Einleitung: Glioblastome sind mit einem Anteil von 25% die häufigste hirneigene, intrakranielle Neoplasie des Erwachsenen. Die Prognose ist vergleichsweise schlecht: viele der durchschnittlich 65-70 Jahre alten Patienten versterben innerhalb eines Jahres. Das Gesamtüberleben zeigt sich dabei von verschiedenen prognostischen Faktoren abhängig, die wichtigsten sind hierbei das Alter, der Karnofsky Performance Status, neurologische Ausfälle, eine MGMT-Promotormethylierung sowie das Resektionsausmaß. Aufgrund ihres äußerst breiten Symptomspektrums, des invasiven, infiltrativen und diffusen Wachstumsmusters sowie der Vielzahl an transkriptionalen Subtypen – sogar in ein und demselben Tumor – stellen sie an hohe Anforderungen an die Behandler. Den therapeutischen Standard stellt hierbei seit 2005 die operative Resektion mit nachfolgender Radiochemotherapie und adjuvanter Chemotherapie mit Temozolomid nach dem Stupp-Schema dar.
Material und Methoden: Es erfolgte die retrospektive Analyse von 391 prospektiv eingeschlossenen Patienten, die zwischen Januar 2005 und Dezember 2013 am Universitätsklinikum Regensburg (UKR) die histologisch bestätigte Erstdiagnose eines Glioblastoma multiforme WHO Grad IV erhalten haben. Neben der Erhebung demografischer Daten erfolgte eine Morbiditäts- und Mortalitätsanalyse, um die Behandlungsgüte am UKR widerzuspiegeln. Beurteilungsmaßstäbe für das funktionelle Patientenoutcome bildeten der Karnofsky-Performance-Score (KPS), der neurological severity score (MCR – NPS scale) sowie die Besserungsraten von fokalen neurologischen Defiziten und Krampfanfällen insbesondere in Abhängigkeit vom Resektionsstatus. Für die Analyse des onkologischen Outcomes wurde die Komplikationsrate, das progressionsfreie Überleben (PFS) und das Gesamtüberleben (OS) ermittelt. Zudem fand eine Prüfung verschiedener klinischer, molekular-biologischer und epidemiologischer Faktoren hinsichtlich ihrer prognostischen Bedeutung statt. Um einen Vergleich der Outcome-Daten in Abhängigkeit von der Gründung des Zentrums für Hirntumore am Universitätsklinikum Regensburg zu ermöglichen, wurden schließlich zwei Kohorten gebildet - abhängig vom Beginn der Behandlung vor (Januar 2005 bis Juli 2009) bzw. nach der Zentrumsgründung (August 2009 bis Dezember 2013).
Ergebnisse: Das durchschnittliche Patientenalter lag bei 62.2 Jahren, dabei waren 33.5% der Patienten ≥70 Jahre. ) Leitsymptome waren neurologische Ausfälle (39.4%), Kopfschmerzen (33.8%) und Krampfanfälle (28.6%). 80.8% der Patienten wiesen dabei einen KPS von ≥70% auf. Die postoperative Morbidität betrug 13.0%, die Mortalität lag bei 0.51% - ohne signifikanten Unterschied zwischen den Resektionsgraden (p = 0.321). Ein neues, dauerhaftes neurologisches Defizit wurde in 2.1% der Fälle beobachtet. Sowohl der KPS, der MCR-NPS als auch neurologische Ausfallserscheinungen konnten durch die Operation signifikant verbessert werden. Das mittlere PFS aller Patienten betrug 8.65 Monate, das Gesamtüberleben ab Diagnose lag bei 13.7 Monaten.
24.3% der Patienten wurden ausschließlich biopsiert, 75.7% reseziert – davon erhielten 47.1% eine komplette Resektion(CR) und 52.9% eine inkomplette Resektion(IR). Den höchsten Benefit hatten Patienten mit CR mit einem OS von 20.6 Monaten, wobei IR-Patienten immer noch einen höheren Benefit hatten als nur biopsierte (13.3Monate vs. 5.8 Monate). Bezogen auf das Resektionsausmaß zeigte sich, dass sich der KPS durch eine operative Resektion (IR oder CR) signifikant verbesserte, während die Biopsie hier nur einen marginalen Effekt entwickelte (p < 0.001). Bezogen auf den MCR-NPS-Scale Score zeigte sich kein signifikanter Unterschied zwischen IR und CR, jedoch ein deutlicher Vorteil der Resektion gegenüber der Biopsie (p = 0.001). Das Gleiche zeigte sich sowohl für die Aphasie als auch für die Hirnnervenausfälle. Die Hirndrucksymptomatik besserte sich – wie bereits vorher erwähnt –generell infolge der Massereduktion am häufigsten, jedoch zeigte sich sowohl für die IR als auch für die CR eine ähnliche Verbesserungstendenz (94.4% vs. 93.8%; p = 0.88). Blickt man nun auf die verbleibenden fokalen neurologischen Defizite, zeigt vor allem die Hemisymptomatik (94.4% vs. 41.2%; p = 0.010) eine signifikante Verbesserung bei Erreichen einer CR. Die Gesichtsfeldausfälle, welche grundsätzlich bereits eine geringere Verbesserungstendenz zeigen, erfuhren durch die Biopsie hier keinerlei Verbesserung. Allerdings hatte hier die IR einen signifikant besseren Einfluss auf das Outcome als die CR (p < 0.01).
