Abstract (German)

Hintergrund: Die auditorische Phantomsensation Tinnitus kann bei betroffenen Patienten zu weitgreifenden somatischen, psychischen und sozialen Einschränkungen führen. Die Ursachenforschung ergibt nach wie vor kein wegweisendes Bild des Entstehungshintergrundes.

Methoden: Aufbauend auf einer Vorstudie (Mühlau et al. 53) wurden 28 Patienten mit chronischem Tinnitus mit Hilfe der Voxelbasierten Morphometrie untersucht und die Ergebnisse mit einer in Geschlecht, Alter und Hörvermögen übereinstimmenden Kontrollgruppe verglichen.

Ergebnisse: Es ergab sich eine signifikante Abnahme der grauen Substanz im Colliculus inferior der rechten Seite.

Schlussfolgerung: In Übereinstimmung mit der Vorstudie folgern wir, dass Tinnitus nicht nur mit funktionellen, sondern ebenso mit strukturellen Veränderungen verknüpft ist. Aufgrund der im Vergleich zur Vorstudie unterschiedlichen Lokalisation der strukturellen zerebralen Veränderungen schließen wir auf eine heterogene Pathogenese. Diese Erkenntnisse werden die HNO-ärztlichen als auch die neuropsychiatrischen Therapiekonzepte prägen

Translation of the abstract (English)

Background: Tinnitus as a subjective auditory sensation can lead to serious somatic, mental and social problems in patients. The origins of the syndrome are still hard to detect despite broad research.

Methods: In order to compare 28 patients with chronic tinnitus with a study by another research group (Mühlau et al. 53), we took images by voxel-based morphometry and matched them to a control group conform with sex, age and hearing.

Results: There was a significant decrease of grey matter in the colliculus inferior of the right side.

Conclusion: We agree with the survey of Mühlau et al. that Tinnitus is due to structural and functional changes. As we found structural changes in a brain-area different from the preliminary study, we suggest a heterogeneous aetiopathology of this disease pattern and its influence on otolaryngologic and neuropsychiatric therapy.
