Ziel der vorliegenden Studie war es, den APOE-Genotyp, Diabetes mellitus, eine positive Familienanamnese Demenz sowie eine positive Familienanamnese Diabetes mellitus hinsichtlich ihrer Bedeutung als Risikofaktoren für Demenz in einen Gesamtzusammenhang zu stellen. Wir untersuchten 178 Demenzpatienten und 35 Kontrollprobanden. Es wurden eine neuropsychologische Testung, Consortium to Establish a registry on Alzheimer’s disease Testbatterie (CERAD), Uhrentest und ApoE Genotypisierung durchgeführt.

Dabei konnten folgende Schlussfolgerungen gezogen werden:

• Sowohl Apo ε4 als auch eine familiäre Häufung hinsichtlich Demenz stellen Risikofaktoren für DAT dar.
• Das Risiko für Demenz vom Alzheimertyp (DAT) steigt dabei mit der Anzahl der Apo ε4 Allele:
Während der heterozygote Apo ε4 Genotyp doppelt so häufig im Patienten- wie im Kontrollkollektiv auftrat, waren alle homozygoten Apo ε4 Genträger erkrankt und litten an DAT oder Mischdemenzen bestehend aus DAT und vaskulärer Demenz (VD).
• Es besteht zudem ein offensichtlicher Zusammenhang zwischen Apo ε4 und einer positiven Familienanamnese Demenz, da homozygote Apo ε4 Genträger häufiger eine positive Familienanamnese Demenz aufweisen als heterozygote oder Personen ohne Apo ε4 Allel.
• Diabetes mellitus und eine positive Familienanamnese Diabetes mellitus korrelieren besonders stark mit der Entwicklung einer VD.
• Im Gegensatz zur DAT ergibt sich kein Zusammenhang zwischen Apo ε4 und VD.
• Es besteht ebenfalls keine Beziehung zwischen Apo ε4 und Diabetes und Apo ε4 und einer positiven Familienanamnese Diabetes.

Im Hinblick auf die Prävention von Demenzerkrankungen spielt das Wissen um diese Risikofaktoren eine große Rolle. So können prädisponierte Personen bewusst das Auftretensrisiko einer Demenz reduzieren, indem sie für den Erhalt der Selbstständigkeit im höheren Lebensalter Gedächtnis, Kompetenz und Psychomotorik trainieren und sich um einen gesunden Lebensstil bemühen.

Langfristiges Ziel der vorliegenden Arbeit soll darüber hinaus sein, die Wissenschaft durch die erhaltenen Resultate in weiteren Forschungsstudien unterstützen. So wäre es beispielsweise interessant noch genauer zu untersuchen inwiefern sich speziell der juvenile Diabetes im Gegensatz zu Diabetes mellitus auf die Entwicklung von Demenzerkrankungen auswirkt oder ob Diabetiker mit HbA1c Werten im Normbereich das gleiche Risiko für Demenz besitzen wie Diabetiker mit erhöhten HbA1c Werten. Möglicherweise könnten die Ergebnisse der vorliegenden Studie sogar dazu beitragen Gene zu identifizieren, die bei Demenz eine Rolle spielen, wodurch neue therapeutische Ansätze für Demenzerkrankungen entwickelt werden könnten, die den Patienten und ihren Angehörigen zu Gute kommen.

Translation of the abstract (English)

Objective was to test that diabetes, a positive family history of dementia and a positive family history of diabetes are prevalent among dementia patients with onset before 70 years of age compared with healthy spousal controls. We collected 178 dementia patients and 35 control subjects. Neuropsychiatric examination, Consortium to establish a registry on Alzheimer’s disease test battery (CERAD), Clock-drawing Test and ApoE genotyping were performed in patients and controls.

We draw several conclusions:

• Both Apo ε4 and a family history of dementia are a risk for the development of dementia.
• The risk for Alzheimer’s disease (AD) rises according to the number of the Apo ε4 alleles:
• Whereas we had twice as many patients heterozygous for Apo ε4 than controls heterozygous for Apo ε4, all patients homozygous for Apo ε4 suffered from AD or mixed dementia (AD and vascular dementia (VD)).
• There is an obvious relation between Apo ε4 and a positive history of dementia. Subjects homozygous for Apo ε had more frequently a positive history of dementia than subjects heterozygous for Apo ε4 or subjects without Apo ε4.
• Diabetes mellitus and a positive history of diabetes mellitus strongly correlate with the development of VD.
• In contrast to AD there is no relation between Apo ε4 and a positive family history of diabetes.

Concerning prevention of dementia it is vital to know the risk factors. Subjects at risk may reduce their risk by training their self-dependence, memory and psychomotor domain and by aspiring to a healthy lifestyle.

In the long term, aim of the study is to support science with the results and maybe to inspire future studies. It might be of interest to further investigate the effect of juvenile-onset diabetes on the development of dementia or to test if the risk of diabetics with normal HbA1c values differs from the risk of diabetics with elevated HbA1c values. Maybe the results of the present study contribute to the identification of genes interfering with dementia and new therapeutic strategies might be developed in the treatment of dementia.