In Abhängigkeit von der Zentrumsgründung zeigt sich auch hier eine Verbesserung des Outcomes nach Gründung des ZHT: das PFS konnte von 7.2 Monaten auf 10.1 Monate (p = 0.021) gesteigert werden, das OS von 11.5 Monaten auf 15.4 Monate (p = 0.016).
Schlussfolgerung: Wir konnten zeigen, dass nicht nur die komplette Resektion, sondern auch die inkomplette Resektion in Hinblick auf das funktionelle Patientenoutcome der Biopsie überlegen ist. Zudem zeigte sich das onkologische Outcome in Abhängigkeit von der Gründung des ZHT signifikant gebessert. Wie unsere Ergebnisse somit zeigen, profitieren Patienten mit eher seltenen Erkrankungen von speziell organisierten Zentren, welche eine entsprechende Behandlungsstruktur und Expertise aufbieten können. Studien zur Behandlungsstruktur und –qualität bieten belegte Aussagen zum Wert der Resektion in der individuellen Patienten-Situation und bilden damit auch die Grundlage für eine möglichst Evidenz – basierte Beratung.

Introduction: Glioblastomas are the most common intracranial neoplasia of the adult brain with a proportion of 25%. The prognosis is comparatively poor: many of the 65-70 year old patients die within one year. Overall survival depends on various prognostic factors, the most important of which are age, Karnofsky performance status, neurological deficits, MGMT promoter methylation and the extent of resection. Due to their extremely broad symptom spectrum, invasive, infiltrative and diffuse growth pattern as well as the multitude of transcriptional subtypes - even in one and the same tumor - they place high demands on the practitioners. Since 2005, the therapeutic standard has been surgical resection followed by radiochemotherapy and adjuvant chemotherapy with temozolomide according to the Stupp scheme.
Material and methods: The retrospective analysis of 391 prospectively included patients who received the histologically confirmed first diagnosis of glioblastoma multiforme WHO grade IV at the University Hospital Regensburg (UKR) between January 2005 and December 2013 was performed. In addition to collecting demographic data, a morbidity and mortality analysis was performed to reflect the quality of treatment at the UKR. The Karnofsky Performance Score (KPS), the neurological severity score (MCR - NPS scale), and the improvement rates of focal neurological deficits and seizures, in particular depending on resection status, were used to assess functional patient outcome. For the analysis of the oncological outcome the complication rate, the progression-free survival (PFS) and the overall survival (OS) were determined. In addition, various clinical, molecular-biological and epidemiological factors were examined for their prognostic significance. In order to allow a comparison of the outcome data depending on the establishment of the Center for Brain Tumors at the University Hospital Regensburg, two cohorts were finally formed - depending on the start of treatment before (January 2005 to July 2009) and after the establishment of the Center (August 2009 to December 2013).
Results: The average patient age was 62.2 years, 33.5% of the patients were ≥70 years. ) The main symptoms were neurological deficits (39.4%), headaches (33.8%) and seizures (28.6%). 80.8% of the patients had a KPS of ≥70%. Postoperative morbidity was 13.0%, mortality was 0.51% - with no significant difference between the degrees of resection (p = 0.321). A new permanent neurological deficit was observed in 2.1% of cases. KPS, MCR-NPS and neurological deficits were significantly improved by surgery. The mean PFS of all patients was 8.65 months, the overall survival from diagnosis was 13.7 months.
24.3% of the patients were exclusively biopsied, 75.7% resected - 47.1% received a complete resection (CR) and 52.9% an incomplete resection (IR). Patients with CR had the highest benefit with an OS of 20.6 months, whereas IR patients still had a higher benefit than only biopsied patients (13.3 months vs. 5.8 months). In terms of resection extent, KPS was significantly improved by surgical resection (IR or CR), while biopsy showed only a marginal effect (p < 0.001). With respect to the MCR-NPS scale score, there was no significant difference between IR and CR, but a clear advantage of resection over biopsy (p = 0.001). The same was shown for both aphasia and brain nerve deficits. As mentioned earlier, cerebral pressure symptoms generally improved most frequently due to mass reduction, but a similar improvement was observed for both IR and CR (94.4% vs. 93.8%; p = 0.88). If one now looks at the remaining focal neurological deficits, the hemisymptomatics (94.4% vs. 41.2%; p = 0.010) in particular show a significant improvement when CR is reached. The visual field deficits, which in principle already show a lower improvement tendency, were not improved by the biopsy. However, the IR had a significantly better influence on the outcome than the CR (p < 0.01).
Depending on the establishment of the centre, there was also an improvement in the outcome after the establishment of the ZHT: the PFS could be increased from 7.2 months to 10.1 months (p = 0.021), the OS from 11.5 months to 15.4 months (p = 0.016).
Conclusion: We were able to show that not only complete resection but also incomplete resection is superior to biopsy in terms of functional patient outcome. In addition, the oncological outcome was significantly improved depending on the establishment of the ZHT. As our results show, patients with rather rare diseases benefit from specially organized centers, which can offer an appropriate treatment structure and expertise. Studies on the treatment structure and quality offer proven statements on the value of resection in the individual patient situation and thus form the basis for evidence-based counselling.
