Unsere Studie untersuchte die Assoziation bestimmter Merkmale alkoholabhängiger Patienten, die sich in der stationären Behandlung der Universität Regensburg befanden. Dabei konzentrierten wir uns auf drei Subgruppen, nämlich Geschlecht, Allelverteilung bezüglich des 141C Ins/Del-Polymorphismus im DRD2-Gen sowie zwei Gruppen von Alkoholikern, die nach Cloninger in Typ-1 und Typ-2 klassifiziert wurden.
Das 141C-Del-Allel des DRD2-Gens scheint vor der Entwicklung einer schweren Entzugssymptomatik zu schützen, obwohl diese Patienten signifikant höhere Mengen Alkohol trinken. Ob dieser Befund eventuell sogar nachteilig für die Prognose ist, müssen prospektive Studien mit Berücksichtigung des Verlaufes klären.

In unserer Arbeit sind deutliche Unterschiede zwischen Frauen und Männern zu erkennen, und zwar sowohl bezüglich der Entwicklung der Alkoholsucht als auch bezüglich der konsumierten Trinkmenge.
So waren Frauen signifikant älter beim Auftreten des ersten Rausches, beim ersten Kontrollverlust, bei der Entwicklung der Toleranz sowie beim Auftreten der ersten Entzugssymptome im Vergleich zu Männern. Männer zeigen höhere Trinkmengen im Vergleich zu Frauen.
Vor dem Hintergrund einer steigenden Anzahl alkoholkranker Frauen in der Gesellschaft und in den Therapieeinrichtungen scheint demnach zukünftig eine geschlechtsspezifische Therapie vielversprechend, die spezifische Aspekte der Alkoholkrankheit bei weiblichen Patienten berücksichtigt.
Unsere Daten zeigen darüber hinaus, dass die von Cloninger unterschiedenen Typ-1- und Typ-2-Alkoholiker für die Ereignisse in der Entwicklung der Alkoholabhängigkeit deutliche Altersunterschiede aufweisen. So ist das Alter bei Beginn des Rauschgiftkonsums und bei Beginn von Kontrollverlust zwischen den beiden Typen signifikant verschieden; Typ-2-Alkoholiker waren in unseren Daten deutlich jünger. Darüberhinaus zeigen Typ-2-Alkoholiker statistisch signifikant häufiger eine Toleranzentwicklung sowie negative soziale Folgen.
Die vorliegende Studie liefert neue Erkenntnisse zur Relevanz der für die Alkoholerkrankung wichtigen Dimensionen der Genetik, des Geschlechts und der Cloninger-Typologie. In allen drei Dimensionen konnten Zusammenhänge identifiziert werden, die die Relevanz jeder einzelnen dieser Dimensionen belegt. Damit konnten weitere Bausteine auf dem Weg zu dem übergeordneten Ziel eines Gesamtmodells geliefert werden, das der Komplexität dieser Zusammenhänge gerecht wird und ein vollständigeres Bild der Alkoholabhängigkeit in Aussicht stellt.

Translation of the abstract (English)

In our study we examined the coherence certain characteristics of alcohol dependent patients who were in stationary treatment at University Of Regensburg. We focused on the three sub-groups sex, distribution of alleles 141C Ins / Del polymorphism in DRD2 gene and the two groups of alcoholics categorized according to Cloninger’s typology of type 1 and type 2.
The Del-141C allele of the DRD2 gene appears to protect from developing severe withdrawal symptoms, although these patients drink significantly higher quantities of alcohol. Prospectively designed studies which regard the progressive course have to clarify whether this indication may even deliver a negative prognosis.
In our study we can see significant differences between women and men, both in terms of development of alcohol dependence and in amount of alcohol consumed.
Women compared to men were significantly older when developing their first intoxication, at their first loss of control, at developing a tolerance as well as at suffering from first withdrawal symptoms. Men show higher drinking levels then women.
Regarding the increasing number of alcoholic women both in society and therapy facilities a gender specific therapy, that considers the specific aspects of alcoholism with women, appears to be promising for the future.
Our data show that Cloninger’s type 1 and type 2 alcoholics have significant age differences for the key events in developing alcohol dependence. The age at which the drug consumption starts and at which loss of control begins is significantly different between the two types. Type 2 alcoholics were considerably younger according to our data. Also, type 2 alcoholics statistically show significant more often development of tolerance and negative social consequences.
The study at hand delivers new insights into the relevance of the dimensions genetics, sex and Cloninger typology, important for alcohol disease. We could identify correlations of all three dimensions which documents the relevance of each of the dimensions. That delivered more components of the greater goal of a comprehensive model, which copes with the complexity of that context, announcing a more comprehensive image of alcohol dependence.
