Das Neuroblastom (NB) stellt eine der häufigsten Krebserkrankungen im Kindesalter dar. Patient/innen mit Hoch-Risiko- bzw. rezidivierten oder unter Therapie refraktären Tumoren haben weiterhin eine ungünstige Prognose, weshalb die Entwicklung neuer Therapiemöglichkeiten dringend notwendig ist. Die RIST-Therapie, ein multimodaler Therapieansatz, in dem Biologika (Dasatinib und Rapamycin) mit einer „milden“ konventionellen Chemotherapie (Irinotecan und Temozolomid) in metronomischer Weise kombiniert werden, konnte in einer Pilotstudie bereits vielversprechende Ergebnisse liefern.
Entgegen der Erwartungen konnte der in der Kombinationstherapie bisher verwendete allosterisch wirkende mTOR-Inhibitor Rapamycin in klinischen Studien verschiedener Krebserkrankungen nur moderate Effekte erzielen. Es wird angenommen, dass die Effektivität der Behandlung durch neue ATP-kompetitive mTOR-Inhibitoren – wie Torin 1, Torin 2, AZD3147 oder PP242 – aufgrund ihrer hochselektiven und potenten Hemmung des mTOR-Signalwegs gesteigert werden kann. Das Ziel dieser Studie war eine vergleichende Untersuchung von Torin 2 und Rapamycin in Einzelbehandlung beziehungsweise in Kombination entsprechend dem RIST-Therapieschema, um möglicherweise zu einer Verbesserung des Therapieprotokolls beizutragen. Es konnte gezeigt werden, dass die IC50-Konzentrationen durch eine höhere Sensitivität der NB-Zelllinien gegenüber Torin 2 im Vergleich zu Rapamycin deutlich reduziert werden. Die Kombinationstherapien der mTOR-Inhibitoren Rapamycin beziehungsweise Torin 2 mit Dasatinib wiesen beide eine synergistische Wirkung auf. Zudem wurde eine stärkere und umfassendere Hemmung des mTOR-Signalwegs durch Torin 2 nachgewiesen. Die Untersuchung des Einflusses der Kombinationstherapie auf die Zellzyklus-Progression zeigte eine Inhibition der Cyclin-D1- und Induktion der p21-Expression. Dieser Mechanismus trägt möglicherweise zur Induktion des G1-Zellzyklus-Arrests bei. Die alternative RIST-Therapie mit Torin 2 führte trotz der tausendfach reduzierten Behandlungsdosis zu vergleichbaren Ergebnissen hinsichtlich einer Hemmung der Zellviabilität und Induktion von Apoptose.

Translation of the abstract (English)

Neuroblastoma (NB) is one of the most common malignancies in childhood. The prognosis for patients with high-risk NB or relapsed respectively treatment refractory disease (rNB) remains poor and novel therapeutic options are urgently needed. The RIST therapy, a multimodal therapy approach combining biologicals (Rapamycin and Dasatinib) with conventional chemotherapy (Irinotecan and Temozolomide) in a metronomic way has shown promising results in a compassionate use setting.
Contrary to expectations the anti-tumor effect of the actual used allosteric mTOR inhibitor Rapamycin was only modest in clinical investigation and it is assumed that new ATP competitive mTOR inhibitors – like Torin 1, Torin 2, PP2A and AZD3147 – could improve the efficiency by a more selective and potent inhibition of the mTOR signalling pathway. After testing various ATP competitive mTOR inhibitors in viability assays Torin 2 stood out as significant improvement compared to Rapamycin in inhibiting the viability of three different NB cell lines at significant lower concentrations. Therefore, Rapamycin and Torin 2 were compared alone or in combinations that are used in the RIST therapy approach to potentially contribute to an improvement of the therapy protocol. The study demonstrated that tested neuroblastoma cell lines are more sensitive to Torin 2 leading to a considerably lower treatment dose compared to Rapamycin. The effect of both combinations of the mTOR inhibitors with Dasatinib has shown to be synergistic. Additionally, Torin 2 inhibited mTOR signalling more potent and completely than Rapamycin. A further investigation of the impact on cell cycle progression by combination treatments showed an inhibition of Cyclin D1 and induction of p21 expression. This mechanism might contribute to the induction of G1 cell cycle arrest in NB cells. The alternative RIST treatment with Torin 2 affected NB cells lines relating to suppression of cell viability and induction of apoptosis as effective as Rapamycin despite a thousandfold reduced concentration applied.
