Parodontale Erkrankungen sind hochprävalent und betreffen bis zu 90% der weltweiten Bevölkerung (Pihlstrom et al., 2005). Herkömmliche Therapien konnten noch keine komplette Regeneration des parodontalen Gewebes erreichen (Inglehart, 2015; Sculean et al., 2015).
In der modernen Zahnmedizin stellen aus Fett gewonnene Stammzellen (ADSCs) eine vielversprechende Grundlage für zukünftige Behandlungsalternativen parodontaler Defekte dar. Diese Stammzellen lassen sich komplikationsarm und in großer Anzahl aus Lipoaspirat gewinnen. Da sie jedoch die Tendenz besitzen ohne exogene Stimulation adipogen zu differenzieren, ist die Steigerung der Effektivität der osteogenen bzw. zementogenen Differenzierung durch eine geeignete Supplementation Gegenstand aktueller Forschung.

In dieser Arbeit wurde durch Supplementation der Faktoren Bone Morphogenetic Protein 2 (BMP 2), Noggin, Dikkopf 1 (DKK1) und Wnt 3a die zielgerichtete Aktivierung und/oder Inhibition des BMP- bzw. Wnt-Signalweges von ADSCs getestet. Darüber hinaus wurde als weiterer Faktor Dexamethason (Hauptbestandteil des osteogenen Differenzierungsmediums (ODM)) eingesetzt.

Die Kombination aus BMP2, DKK1 und Dexamethason lieferte hierbei sehr vielversprechende Ergebnisse. Es liegt nahe, dass eine gleichzeitige Aktivierung des BMP-Signalweges und Inhibition des Wnt-Signalweges die osteogene Differenzierung der Stammzellen unterstützten kann. Dies deutet daraufhin, dass eine Wechselwirkung zwischen den Signalwegen besteht. Außerdem könnte ein durch Dexamethason induzierter Mechanismus entscheidend für die osteogene Differenzierung sein.

Periodontal diseases are highly prevalent and affect up to 90% of worldwide population (Pihlstrom et al., 2005). Standard therapy methods, however, have not yet succeeded in completely regenerating the periodontium (Inglehart, 2015; Sculean et al., 2015). A promising base for future treatment of periodontal defects in dental medicine is provided by adipogenic stem cells (ADSCs). These stem cells prove to be extractable easily and in high amounts from lipoaspirate. However, due to their tendency of adipogenic differentiation without exogenic stimulation the increase of efficiency of osteogenic and cementogenic differentiation by fitted supplementing is a matter of current research.
In this thesis the targeted activation or inhibition of the BMP- and Wnt-signaling pathways of ADSCs is investigated by supplementing with the proteins Bone Morphogenetic Protein 2 (BMP 2), Noggin, Dikkopf 1 (DKK1), and Wnt 3a. Moreover, dexamethasone (main constituent of the osteogenic differentiation medium (ODM)) was applied as an additional factor.
A combination of BMP2, DKK1, and dexamethasone yielded promising results. It is concluded that simultaneous BMP-signaling pathway activation while inhibiting the Wnt-signaling pathway facilitates osteogenic stem cell differentiation. This indicates that correlation of the signaling pathways plays an important role. It is furthermore reasoned, that a mechanism induced by dexamethasone might be crucial to osteogenic differentiation.
