Abstract (German)

Die vorliegende Arbeit verfolgt das Ziel, ein neues In-vitro-Verfahren zur Testung von Desinfektionsmitteln zu entwickeln, das der Situation in vivo noch etwas näher kommt als das bisher gebräuchliche Versuchsmodell nach Orstavik und Haapasalo und deren Modifikationen. Die entscheidenden Veränderungen des angewendeten Versuchsmodells betreffen den Inkubationsvorgang unter ständiger Bewegung der Versuchsansätze und die Bohrrichtung bei der Probenentnahme im Verlauf der Dentinkanälchen. Außerdem wird Wert darauf gelegt, dass sowohl die Dentinproben für die Infektion und für die Desinfektion, als auch die Sterilkontrolle von ein- und demselben Zahn stammen.

Dazu wurden aus den Zahnwurzeln extrahierter Rinderzähne jeweils drei Segmente geschnitten und in eine Sterilkontrolle, eine Infektionsprobe und eine Desinfektionsprobe unterteilt. Nach einer Vorbehandlung mit 20 % EDTA und der Inkubation mit einer Bakteriensuspension des Enterococcus faecalis für sieben Tage bei 37 °C erfolgte die Aufteilung der Probentripletts in fünf verschiedene Desinfektionsmittelgruppen: 0,5 % NaOCl, 1,0 % NaOCl, 3,0 % NaOCl sowie 0,2 % CHX und 1,0 % CHX. Die Einwirkzeit der Desinfektionslösungen betrug in einer weiteren Untergruppe 30 Sekunden und in der anderen Untergruppe zehn Minuten. Für die Überprüfung der Infektions- und Desinfektionsresultate wurden mit einem Rosenbohrer Dentinproben entnommen und anhand von daraus angefertigten Agarkulturen die Anzahl der gewachsenen Bakterienkolonien bestimmt und schließlich die prozentuale Überlebensrate berechnet.

Obwohl für die statistische Auswertung der Desinfektionsergebnisse nur wenige Stichproben zur Verfügung stehen, lassen sich auch mit dem neuen Modell die bisherigen Ergebnisse auf diesem Forschungsgebiet bestätigen: Sowohl NaOCl als auch CHX ermöglichen in Konzentrationen von weniger als 1,0 % nur eine unzuverlässige Desinfektion. Bei höheren Konzentrationen von 1,0 % und 3,0 % NaOCl oder 1,0 % CHX kann dagegen, unabhängig von der Einwirkzeit, von einer minimalen Überlebensrate der Bakterien von weniger als 0,1 % ausgegangen werden.

Translation of the abstract (English)

The aim of the actual study is to develop an in-vitro-method for testing the antibacterial efficacy of intracanal disinfectants, which is more close to the nature than the recent models by Orstavik and Haapasalo and their earlier modifications using in endodontolgy. The relevant changes in this method concern the conditions of incubation by continuous movement of the samples and the well direction of taking the test material following the dentin tubuli. In addition, it is important, that the samples for infection, disinfection und control originate from the same tooth.

Therefor the roots of extracted bovine teeth were cut into three segments und divided into an infection, a disinfection and a control specimen. After the pretreatment with 20% EDTA and incubation with a suspension of Enterococcus faecalis by 37°C for seven days, the triplets were spit into five disinfection groups: 0.5% NaOCl, 1.0% NaOCl, 3.0% NaOCl and 0.5% CHX, 1.0% CHX. The time of disinfection was determined in one group by 30 seconds and in another group by 10 minutes. The results of disinfection are examined by counting the bacteria colonies which were grown on agar-plates after taking dentin shavings with a round bur and incubating the sowed resuspensions.

Even though only few specimes were tested and analysed statistically, the established results in the endodontic research could be confirmed by the new method: NaOCl as well as CHX are not sufficient in disinfection in concentrations lower than 1.0%. Using higher concentrations of 1.0% NaOCl and CHX or 3.0% NaOCl, the minimal survival rate of the bacteria, independent of the disinfection time, do not exceed the level of 0.1%.
