Abstract (German)
Das humanpathogene, gramnegative Bakterium Helicobacter pylori (Hp) besiedelt den menschlichen Magen und persistiert ohne Therapie lebenslang. Es kommt zu einer chronischen Gastritis mit oder ohne klinische Symptomatik. Es können sich jedoch auch Ulzera oder maligne Erkrankungen, wie Magenkarzinom oder MALT-Lymphom, bilden.
Ziel der Arbeit war die Analyse die Einflüsse von Hp auf das angeborene Immunsystem. Die immunstimulatorischen Eigenschaften von Hp wurden zunächst mit Maus-Splenozyten anhand eines Sekretionsprofils analysiert.
Dazu wurde die Sekretion der Zytokine TNF-a, IL-6, IL-10, IFN-g und IL-12 als Reaktion auf Hp über 36 Stunden untersucht. Es konnte gezeigt werden, dass die Konzentration von TNF-a nach 6 Stunden am höchsten war und dann zurückging. Die Sekretion der Zytokine IL-6, IFN-g und IL-10 setzte später ein, die Konzentration stieg bis zum Ende des Versuchs. Die Konzentration von IL-12 war vergleichsweise niedrig, sie lag nicht immer eindeutig über der Nachweisgrenze.
Als nächstes wurde der Beitrag einzelner Zellpopulationen zur Immunantwort auf HP analysiert. Hierzu wurden Makrophagen und dendritische Zellen (DCs) als wichtige Effektorzellen der angeborenen Immunantwort ausgewählt. In den Versuchen mit der Makrophagenzelllinie RAW264.7 wurden im Vergleich zu Experimenten mit Splenozyten unterschiedliche Sekretionsprofile der Zytokine gemessen. Um näher an den natürlichen Gegebenheiten zu sein, wurden die weiteren Versuche mit primären Makrophagen aus dem Knochenmark von Mäusen fortgesetzt.
Die Modulation der Immunantwort erfolgt nicht nur über Zytokine, sondern auch durch Zell-Zell-Kontakte, die von kostimulatorischen Molekülen wie CD40 und CD54 vermittelt werden. In den Experimenten mit primären Makrophagen konnte gezeigt werden, dass die Stimulation durch Hp die Sekretion der untersuchten Zytokine (TNF-a, IL-6 und IL-10) sowie die Expression von CD40 und CD54 steigert. Die Stärke dieser Veränderungen ist abhängig von der verwendeten MOI.
Wichtig für die Induktion der Immunantwort sind die Toll-like-Rezeptoren (TLRs), welche konservierte Antigene von Bakterien oder Viren erkennen. Daher wurde im folgenden Abschnitt der Arbeit die Bedeutung von TLR4 für die Reaktion auf Hp ermittelt. Dazu wurden die Veränderungen der Zytokinsekretion und der Expression von Oberflächenproteinen in Makrophagen und DCs aus Mäusen der Stämme C3H/HeJ (TLR4-Defektmutante, DM) und C3H/HeN (isogener Wildtyp) nach Stimulation mit Hp verglichen.
Bei Makrophagen und DCs der DM war eine Reduktion der Sekretion der pro-inflammatorischen Zytokine TNF-a und IL-6 im Vergleich zum Wildtyp zu beobachten. Eine Verringerung der Sekretion des anti-inflammatorischen Zytokins IL 10 konnte nicht gemessen werden.
Bei den untersuchten Oberflächenproteinen (CD11c, CD40, CD54, CD80 und CD86) war dagegen bei der DM stets eine Reduktion der Expression im Vergleich zum Wildtyp festzustellen. Dies zeigte, dass die Erkennung von bakteriellen Bestandteilen durch TLR4 eine erhebliche Rolle bei der Sekretion von Zytokinen und der Expression von Oberflächenantigen und somit bei der Immunreaktion auf Hp spielt.
Da Hp ein humanpathogenes Bakterium ist, wurden die weiteren Versuche mit humanen DCs durchgeführt. Hierbei wurde der Einfluss von Hp auf die Reifung von DCs untersucht. Die Stimulation zeigte, dass eine MOI zwischen 1 und 10 am besten geeignet war, um die Reifung auszulösen, ohne die Zellen zu schädigen. In diesem MOI-Bereich kam es zu einer optimalen Sekretion von IL-6, IL-8, IL-10 und IL-12. Ferner führt die Stimulation mit Hp zu einer erhöhten Expression von CD80, CD83, CD86 und MHC-II. Die Sekretion dieser Zytokine und die gesteigerte Expression der Oberflächenantigene zeigten, dass die Stimulation unreifer DCs mit Hp zur Reifung führte.
Für die Regulation der Reifung von DCs spielen die MAP Kinasen (MAPKs) p38, ERK und JNK eine wichtige Rolle. Zur Untersuchung des Beitrages einzelner MAPKs wurden diese mit spezifischen Inhibitoren für p38, ERK und JNK blockiert.
