Die Bildung von Thromben in ECMO-Pumpenköpfen bleibt ein nicht vollkommen verstandenes Phänomen. Die histologische Analyse der Thromben bietet einige wichtige Hinweise in Bezug auf die Ergründung der Ursachen und Abläufe.
Durch die histologischen Übersichtsfärbungen und die immunhistologische Charakterisierung der Thromben konnte ein besserer Überblick über die Bestandteile der Thromben gewonnen werden. Es wurde eine Methodik etabliert, mit welcher halbautomatisch eine größere Anzahl an Thromben untersucht werden kann, sowie die Vorteile und Nachteile der einzelnen Färbungen und Relevanz verschiedener Antikörper festgestellt. Bei der Untersuchung der Thromben zeigte sich eine große Heterogenität sowohl der Zusammensetzung als auch der Größe und Morphologie, es konnten jedoch einige sich wiederholende Kriterien ausfindig gemacht werden. Makroskopisch waren die Thromben gut in rote, weiße und gemischte Thromben einzuteilen, diese Einteilung konnte mikroskopisch bestätigt werden und die makroskopisch uneindeutigen Thromben einer Gruppe („Rot“ oder „Weiß“) zugeordnet werden. Somit erwies sich die mikroskopische Einteilung anhand der Erythrozyten in den Übersichtsfärbungen als die beste Methode für die Zuordnung zur roten oder weißen Gruppe. Morphologisch fanden sich an Ort A eine Schichtung unterschiedlicher Bestandteile und ortsunabhängig Verwirbelungen, Risse, Lakunen, bestimmte Verteilungsmuster der kernhaltigen Zellen in Aggregaten z.B. an den Rändern dichterer Bereiche innerhalb der Thromben sowie Erythrozyten in lockeren Netzen am Thrombusrand aufgelagert. Die Kolokalisation von CD45 und CD42b kann ein Hinweis auf immunologische Einflüsse bei der Thrombenbildung in ECMO-Pumpenköpfen sein. In der Quantifikation der Thrombusbestandteile zeigte sich bei den Thromben an Ort A eine signifikant höhere Thrombusfläche der roten Thromben im Vergleich zu den weißen Thromben, an den Orten B und C bestand dieser Zusammenhang nicht. In den erythrozytenreichen Thromben der Gruppe „Rot“ fanden sich weniger Thrombozyten und umgekehrt viele Thrombozyten in der erythrozytenarmen Gruppe „Weiß“. Fibrin und von Willebrand Faktor stellten den größten Flächenanteil in den Thromben beider Gruppen dar.
Die Patientendaten zeigten sich sehr heterogen, es konnten jedoch in der Gruppe „Weiß“ signifikant höhere Leukozytenzahlen und Fibrinogenwerte sowie niedrigere Thrombozytenzahlen als in der Gruppe „Rot“ bei der Analyse der letzten 6 Tage vor Explantation der Systeme gemessen werden.

Translation of the abstract (English)

The formation of thrombi in ECMO pump heads remains an incompletely understood phenomenon. The histological analysis of the thrombi offers some important insights into the etiology and mechanisms.
The histological staining and immunohistological characterisation have provided a better overview of the components of the clots. A method was established with which a larger number of thrombi can be analysed semi-automatically. The advantages and disadvantages of the individual stains and the relevance of different antibodies were determined. The examination of the clots revealed great heterogeneity in terms of composition, size and morphology, but some recurring characteristics could be identified. Macroscopically, the clots were categorised into red, white and mixed thrombi; the white and red group could be confirmed microscopically and the macroscopically not definable thrombi could be assigned to the red or white group. Thus, the microscopic classification based on the red blood cells (RBC) in the overview stains proved to be the best method for classification into the red or white group. At the rotor bearing (location A) there was a circular layering of different components. Independent of the location in the pump the recurring morphological features included areas of suspected turbulence, cracks, lacunae, certain distribution patterns of nucleated cells in aggregates, e.g. at the edges of denser areas within the clots, as well as RBCs in loose nets at the edge of the clot. The colocalisation of CD45 and CD42b may be an indication of immunological influences on thrombus formation in ECMO pump heads. The quantification of clot components showed a significantly larger area for the red clots at location A compared to the white clots in the same location; this correlation did not exist at sites B and C. There were fewer platelets in the RBC-rich clots of the ‘red’ group and significantly more platelets in the RBC-poor ‘white’ group. Fibrin and von Willebrand factor were the predominant components of the clots in both groups.
The patient data was very heterogeneous, but significantly higher leucocyte counts and fibrinogen values as well as lower platelet counts were measured in the ‘white’ group compared to the ‘red’ group when analysing the last 6 days before explantation of the systems.
