Hypophysenadenome sind der dritthäufigste intrakranielle Tumor bei Erwachsenen. Die Indikation zur Operation besteht bei symptomatischen Tumoren mit Hormonsekretion oder Masseneffekt oder bei asymptomatischen Tumoren mit Wachstumspotenzial. Die Hauptziele der Hypophysenchirurgie sind eine maximale Tumorresektion ohne neurologische Schäden und eine Normalisierung oder Reduktion der hormonellen Dysfunktion bei Erhalt der physiologischen Hypophysenfunktion. Seit den frühen 1990er Jahren hat sich die endoskopische transsphenoidale Technik an Popularität gewonnen und hat in vielen Zentren die mikroskopische transsphenoidale Technik, die in den 1960er Jahren etabliert wurde, ersetzt. Mehrere Metaanalysen haben jedoch keinen klaren Vorteil der endoskopischen gegenüber der mikroskopischen Methode gezeigt. Die transkranielle Technik, die immer noch für Tumoren mit ausgeprägter suprasellärer und parasellärer Beteiligung verwendet wird, wurde in aktuellen Studien, die die Operationsmethoden vergleichen, nicht berücksichtigt. In dieser Studie haben wir alle drei Techniken verglichen, um umfassendere Daten zu liefern und die Einführung der endoskopischen Technik am Universitätsklinikum Regensburg zu bewerten. Der Vergleich der technischen Aspekte, des Outcomes und der Komplikationen aller drei Techniken bestätigte die Überlegenheit des transnasal-transsphenoidalen Zugangs als Goldstandard in der Hypophysenchirurgie, basierend auf einem signifikant niedrigeren Risiko für neurologische und schwere Komplikationen, einem signifikant kleineren Resttumorvolumen und signifikant kürzeren stationären Aufenthalten und Operationszeiten nach transnasaler im Vergleich zu transkranieller Operation.
Zwischen der endoskopischen und der mikrochirurgischen transsphenoidalen Resektion gab es keine signifikanten Unterschiede bezüglich der Tumorresektion und des Komplikationsrisikos. Eine signifikant kürzere stationäre Verweildauer, eine Tendenz zu einem geringeren Resttumorvolumen und ein ansonsten vergleichbares Outcome nach endoskopischer Operation durch weniger erfahrene Neurochirurgen im Vergleich zur mikrochirurgischen Operation durch sehr erfahrene Operateure bestätigen die erfolgreiche Implementierung der endoskopischen transsphenoidalen Technik am Universitätsklinikum Regensburg.

Translation of the abstract (English)

Pituitary adenomas are the third most common intracranial tumor in adults. Surgery is indicated for symptomatic tumors with hormone secretion or mass effect, or for asymptomatic tumors with growth potential. The main objectives of pituitary surgery are to achieve maximal tumor resection without neurological damage and to normalize or reduce hormonal dysfunction while preserving physiological pituitary function. Since the early 1990s, the endoscopic transsphenoidal technique has gained popularity and has replaced the microsurgical transsphenoidal technique established in the 1960s in many centers. However, several meta-analyses have not demonstrated a clear advantage of the endoscopic over the microsurgical method. The transcranial technique, which is still used for tumors with extensive suprasellar and parasellar involvement, has not been considered in recent studies comparing the surgical methods. In this study, we compared all three techniques to provide more comprehensive data and to evaluate the implementation of the endoscopic technique at the University Hospital Regensburg. The comparison of technical aspects, outcomes and complications of all three techniques confirmed the superiority of the transnasal-transsphenoidal approach as the gold standard in pituitary surgery, based on a significantly lower risk of neurological and major complications, a significantly smaller residual tumor volume, and significantly shorter hospital stays and operative times after transnasal compared to transcranial surgery.
The endoscopic and microsurgical transsphenoidal resection methods showed no significant differences in tumor resection and complication risk. A significantly shorter hospital stay, a trend towards a smaller residual tumor volume and a similar outcome after endoscopic surgery by less experienced neurosurgeons compared to microsurgical surgery by very experienced neurosurgeons confirm the successful implementation of the endoscopic transsphenoidal technique at the University Hospital Regensburg.
