Durch eine zunehmende Entwicklung von Resistenzen gegen die gängigen Antibiotika sowie ein deutlicher Rückgang neu zugelassener Antibiotika muss nach neuen Alternativen zur Bekämpfung der Keime gesucht werden. Die photodynamische Inaktivierung von Bakterien (PIB) stellt eine vielversprechende Alternative zur Desinfektion von Oberflächen, Haut und Schleimhäuten dar. Bei der PIB werden durch Zusammenwirkung eines photoaktiven Farbstoffs, dem Photosensibilisator, und Licht reaktive Sauerstoffspezies erzeugt, welche unspezifisch humanpathogene Keime, wie Bakterien, Pilze oder Viren abtöten. In dieser Arbeit wurde in zwei Teilen einerseits durch Experimente in vitro sowie andererseits ex vivo am Schweinehautmodell die photodynamische Wirksamkeit von zwei neuen TMPyP-Derivaten untersucht.
Im Rahmen dieser Arbeit wurde TMPyP, ein kationischer PS aus der Gruppe der Porphyrine, der bereits erfolgreich in der PIB eingesetzt wurde, mit zwei von der Arbeitsgruppe in Kooperation mit dem Institut für Organische Chemie der Universität Regensburg neu synthetisierten TMPyP-Derivaten (TPyP 8+A sowie TPyP 8+B) verglichen. Die neuen Derivate tragen acht positive Ladungen an ihren funktionellen Gruppen, statt vier wie TMPyP. Ziel des ersten Teils war es zu untersuchten, ob mit den neuen Derivaten eine Wirkoptimierung zur Eradizierung von Gram-positiven (S. aureus und MRSA) sowie Gram-negativen Keimen (E. coli und CIP) im Vergleich mit TMPyP bei einer Bestrahlungszeit von nur 10 s erzielt werden konnte. Bei den Ergebnissen der in vitro Suspensionsexperimente zeigte sich bei einer Inkubationszeit von 30 s eine Verbesserung der antibakteriellen photodynamischen Wirkung der zwei neuen PS gegenüber TMPyP. Bei Letzterem zeigte sich bei 5 µM des PS eine Keimreduktion um über 5 log10-Stufen, was der Definition der Desinfektion entspricht. Diese Wirkung wurde bei TPyP 8+A sowie 8+B bereits mit 2.5 µM des jeweiligen PS erreicht. Bei einer Inkubationszeit von 10 min sowie 60 min zeigte sich kein Unterschied zwischen den neuen PS und dem Referenzfarbstoff mit allen vier untersuchten Keimen. Dies zeigt, dass bei einer kurzen Inkubationszeit die höhere positive Ladung der neuen PS einen Vorteil gegenüber TMPyP hat. Die in vitro Experimente wurden noch mal mit einer vorangehenden Waschprozedur durchgeführt, um zu überprüfen ob sich die Photosensibilisatoren an Bakterien anlagern bzw. aufgenommen werden. Die Waschexperimente zeigten, dass eine ausreichende Menge an Photosensibilisatormolekülen sich an die Bakterien anlagern bzw. aufgenommen wurden und somit eine photodynamische Inaktivierung der Bakterien bewirkten. Dies war abhängig von der Anzahl der positiven Ladungen.
In einem zweiten Schritt wurden die neuen PS an einem ex vivo Schweinehautmodell mit TMPyP verglichen. Hier zeigte sich eine photodynamische Keimreduktion bei S. aureus sowie E. coli, dennoch konnte eine Reduktion > 5 log10-Stufen (Desinfektion) nicht immer erzielt werden.
Dem ungeachtet konnte man mit diesen Experimenten auf lebender Haut den erfolgsversprechenden Einsatz der PIB für die Dekolonisierung von MRSA im Nasenrachenraum oder zur Eradizierung von Keimen auf superinfizierten Wunden mit TMPyP und den neuen Derivaten demonstrieren.

Translation of the abstract (English)

Because of an increasing development of resistances against current anbitiotica and a retreat of newly accepted antibiotica, it is important to look for new ways to fight microbes. The photodynamic inactivation of bacteria (PIB) is a promising possibility for the disinfection of surfaces, skin and mucosa. The combination of light and a photoactive dye, the photosensitizer, produces reactive oxygen species which kill microbes that are pathogenic to humans, e.g. bacteria, fungi or viruses. For this dissertation, the effect of two new TMPyP-derivatives has been examined in vitro and ex vivo with a porcine skin model.
TMPyP is a cationic porphyrin photosensitizer which has already been established successfully for the PIB. It has been compared to two newly synthesized TMPyP derivatives (TPyP 8+A and TPyP 8+B). The derivatives have been developed in cooperation with the institute for organic chemistry of the university of Regensburg. The new derivatives carry eight positive charges instead of four as TMPyP. The goal of the first part of this dissertation has been to examine whether the new derivatives have a better effect for the eradication of Gram-positive (S. aureus and MRSA) as well as Gram-negative (E. coli and CIP) bacteria in comparison to TMPyP. The results of the in vitro experiments have shown an improvement of the antibacterial effect of the two new photosensitizers with an incubation time of 30 s. With a concentration of 5 µM, TMPyP has lead to a 5 log10-step decrease of bacteria, which is the definition of disinfection. The same effect has been reached with 2.5 µM of TPyP 8+A and 8+B respectively. After an incubation time of 10 min as well as 60 min, there has been no difference between the two new dyes and the reference dye with all four tested bacteria. This demonstrates the advantage of a higher positive charge within a shorter incubation time. The in vitro experiments have been repeated with a preceding washing procedure to investigate whether the dyes have been attached or taken in by the bacteria. The washing experiments have demonstrated that a sufficient amount of photosensitizer molecules have gotten attached or taken in by the bacteria, which has lead to an inactivation of the bacteria consequently. This has been dependent on the number of positive charges.
In a second part, the new dyes have been compared to TMPyP using an ex vivo porcine skin model. A photodynamic decrease of bacteria could be observed for S. aureus and E. coli. Inspite oft hat, a 5 log10-step reduction (disinfection) of the microbes hasn´t always been achieved.
Regardless of that, the experiments on living skin pointed out the promising use of the PIB for the decolonization of MRSA in the nasopharynx as well as for the eradication of microbes on superinfected wounds with the help of TMPyP and the new derivatives.
