Die allogene hämatopoetische Stammzelltransplantation ist ein kurativer Heilungsansatz der akuten myeloischen Leukämie. Die bei 80% der 10/10 HLA-gematchten Spender-/Empfängerkombinationen aufgrund eines Verteilungsungleichgewichts vorliegende HLA-DPB1 Mismatch-Situation verminderte in Studien das Rezidivrisiko. Daher scheinen HLA-DPB1-Antigene ein mögliches Ziel der Graft-versus-leukemia Reaktivität und der adoptiven T-Zell Therapie darzustellen. In Vorarbeiten der Arbeitsgruppe wurden spezifische HLA-DPB1-spezifische AML-reaktive zytolytische T-Lymphozyten durch die Stimulation von naiven, CD4+CD45RA+ selektierten T-Zellen mit primären in 10/10 HLA übereinstimmenden aber HLA-DP ungleichen AML-Blasten in vitro generiert. Diese CD4+ T-Zellen lysierten sehr effizient AML-Blasten in vitro, die das fehlgepaarte HLA-DPB1 Allel trugen. Zusätzlich konnte in immundefizienten NOD/SCID/IL2Rgc-/- (NSG) Mäusen eine vollständige Eradikation HLA-DPB1 exprimierender AML-Blasten im xenogenen Transplantatmodell nachgewiesen werden.
Um die Herstellung von HLA-DPB1 reaktiven T-Zellen zu vereinfachen, wurde der T- Zell Rezeptor eines stark lytischen HLA-DPB1*04:01 spezifischen CD4+ T-Zell Klons (Klon 11C12) isoliert und anschließend in T-Zellen gesunder Spender transferiert. Die Feinanalyse dieses TZR11C12 zeigt eine Kreuzreaktivität des Rezeptors gegen das HLA-DPB1*04:02, das dem HLA-DPB1*04:01 sehr ähnlich ist. Damit kann der Rezeptor in über 50% der Patienten für eine Therapie angewandt werden. Zusätzlich fand die Erkennung des Rezeptors unabhängig von der alpha-Kette des HLA-DP statt, sodass die Zielstruktur von der beta-Kette des HLA-DPA1*01:03/ -DPB1*04:01 ausgeht. Nachdem der TZR zusätzlich ein breites Spektrum von HLA-DPB1*04:01 exprimierenden hämatopoetischen und nicht hämatopoetischen Zellen erkannte, wurde die Kreuzreaktivität von TZR11C12 transfizierten T-Zellen gegenüber xenogenen Zellen überprüft, die zuvor mit HLA-DPB1*04:01 kodierender mRNA transfiziert wurden. Hierbei zeigte sich eine spezifische Erkennung von HLA-DPB1*04:01 überexprimierenden Zellen verschiedener Spezies. Diese Daten lassen vermuten, dass der TZR11C12 das HLA-DPB1*04:01 „peptidunabhängig“ erkennt.
Da TZR11C12 ein großes Spektrum an nicht hämatopoetischen Zellen erkannte, wurden GvHD-Surrogatzellen aus Zelllinien humaner Leberzellen, Fibroblasten und Darmzellen nach IFN-y Vorbehandlung getestet, um die HLA-DP Expression heraufzuregulieren. In Anwesenheit von inflammatorischen Zytokinen zeigte sich eine Reaktivität von CD4+ TZR11C12+ T-Zellen gegen eine HLA-DPB1*04:01 positive Fibroblastenzelllinie, sodass die T-Zellen in der Lage sind, eine HLA-DPB1*04:01 spezifische GvHD im Patienten zu induzieren. Somit müssen für eine klinische Translation Sicherheitsmechanismen integriert werden, die eine solche GvHD Reaktivität verhindern oder diese nach Auftreten stoppen können.

Translation of the abstract (English)

Allogeneic hematopoietic stem cell transplantation is a curative therapy for AML. In 10/10 HLA-matched donor/patient pairs HLA-DPB1 mismatches occur with a frequency of about 80% due to a linkage disequilibrium. Studies demonstrated that HLA-DP mismatches reduce the risk for leukemia relapse, suggesting HLA-DPB1 as a target for graft-versus-leukemia reactivity and adoptive T-cell therapy. In preliminary studies, HLA-DPB1 specific AML-reactive T-cells could be stimulated by the in vitro stimulation of CD4+ CD45RA naïve-derived T-cells with 10/10 HLA-matched but HLA-DPB1 mismatched primary AML blasts. These CD4+ T-cells efficiently lysed AML-blasts carrying the mismatched HLA-DPB1 in vitro. Additionally, a complete eradication of HLA-DPB1 expressing AML blasts could be shown in a AML xenograft model in NOD/SCID/IL2Rgc-/- mice.
To simplify the generation of HLA-DPB1 reactive T-cells, the T-cell receptor (TCR) of an HLA-DPB1*04:01 specific T-cell clone (Clone 11C12) was isolated and transferred in T-cells of healthy donors.
Detailed analysis of TCR11C12 showed a cross-reactivity of the TCR against HLA-DPB1*04:02, which is closely related to HLA-DPB1*04:01. This offers the possibility to applicate the receptor in more than 50% of the caucasian population. Furthermore, the receptor recognized its target independently of the alpha-chain of HLA-DPA1*01:03/-DPB1*04:01, indicating the specific antigenic target structure of the receptor on the beta chain of HLA-DP.
Since the TCR showed specific reactivity against a broad panel of HLA-DPB1*04:01 expressing hematopoietic and non-hematopoietic cells, cross-reactivity of TCR11C12 was tested against HLA-DPB1*04:01 electroporated xenogeneic cells. Herein an HLA-DP specific recognition of HLA-DPB1*04:01 overexpressing cells from different species could be shown. This data suggests a peptide “independent” reactivity against HLA-DPB1*04:01 for TCR11C12.
Because the receptor had a specific reactivity against non-hematopoietic cells, the recognition of GvHD surrogate cell lines (i.e.liver cells, fibroblasts and colon cells) after IFN-y incubation to upregulate HLA-DP expression was tested. In presence of inflammatory cytokines, TCR11C12 expressing CD4+ T cells specifically recognized an HLA-DPB1*04:01 positive fibroblastic cell line, suggesting that TCR11C12 expressing T cells are capable to induce an HLA-DPB1*04:01 specific GvHD in patients. For a safe clinical application, mechanisms to prevent or stop such GvHD reactivity must be integrated.
