Theoretischer Hintergrund: Psychischer Stress und traumatische Ereignisse spielen eine bedeutende Rolle im Alltag vieler „helfender“ Berufsgruppen. Eine diesbezüglich besonders exponierte Berufsgruppe ist die der Rettungsdienstmiarbeiter/-innen. Erst in den vergangenen 10 bis 15 Jahren wurde diesem Umstand zunehmend Rechnung getragen. Präventive und therapeutische Konzepte befinden sich aktuell in der Konzeptions-, Erprobungs- oder Verbreitungsphase. Entspannungsverfahren stellen hierbei einen Ansatz dar. Neben bereits in der Psychotherapie etablierten Entspannungsverfahren wie der Progressiven Muskelrelaxation (PMR) gilt es, weitere Verfahren zu finden und deren stressprotektive und stressreduzierende Wirkung, sowie deren Einsatztauglichkeit nachzuweisen. Ein erfolgversprechendes Verfahren ist die in Russland entwickelte und erst vereinzelt in Deutschland zur Anwendung kommende Short Universal Regulative Exercise (SURE), die durch die Ausführung gleichbleibender Pendelbewegungen entspannend und selbstregulierend wirken soll. Fragestellungen: Stellt das Entspannungs- und Selbstregelungsverfahren SURE eine effektive und praktikable Methode zur Prävention und Therapie von psychischem Stress bei Rettungsdienstmitarbei-tern/-innen dar? Methode: In einer prospektiven, gruppenrandomisierten, kontrollierten und einfachblinden Studie wurde an 46 haupt- und nebenberuflichen Rettungsdienstmitarbeitern/-innen der stresspräventive und stressreduzierende Effekt von SURE und PMR anhand der Ergebnisveränderung standardisierter Fragebögen (Eigenschaftswörterliste, Erholungs-Belastungs-Fragebogen), der Pulsfrequenzreduktion, der subjektiven Stressreduktion und dem subjektiven Benefit der Anwendung nachgewiesen. Ergebnisse: In 25 der 27 Subskalen (>90%) der standardisierten Fragebögen konnte nach achtwöchiger Anwendung von SURE eine positive Veränderung in Relation zu den Kontrollgruppenergebnissen nachgewiesen werden, die in vier Subskalen signifikant und in einer Subskala hochsignifikant ausfällt. Ein Vergleich zwischen SURE und PMR zeigte - bis auf die Ergebnisse einer Subskala - keine signifikanten Unterschiede. Schlussfolgerungen: SURE hat einen vergleichbar starken stresspräventiven und stressreduzierenden Effekt als das bereits in Deutschland etablierte Verfahren PMR. Bei beiden Verfahren kommen die positiven Effekte insbesondere mittel- bis langfristig zum Tragen. SURE zeigt gegenüber PMR eindeutige Anwendungsvorteile, da es keine Lernphase voraussetzt und der Zeitaufwand pro Anwendung bei vergleichbaren Resultaten deutlich kürzer ausfällt.

Translation of the abstract (English)

Background: In day-to-day life of people working in helping professions, psychological stress and traumatic events are important parts. That applies especially for emergency medical technicians (EMT). It was as recently as ten to fifteen years ago that there has been a closer focus on the above mentioned facts. Currently, preventive and therapeutical concepts are in the stage of preparation, proving and diffusion. One approach in this context is the use of relaxation techniques like the Progressive Muscle Relaxation (PMR) which is effectively used in psychotherapy. It is necessary to detect additional techniques, to prove their stress-protective and stress-reducing effects and to verify their efficiency during action. A promising technique is the Short Universal Regulative Exercise (SURE). The method was developed in Russia and is currently deployed very occasionally in Germany. It is believed to have a relaxing, self-adjusting effect which is based on performing constant pendular movements.
Objective: Is the relaxation and autoregulation technique SURE an effective and practicable method for preventing and treating psychological stress of emergency medical technicians?
Methods: The author conducted a prospective, group randomised, controlled and single-blind study with 46 full- and part-time emergency medical technicians to verify the stress-preventive and stress-reducting effect of SURE and PMR on the basis of changes in the results of standardized questionnaires (table of characteristics, recreational and stress questionnaire), the reduction of pulse frequency, the subjective stress reduction and the subjective benefit of the techniques. Results: After practicing SURE for eight weeks, there was a positive difference in 25 out of 27 subscales (>90%) of the standardized questionnaires in relation to the results in the control group. Four of these differences are significant, one is high significant. Comparing the results of SURE- and the PMR-group there were no significant differences found, except in one subscale. Conclusions: The stress-preventive and stress-reducing effect of SURE is comparable to the one of PMR. In both techniques the positive effects become important medium to long-term. SURE offers advantages in usage as it does not imply a didactive phase and as the time needed per application is distinctly shorter whilst showing comparable results.
