Pathological plasmatic thrombin generation can lead to many complications such as thrombotic/atherosclerotic diseases. The contact phase of blood coagulation can be triggered by low plasma concentrations of lipophilic or negatively charged groups. There are two main proteases that can start the whole intrinsic coagulation cascade: activated factor 12 (F12a) and kallikrein. For activation of intrinsic coagulation it is sufficient to activate just one of them. There is a large inter-individual susceptibility of the F12a/kallikrein start of intrinsic hemostasis (e.g. by different protein glycosylation, different plasma matrix). Therefore, some individuals are especially susceptible to drugs that trigger pathophysiological thrombin generation. To avoid unwanted complications, these patients could be identified prior to drug administration by the recalcified coagulation activity assay (RECA) by determining the 200% stimulating concentration (SC200). This new ultra-specific thrombin generation test detects any pro-thrombotic tendency in plasma. Since valproate, a widely used drug to treat different diseases, such as epilepsy, is a simple lipophilic and negatively charged molecule, this thesis focused mainly on the action of valproate on thrombin generation. Other substances that can also trigger thrombin generation due their lipophilic or negatively charged groups such as tryptophan, leucine, or glucose were also investigated for comparison. 195 individuals of 235 (83 %) with approx. SC200  100 mg/l had an approx. SC200 value of 28  27 mg/l (MV  1 SD). Very fresh normal human plasma (<1 hour old) gave similar results to normal human plasma stored for some hours at 23°C (mean SC200 of the mean thrombin generations = 15 mg/l) and the 31 normal pools had approx. SC200 values of 119 mg/l (MV1SD; range: 2-35 mg/l). Furthermore, valproate also induced thrombin generation, although to different extent, in lyophilized plasma depleted for different coagulation factors. The thrombin generation was inhibited by prophylactic concentrations (0.5 IU/ml) of low molecular weight heparin (LMWH). Similarly, tryptophan and leucine at clinically used concentrations as well as added glucose could lead to the pathological generation of thrombin. Consequently, in order to protect patients (especially with other pro-thrombotic risk factors), who prove to be sensitive to valproate using highly specific and sensitive thrombin generation assay, such as the RECA, from unwanted thrombotic complications, anticoagulant (e.g. LMWH) prophylaxis could be considered. Using different in vitro and in vivo models the present thesis provides evidence that valproate used at clinically relevant concentrations induces thrombin generation.

Pathologisch-erhöhte Aktivität von Thrombin in Blut kann zu Komplikationen führen, wie zum Beispiel zu thrombotischen/ atherosklerotischen Erkrankungen. Negativ geladene oder lipophile Substanzen können bereits in niedrigen Plasma-Konzen¬trationen die intrinsische Blutgerinnung (Starterenzyme: F12a und/oder Kallikrein) aktivieren. Es besteht eine große individuelle Variabilität bezüglich des Starts der intrinsischen Hämostase (z. B. durch unterschiedliche Proteinglykosylierung, unterschiedliche Plasmamatrix). Daher sind einige Personen besonders anfällig für gewisse Medikamente, die eine pathophysiologische Thrombinbildung auslösen können. Um unerwünschte Komplikationen zu vermeiden, könnten diese Patienten vor der Medikamen¬tengabe durch den Recalcified Coagulation Activity Assay (RECA) mittels Bestimmung der 200% stimulierenden Konzentration (SC200) identifiziert werden. Dieser neue hochspezifische Thrombinbildungstest erkennt jede prothrombotische Tendenz im Blutplasma. Da Valproat, ein einfaches lipophiles und negativ geladenes Molekül, ein weit verbreitetes Medikament zur Behandlung verschiedener Krankheiten wie Epilepsie ist, konzentrierte sich diese Arbeit hauptsächlich auf die Wirkung von Valproat auf die Thrombinbildung. Andere Substanzen, die aufgrund ihrer lipophilen oder negativ geladenen Bestandteile die Thrombinbildung auch auslösen können, wie Tryptophan, Leucin oder Glucose, wurden zum Vergleich ebenfalls untersucht.
195 von 235 normalen Plasmen mit SC200 ≤ 100 mg/l Valproat hatten eine Thrombin-Generierung von 28  27 mg/l (MV  1 SD). 31 normale Citratplasma-Pools hatten SC200-Werte von ca. 11 ± 9 mg /l (MV±1 SD; Gesamtbereich: 2-35 mg / l). Sehr frisches (<1 Stunde altes) normales humanes Plasma ergab ähnliche Ergebnisse wie normales humanes Plasma, das einige Stunden bei 23°C gelagert wurde. Valproat induzierte die Thrombinbildung auch in verschiedenen lyophilisierten Mangel-Plasmen. Die Thrombinbildung durch Valproat wurde durch prophylaktische Konzentrationen (0.5 IU/ml) von niedermolekularem Heparin (LMWH) stark gehemmt.
In ähnlicher Weise führen Tryptophan und Leucin in klinisch verwendeten Konzentrationen sowie zu Plasma zugesetzte Glukose zur pathologischen Thrombinbildung. Die in vitro-Ergebnisse zu Valproat und Glucose in humanen Plasmen konnten in vivo bei Mäusen bestätigt werden. Um Patienten (insbesondere mit weiteren pro-thrombotischen Risikofaktoren), die sich in einem hoch-spezifischen und -sensitiven Thrombin-Generierungstest als empfindlich für Valproat erweisen, vor unerwünschten thrombotischen Komplikationen zu schützen, könnte eine Antikoagulans-Prophylaxe in Betracht gezogen werden.
Die vorliegende Arbeit liefert anhand verschiedener in-vitro- und in-vivo-Modelle den Nachweis, dass in klinisch relevanten Konzentrationen eingesetztes Valproat die Thrombinbildung induziert.
