Die Transplantvaskulopthie (TVP) ist eine heutzutage noch unvermeidbare Komplikation nach Herztransplantation und beeinflusst signifikant die Funktion des Organs.
Zahlreiche Studien zeigten, dass die Induktion von HO-1 Protein vor einem
Ischämie-/Reperfusionsschaden schützen kann. Diese Schäden führen zu einer Intimaverletzung und einer daraufhin bedingten inflammatorischen Reaktion durch Immunzelleninfiltration. Diese immunologische Reaktion spielt die wesentliche Rolle bei der Entstehung der Intimahypertrophie, die bei der TVP typisch ist.

In dieser experimentellen Arbeit stellten wir ein Tranduktionsprotein her, um zu prüfen, ob die Induktion dieses Proteins die Intimahypertrophie verhindern bzw. reduzieren kann. Hierzu erzeugten wir ein rekombinantes Hämoxygenase-1 (HO-1)-Protein, das eine modifizierte Proteinstransduktionsdomäne (PTD) enthielt. Vor der erfolgten Aortentransplantation haben wir diese mit PTD-HO-1 und PTD-eGFP und HTK-Lösung infundiert. Hierbei zeigte sich, dass eine signifikant geringere Intimahypertrophie bei den PTD-HO-1-transduzierten Aortentransplantaten nachzuweisen war im Vergleich zu der Kontrollgruppe (PTD-eGFP transduzierten Aortentransplantate und HTK-Lösung). Dies ist mit signifikant weniger endothelialer Zellaktivierung, sowie deutlich geringerer Leukozyten- und Lymphozyteninfiltration in der Intima assoziiert. Diese neuen Erkenntnisse könnten eine neue Strategie bei der Therapie der TVP darstellen.

Although heart transplantation is a life-saving intervention for patients with severe heart disease, chronic rejection (CR) continues to provide a major obstacle to the long-term success of heart transplantation. CR is characterized by transplantvaskulopthie (TVP).

Previous studies have already demonstrated that induction of HO-1 protein
can protect ischaemia/reperfusion (I/R) injury. This damage leads to an intimal injury and induces inflammatory response by immune cell infiltration. This immunological response plays the major role in the development of intimal hypertrophy, which is typical of the TVP.

We hypothesized, that the induction of HO-1 protein might reduce intimal hypertrophy after heart transplantation. A tranduction protein was produced to check whether the induction of this protein supports this hypothesis. For this purpose a recombinant heme oxygenase-1 (HO-1) protein was generated, containing a modified protein transduction domain (PTD). Prior to this, we transplantated rat aorta and infused them intraluminal with PTD-HO-1 (study group) and PTD-eGFP and HTK (control groups) solution. PTD-HO-1-transduced allografts showed significantly lower intimal hypertrophy compared to the control group (PTD-eGFP transduced allografts and HTK solution). In addition, the studyl group demonstrated a significantly lower endothelial cell activation, as well as a lower leukocyte and lymphocyte infiltration compared to the control groups.

In conclusion, these new findings could represent a new strategy in the treatment of TVP.
