Diffuse Gliome, insbesondere das Glioblastom, sind mit einer sehr schlechten Prognose für den Patienten verbunden und bieten nur limitierte therapeutische Optionen. Um ein besseres Verständnis für die durch die Anwendung alkylierender Agenzien induzierten molekularen Veränderungen zu bekommen, wurden in dieser Arbeit Tumoren von insgesamt 47 Patienten longitudinal in der Primär- und Rezidivsituation untersucht. Von besonderem Interesse war die Analyse der Mismatch-Repair-Defizienz sowie die Entwicklung eines hypermutierten Phänotyps.
Die Auswertung der Immunhistochemie offenbarte eine deutlich verminderte Expression der Mismatch-Repair-Proteine in Rezidivtumoren. Der Zusammenhang zwischen einer Mutation der MMR-Gene und einer folglich verminderten Expression konnte dargestellt werden. Damit war eine Selektion für potenziell MMR-defiziente Tumorpaare möglich. Innerhalb dieser vorselektierten Paare konnten keine Hotspotmutationen des POLE-Gens detektiert werden. Als Erklärung dafür dient die Größe der Stichprobe sowie die Seltenheit solcher Mutationen. Eine MMR-Defizienz sowie Mutationen des POLE-Gens bei Gliomen sind entscheidende Veränderungen bei der Bildung eines hypermutierten Phänotyps. Ein Drittel der untersuchten Tumorpaare weist einen hypermutierten Phänotyp auf. Jedoch zeigen sich keine spezifischen Genveränderungen in den Rezidivtumoren im Vergleich zu den Primärtumoren. Die Veränderungen auf Ebene des Transkriptoms durch die Anwendung einer alkylierenden Chemotherapie können am ehesten als zufällig interpretiert werden. Trotzdem konnte damit die Heterogenität der Tumoren zueinander als auch innerhalb eines Tumorpaars bewiesen werden. Insgesamt konnten im Rahmen der NGS-mRNA-Sequenzierung neun relevant überexprimierte Kandidatengene (CD2, CD5, CD6, FMO3, MSLN, PIGR, PTCRA, SLAMF7 und SLC6A20) identifiziert werden. Für eine exaktere Einordnung, ob es sich dabei um mögliche therapierelevante Neoantigene handelt, sind weitere Auswertungen mittels Whole-Exome-Sequencing notwendig.

Diffuse gliomas, particularly glioblastomas, are associated with a very poor prognosis for patients and offer only limited therapeutic options. To gain a better understanding of the molecular changes induced by the application of alkylating agents, this study longitudinally examined tumors from a total of 47 patients in both primary and recurrent situations. Of particular interest was the analysis of mismatch repair deficiency and the development of a hypermutated phenotype.
The evaluation of immunohistochemistry revealed a significantly reduced expression of mismatch repair proteins in recurrent tumors. The correlation between a mutation in the MMR genes and the consequently reduced expression was demonstrated. This allowed for the selection of potentially MMR-deficient tumor pairs. Within these preselected pairs, no hotspot mutations of the POLE gene could be detected. This can be explained by the sample size and the rarity of such mutations. An MMR deficiency as well as mutations in the POLE gene in gliomas are crucial changes in the formation of a hypermutated phenotype. One-third of the examined tumor pairs exhibit a hypermutated phenotype. However, there are no specific gene changes in the recurrent tumors compared to the primary tumors. The changes at the transcriptome level due to the application of alkylating chemotherapy can most likely be interpreted as random. Nevertheless, the heterogeneity of the tumors both among each other and within a tumor pair could be demonstrated. Overall, nine relevantly overexpressed candidate genes (CD2, CD5, CD6, FMO3, MSLN, PIGR, PTCRA, SLAMF7, and SLC6A20) could be identified in the context of NGS mRNA sequencing. Further evaluations using whole-exome sequencing are necessary to more accurately determine whether these are potential therapy-relevant neoantigens.
