Langlebigkeit von Komposit- und Kompomer-Restaurationen an Milchmolaren mit und ohne Ruhigstellung der Patienten: eine retrospektive Studie

Ziel der vorliegenden Studie ist, die Langlebigkeitt von Komposit- und Kompomer-Restaurationen von Klasse-II-Restaurationen an Milchmolaren retrospektiv über eine Beobachtungsdauer von 5 Jahren zu untersuchen. Des Weiteren wurde der Frage nachgegangen, ob eine Abhängigkeit der Langlebigkeit dieser Restaurationen von der Behandlung "Ohne Ruhigstellung" und "Mit Ruhigstellung" besteht. In einer kinderzahnärztlichen Praxis wurden alle Daten erhoben und nachfolgend ausgewertet.
Für die Langlebigkeit von Komposit- und Kompomer-Restaurationen zeigte diese Studie bei Verwendung des gleichen Adhäsivsystems nach 5-jähriger Beobachtungsdauer keinen signifikanten Unterschied bei einem kumulativen Überleben der Komposite von 42,5% (n=48 von 112) und 49,3% (n=73 von 148) der Kompomere. Bezüglich der Langlebigkeit dieser Füllungsmaterialien bei Behandlung "Ohne Ruhigstellung" und "Mit Ruhigstellung" zeigten sich ebenfalls keine signifikanten Unterschiede. Das kumulative Überleben aller Restaurationen bei Behandlung "Ohne Ruhigstellung" lag bei 39,4% (n=35 von 90) und "Mit Ruhigstellung" bei 50,2% (n=85 von 170) nach 5 Jahren Beobachtungsdauer.
Des Weiteren konnte für die Langlebigkeit von Kompositen und Kompomeren im Vergleich in Abhängigkeit der Lokalisationen okklusal-mesial und okklusal-distal, der unterschiedlichen Berufserfahrung der Behandler, der Anzahl an kariöser Läsionen, der Anzahl an Recalls und der Dauer der Recallintervalle, des Geschlechtes der Patienten, des Alters der Patienten und der Austauschgründe ebenfalls keine signifikanten Unterschiede gezeigt werden.
Signifikante Unterschiede konnte die vorliegende Studie für die Langlebigkeit von okklusal-distalen Restaurationen der Kompomere im Vergleich zu okklusal-mesialen Restaurationen (p=0,003) zeigen. In Abhängigkeit der medianen Recallintervalle zeigte sich für beide Werkstoffklassen, dass Restaurationen von Patienten mit einem Recallintervall ≤ 3 Monate signifikant niedrigere Langlebigkeit aufwiesen (p=0,001 für Komposite und p=0,000 für Kompomere).
Als Fazit der vorliegenden Arbeit lässt sich festhalten, dass die Füllungswerkstoffe der Komposite und Kompomere sich in ihrer klinischen Langlebigkeit für die Behandlung von Milchmolaren nicht unterscheiden. Hinsichtlich der Behandlung "Ohne Ruhigstellung" oder "Mit Ruhigstellung" der Patienten bei Behandlung zeigten sich gleichermaßen keine unterschiedlichen Langlebigkeiten der Restaurationen.

Translation of the abstract (English)

The objective of the study was to retrospectively analyse the longevity of composite and compomer restorations in primary molars of class II restorations over a 5-year observation period. Furthermore, it was investigated whether longevity of these restorations was dependent on management “with immobilization” or “without immobilisation”. All data were collected in a paediatric dental surgery and subsequently analysed.
With regard to the longevity of composite versus compomer restorations when using the same adhesive, the study showed that there is no statistically significant difference after a 5-year observation period. The cumulative survival of composite and compomer restorations was 42.5% (n=48 of 112) and 49.3% (n=73 of 148), respectively. There was also no statistically significant difference between management “with immobilization” and “without immobilisation”. The cumulative survival of all restorations managed “without immobilisation” was 39.4% (n=35 of 90), and for those managed “with immobilisation” cumulative survival was 50.2% (n=85 of 170) after a 5-year observation period.
Furthermore, the following factors were found to have no statistically significant effect on the longevity of composite versus compomer restorations: the location of the restoration (mesial-occlusal versus distal-occlusal); the degree of experience of the dental practitioner; the number of carious lesions; the number of recalls and the time elapsed between recalls; the gender or age of patients; and the reasons for the exchange.
The study showed significant differences for the longevity of distal-occlusal versus mesial-occlusal compomer restorations (p=0.003). Regarding the average recall intervals for both classes of materials, the study showed that for patients with a recall interval ≤ 3 months, the longevity of the restorations was significantly lower (p=0.001 for composite and p=0.000 for compomer).
In conclusion, the study showed that composite and compomer classes of materials do not differ in their clinical longevity for the management of deciduous molars. Management “with immobilization” or “without immobilisation” also did not lead to different longevities of restorations.
