Einleitung
Das Kniegelenk ist das am häufigsten operativ versorgte Gelenk. Der Nachbehandlung derartiger Eingriffe kommt eine besondere Bedeutung zu, da sie den operativen Erfolg sichert und die schrittweise Rückführung zu voller Funktionalität gewährleistet.

Methodik
Um den aktuellen Stand der Rehabilitation nach operativer Versorgung des Kniegelenkes abzubilden, wurden Nachbehandlungsrichtlinien von 120 orthopädisch-unfallchirurgischen
Institutionen untersucht. Die Module Gewichtsbelastung, Bewegungsausmaß, Orthesenversorgung, Bewegungsschienen-Therapie sowie Reha- und Sportartspezifisches Training wurden innerhalb der Hauptgruppen Knorpel-, Kreuzband-,
Meniskus-, Patellachirurgie und Endoprothetik einander gegenübergestellt und mit den Empfehlungen der aktuellen Literatur verglichen.

Ergebnisse
Trotz Etablierung knorpelchirurgischer Operationstechniken zeigt sich eine hohe Variabilität in der Nachbehandlung. Der größte Konsens besteht in Hinblick auf die
Empfehlungen zur Abstufung der Gewichtsbelastung.
Nach operativer Versorgung von Kreuzbandverletzungen sind für die weitere Nachbehandlung v. a. die Begleitverletzungen wie Innenband- oder Meniskusverletzungen von Bedeutung, die einer prolongierten und speziellen Nachbehandlung bedürfen. Standardisierte Nachbehandlungsprotokolle sind nach derartigen Eingriffen jedoch
nicht etabliert. Die Rehabilitation nach Meniskusrefixation variiert ebenso stark. Es sind v. a. bei der Gewichtsbelastung und dem Bewegungsausmaß Unterschiede sowohl innerhalb
der untersuchten Richtlinien als auch im Vergleich mit der aktuellen Literatur festzustellen.
Zur Nachbehandlung nach MPFL-Rekonstruktion kann gezeigt werden, dass sie ebenfalls einer hohen Variabilität unterliegt. Ein Konsens ist in keinem Modul der Nachbehandlung
auszumachen. Demgegenüber ist ein standardisiertes Vorgehen nach konservativer Therapie des instabilen Patellofemoralgelenkes etabliert.
Nach endoprothetischen Eingriffen wird aktuell ein standardisiertes evidenzbasiertes Vorgehen empfohlen. Unterschiede in der Nachbehandlung von Totalendoprothese im Vergleich zu einer Hemi-Prothese ergibt die Untersuchung nicht.

Fazit
Insgesamt zeigt diese Studie, dass nach operativer Versorgung des Kniegelenkes nicht nach einem standardisierten Protokoll nachbehandelt wird. Jedoch gibt es bezüglich
der Heterogenit�at der Empfehlungen große Unterschiede zwischen den Hauptgruppen
der Kniechirurgie. Es sind für die Module der Rehabilitation, v. a. für Rehaspezifisches und Sportartspezifisches Training hinsichtlich der Art und des optimalen Zeitpunktes
des Trainings, weitere Untersuchungen zur Etablierung einer standardisierten Rehabilitation nötig, um in Zukunft die bestmögliche, evidenzbasierte Nachbehandlung
zu gewährleisten.

Translation of the abstract (English)

Introduction
Surgical therapy of the knee joint is the most common procedure in trauma surgery and orthopaedics. Rehabilitation has a deep impact on the outcome because it does not only guarantee the success of the surgery but also leads to complete recovery of
the functionality of the knee joint.

Methods
To picture the status of rehabilitation after surgical treatment of the knee joint, rehabilitation protocols of 120 departments of trauma surgery and/or orthopaedics were researched. The modules of weight bearing, range of motion, orthosis therapy,
continous motion therapy and rehab/sport specific training were investigated and compared within the main groups of cartilage, ACL and PCL, meniscal, patellar and
endoprosthesis surgery. The findings are also presented in comparison with those found in current literature.

Results
Despite the establishment of cartilage repair techniques there is a wide range of divergent
postoperative rehabilitation protocols. The broadest consensus is reached on
the increase of weight bearing after such techniques.
Concerning ACL replacement, the results show that the rehabilitation period depends
to a large extent on concomitant lesions, like tears of the meniscus or the collateral
ligament. However, there is no agreement on standardized protocols.
There are various methods of rehabilitation after meniscal repair. Especially weight
bearing and range of motion are not released in a standardized way. In comparison
with current literature, there are few common methods of rehabilitation.
After MPFL reconstruction, there is no consent on how to proceed during the rehabilitation
period. In contrast, the conservative treatment of the instable patellofemoral
joint is subject to a nearly standardized protocol.
Endoprosthesis surgery is a widely-used procedure, and therefore the results indicate
a mostly standardized protocol for the rehabilitation. Di�erences in how to rehabilitate
after total knee replacement or unicompartmental replacement could not be
found.

Conclusion
All in all, the results show a wide range of different methods of rehabilitation after surgical techniques of knee surgery. There is no evidence-based standard rehabilitation
protocol. However, there are major differences within the main groups of knee surgery concerning the heterogeneity of the recommendations. Further investigation
is needed to identify an optimal and evidence-based rehabilitation after knee surgery, especially for the accurate beginning and type of rehab training and specific training.
