Abstract (English)

The liver is a strong innate immune organ with a repertoire of liver-resident immune cells. The innate immune system operates as the first-line defense against pathogens but also induces tolerance to harmless antigens. Innate-like immune cells, namely, invariant Natural Killer T cells (iNKT), Type II NKT cells (T2NKT), Mucosal-associated invariant T cells (MAIT) and γδ T cells, contribute to liver homeostasis and pathogenesis (Figure 1). Liver diseases originate from excessive inflammation but it is unlikely to be diagnosed until the late stages. This represents a gap in the clinical management of patients. In this thesis, I address in Chapter 1 the role of innate immune cells in early liver inflammation and the patterns of inflammation according to liver pathology. Chapter 2 presents a case report of a patient with a liver transplant and recurrent hepatocellular carcinoma. Traditional liver function tests and immune cell biomarkers are discussed as poor biomarkers of therapy response and graft rejection. From Chapters 3-5, I evaluate the use of innate immune cells as biomarkers. Chapter 3 tackles T2NKT cells as potential biomarkers and their limitations. Chapter 4 explores iNKT cells as biomarkers of steatosis. Chapter 5 studies a receptor in hepatocytes that modulates iNKT cells' function. The dysregulation of the receptor in steatosis is explored as a biomarker of early liver pathogenesis. Chapter 6 integrates the findings of my work in a broader context and discusses the contributions to the field.

Die Leber ist ein starkes angeborenes Immunorgan mit einem Repertoire an leberresidenten Immunzellen. Das angeborene Immunsystem fungiert als erste Verteidigungslinie gegen Pathogene, induziert aber auch Toleranz gegenüber harmlosen Antigenen. Angeborene Immunzellen, nämlich invariant Natural Killer T-Zellen (iNKT), Typ-II-NKT-Zellen (T2NKT), schleimhautassoziierte invariante T-Zellen (MAIT) und γδ-T-Zellen, tragen zur Leberhomöostase und Pathogenese bei (Abbildung 1). Lebererkrankungen entstehen durch übermäßige Entzündungen, sind jedoch wahrscheinlich erst in fortgeschrittenen Stadien diagnostizierbar. Dies stellt eine Lücke in der klinischen Behandlung von Patienten dar. In dieser Arbeit untersuche ich in Kapitel 1 die Rolle angeborener Immunzellen bei frühzeitiger Leberentzündung und die Entzündungsmuster entsprechend der Leberpathologie. Kapitel 2 präsentiert einen Fallbericht eines Patienten mit Lebertransplantation und rezidivierendem hepatozellulärem Karzinom. Traditionelle Leberfunktionstests und Immunzell-Biomarker werden als unzureichende Biomarker für die Therapieantwort und die Abstoßung des Transplantats diskutiert. In den Kapiteln 3-5 bewerte ich die Verwendung angeborener Immunzellen als Biomarker. Kapitel 3 behandelt T2NKT-Zellen als potenzielle Biomarker und ihre Grenzen. Kapitel 4 untersucht iNKT-Zellen als Biomarker für Steatose. Kapitel 5 untersucht einen Rezeptor in Hepatozyten, der die Funktion von iNKT-Zellen moduliert. Die Dysregulation des Rezeptors bei Steatose wird als Biomarker für frühe Leberpathogenese untersucht. Kapitel 6 integriert die Ergebnisse meiner Arbeit in einen breiteren Kontext und diskutiert die Beiträge zum Fachgebiet.
