. In contrast to the AbESST II trial where abciximab was used, only minor bleeding complications were seen. These results encourage investigating tirofiban in larger, randomised, controlled trials.
