Ziel:
In der vorliegenden in-vitro-Studie sollte der Frage nachgegangen werden, ob die Vorbehandlung einer Zugangskavität nach endodontischer Behandlung einen Einfluss auf die Randdichtigkeit des postendodontischen Verschlusses mit Komposit hat

Material und Methoden:
Hierzu wurden 84 kariesfreie humane Molaren als Untersuchungsgut herangezogen. Zunächst wurden bei allen Zähnen beidseits approximale Kästen präpariert und im Sinne eines präendodontischen Aufbaus direkt versorgt. Anschließend wurde eine Wurzelkanalbehandlung durchgeführt und die Zähne schließlich auf sieben Versuchsgruppen aufgeteilt. Bei der Kontrollgruppe C1 wurde kein Adhäsiv appliziert. Bei der Kontrollgruppe C0 wurde Kontamination durch Spüllösungen, Sealer und Reinigungsalkohol vermieden und das Self-Etch Adhäsivsystem entsprechend den Testgruppen T1-T5 appliziert. T1 wurde ausschließlich mit einem Adhäsiv vorbehandelt. T2 wurde vor der Adhäsivapplikation mit Glycinpulver, T3 mit Aluminiumoxidpulver mikroabrasiv vorbehandelt. Die Zugangskavitäten der Gruppe T4 wurden mit einem Rosenbohrer mechanisch gereinigt. Diese mechanische Reinigung erfolgte in Gruppe T5 nach 24 Stunden provisorischem Verschluss. Alle Zähne wurden anschließend mit Komposit verschlossen. Die Hälfte der Zähne unterlief einer thermomechanischen Wechselbelastung. Anschließend wurden alle Zähne nach der Silbernitratfärbung in oro-vestibulärer Richtung mit einer Mikrotom Säge in 300 µm dicke Schnitte gesägt. Die Schnitte wurden unter einem Makroskop fotographiert und die Penetrationsstrecken nach dem Prinzip Maximum analysiert. Die Daten wurden nichtparametrisch mittels Mann-Whitney-U Test und Error-Rates-Methode analysiert (alpha=0,05)

Resultate:
Bei Betrachtung der Farbpenetrationswerte der Zähne ohne thermomechanische Wechselbelastung zeigte sich für alle Gruppen im Vergleich zur Kontrollgruppe C1 signifikant weniger Farbpenetration. Keine der Vorbehandlungen führte zu signifikant geringerer Penetration. Mit TCML gab es keine signifikanten Unterschiede zwischen allen Gruppen.

Zusammenfassung
Den Resultaten dieser in vitro Studie nach können zwei Hauptaussagen getroffen werden. Verwendet man ein Self-Etch Adhäsivsystem hat die Vorbehandlung einer Zugangskavität keinen Einfluss auf die marginale Integrität eines direkten postendodontischen Verschlusses zur Zahnhartsubstanz. Nach der Entfernung von Sealerresten mit Alkohol führten weder die zusätzliche zeitaufwändige mikroinvasive Reinigung in Form von Abstrahlen noch ein Substanzabtrag mittels Rosenbohrern zu einer Verbesserung der maginalen Integrität. Thermische und mechanische Belastungen beeinflussen die Randdichtigkeit eines direkten postendodontischen Verschlusses negativ.

Translation of the abstract (English)

Aim:
The aim of the study was to investigate the influence of cavity conditioning on marginal integrity of directly placed post-endodontic composite class-I-restorations in vitro.
Methodology:
A total of 84 fully intact human molars without caries or fillings received pre-endodontic composite restorations (2xclass-II) after their extraction. Occlusal endodontic access-cavities were prepared, and root canals were instrumented and filled with gutta-percha and an epoxy resin-based sealer. Prior to post-endodontic class-I-restoration, access cavities were divided into 7 different groups and pre-treated as follows. Group C0: positive control (not contaminated with sealer or hypochloride and adhesive used). Group C1: negative control (contamination, etching but no adhesive used). Group T1: use of adhesive; T2: glycine-polishing; T3: Al2O3 sandblasting; T4: carbide bur; T5: carbide bur and delayed restauration after temporary fillig. Half of the teeth from each group were subjected to thermocycling and mechanical loading (TCML). Marginal integrity of post-endodontic restoration was evaluated in oro-vestibular sections after AgNO3 dye penetration (DP) by standardized photomacroscopic imaging and expressed in per cent of margin length along all segments and separately for enamel, dentine and composite, respectively. Results were analysed non-parametrically (α = .05).
Results:
No restorations or teeth fractured or debonded completely. Without TCML, the median DP of all segments was significantly higher for the negative control compared with all other groups in oro-vestibular cutting direction (53%; p = .002). The other groups without TCML revealed 16%–24% DP (oro-vestibular). With TCML, the median DP in oro-vestibular cutting direction for all segments ranged between 48% and 62% for all groups, a significant difference was only observed between glycine-polishing and carbide bur (p = .041). For all other groups, the median DP of all segments ranged between 28% and 40% with TCML without significant differences. Error rates method (k = 7) revealed a significant influence of TCML in general on penetration of all segments.
Conclusion:
Additional access cavity pre-treatment did not improve the marginal integrity of post-endodontic composite restorations, as long as a self-etch adhesive is being used. And thermomechanical loading has a negative impact on the marginal integrity of direct postendodontic composite restaurations.
