Ziel dieser Arbeit ist die Umsetzung grundlegender Forderungen der aktuellen Richtlinie der Bundesärztekammer (Rili-BÄK) zur Qualitätssicherung laboratoriumsmedizinischer Untersuchungen, erschienen am 23. November 2007, für das klinisch-pharmakologische Labor der Klinik für Psychiatrie und Psychotherapie in Regensburg, welches Wirkstoffkonzentrationsbestimmungen von Psychopharmaka im Blutserum ...

Abstract (German)

Ziel dieser Arbeit ist die Umsetzung grundlegender Forderungen der aktuellen Richtlinie der Bundesärztekammer (Rili-BÄK) zur Qualitätssicherung laboratoriumsmedizinischer Untersuchungen, erschienen am 23. November 2007, für das klinisch-pharmakologische Labor der Klinik für Psychiatrie und Psychotherapie in Regensburg, welches Wirkstoffkonzentrationsbestimmungen von Psychopharmaka im Blutserum durchführt.
Hierfür wurden zahlreiche Arbeits- und Verfahrensanweisungen für die Wirkstoffkonzentrationsbestimmungen von Neuroleptika und Tranquillanzien erstellt. Darin sind alle verwendeten Geräte, Materialien und Reagenzien aufgelistet und alle Arbeitsschritte für die verschiedenen Wirkstoffkonzentrationsbestimmungen detailliert beschrieben.
Die interne Qualitätssicherung fordert die Messung kommerziell erworbener Kontrollproben mit bekannten Konzentrationen bei allen Messreihen um den regelkonformen Ablauf der Messung zu überprüfen. Für den Zeitraum von März 2009 bis Februar 2010 wurden, entsprechend der Vorgaben der Rili-BÄK, die Ergebnisse der Kontrollprobenmessungen in Tabellen festgehalten, alle geforderten Berechnungen durchgeführt und die Messergebnisse in graphischer Form dargestellt.
Im Verlauf der Arbeit stellte sich heraus, dass noch nicht alle Forderungen der aktuellen Rili-BÄK im Regensburger Labor im Zeitraum der Übergangsregelung der Richtlinie umgesetzt werden. Um eine hohe Qualität der Laborarbeit langfristig zu sichern, ist die Umsetzung aller Forderungen dringend anzustreben.

Translation of the abstract (English)

In the context of this thesis basic claims of the current guideline of the German Medical Association for quality control of clinical laboratory analyses, which was published on 23rd November 2007, are transposed for the laboratory of the Department of Psychiatry and Psychotherapy of Regensburg, which operates drug concentration determinations of antipsychotic drugs in serum.
Therefor work instructions and procedures were created for the drug concentration determinations of antipsychotics and tranquillizers. They list all necessary equipment, material and reagents and describe every relevant step for these determinations of drug concentrations.
The internal quality control claims the analysis of commercially available control samples with known drug concentrations to check its correct function. From March 2009 to February 2010 the results of the control sample measurements were documented in well arranged schemata, the requested calculations were realized and the results were illustrated in graphics, all as claimed by the guideline.
This thesis shows that not all claims are followed strictly in the laboratory in Regensburg while the temporary regulation of the guideline. To guarantee a long-term high quality standard, it is essential to transpose all claims of the guideline.