Die Kinetik der Sekretion von TNF-a, IL-6, IL-8, IL-10 und IL-12 sowie die Expression der Oberflächenproteine CD1a, CD80, CD83, CD86, HLA-ABC und HLA-DR wurde in Abhängigkeit von den Inhibitoren bei Stimulation mit Hp gemessen.
Es wurde gezeigt, dass die p38 den größten Einfluss auf die Sekretion der genannten Zytokine und somit die Reifung der DCs hatte. Eine Hemmung von p38 führte in den Versuchen stets zu einer signifikanten Reduktion der Zytokinsekretion. Die Inhibition von ERK und JNK führte meist zu einer Reduktion, die aber geringer ausfiel. Die Sekretion von IL-12 wurde dagegen durch Inhibition von ERK und JNK erhöht. Das ist ein Anzeichen dafür, dass ERK und JNK IL-12 negativ regulieren.
Bei der Regulation der Oberflächenproteine kam es durch die Inhibitoren nur selten zu signifikanten Unterschieden im Vergleich zur Stimulation mit Hp ohne Inhibitor. Auch hier war der Einfluss von p38 am größten, die Inhibition von ERK führte dagegen zu keiner signifikanten Veränderung.

Helicobacter pylori (Hp) is a human-pathogenic, gram-negative bacterium, which settles in the stomach and persists lifelong without therapy. The bacterium induces a chronic gastritis, which can be with or without clinical symptoms. The development of ulcera or malignant diseases such as gastric carcinoma or MALT lymphoma is also possible.
The aim of this thesis was to analyze the influence of Hp on cells the innate immune system. Firstly the immune-stimulatory properties of Hp were analyzed in mouse splenocytes by measuring a secretion profile.
During the experiments the secretion of the cytokines TNF-a, IL-6, IL-10, IFN-g and IL-12 was measured over a period of 36 hours. It was shown that the concentration of TNF-a reached its climax after six hours and declined thereafter. The secretion of IL-6, IFN-g and IL-10 started later, but the concentration increased over the remaining period of time. The concentration of IL-12 was always close to the detection limit.
Next the contribution of single cell populations to the Hp-induced immune response was analyzed. For this purpose macrophages and dendritic cells (DCs) which are important effector cells, were chosen. The experiments with the macrophage cell line RAW264.7 showed results which differed from those obtained by the analysis of splenocytes. Since immortalized cell lines are always at risk of being degenerated, further investigations were carried out with bone marrow derived primary macrophages.
The modulation of the immune response is not only regulated by cytokines but also by direct cell-to-cell contact via co-stimulatory molecules like CD40 and CD54. Cytokines are an activation marker of anti-presenting cells, whereas surface molecules indicate maturation.
In the course of the experiments with primary macrophages it was shown that stimulation by Hp enhances the secretion of TNF-a, IL-6 and IL-10. It also stimulates the expression of CD40 and CD54 in a dose-dependant way.
One important part of the immune response are Toll-like receptors (TLRs), which detect conserved structures of bacteria and viruses. Therefore the importance of TLR4 for the immune stimulation by Hp was assayed in the next chapter of this thesis. This was achieved by comparing the cytokine secretion and the expression of surface antigens of macrophages and DCs from C3H/HeJ and C3H/HeN mice. In C3H/HeJ mice the TLR4 is non functional, C3H/HeN is the corresponding isogenic wild-type strain.
In macrophages and DCs the mutant showed a reduced secretion of pro-inflammatory cytokines TNF-a and IL-6 as compared to the wild-type strain. A change in the secretion of IL-10 could not be demonstrated.
In contrast to this the expression of the surface antigens tested (CD11c, CD40, CD54, CD80 and CD86) was reduced in all cases compared to the wild-type. Thus we could show that the detection of bacterial components by TLR4 does play an important part in secretion of cytokines and the expression of surface antigens following stimulation with Hp.
Since Hp is an important pathogen in humans, further experiments were carried out in human DCs. Firstly the influence of Hp on the maturation of DCs was analyzed. The stimulation with Hp showed that a multiplicity of infection (MOI) between 1 and 10 is most suitable for our purposes. This dose induced the maturation of DCs without damaging the cells. Using an MOI in this range resulted in an optimal secretion of IL-6, IL-8, IL-10 and IL-12. Moreover it was shown that the stimulation with Hp enhanced the expression of surface antigens such as CD80, CD83, CD86 and MHC class II. Secretion of the cytokines mentioned and the increased expression of surface antigens indicate the maturation of DCs induced by Hp.
The MAP kinases (MAPKs) p38, ERK and JNK play an important part in regulating the maturation of DCs. To analyze the contribution of single MAPKs specific inhibitors for p38, ERK and JNK were used to prevent phosphorylation.
In the next step the secretion kinetics of TNF-a, IL-6, IL-8, IL-10 and IL-12 as well as the changes in the expression of CD1a, CD80, CD83, CD86, HLA-ABC and HLA-DR were measured depending on the inhibitors used during stimulation with Hp.
It was shown that p38 is most influential on the secretion of the tested cytokines and therefore on the maturation of DCs. Inhibition of p38 resulted in significantly reduced cytokine levels at all times. Inhibiting ERK and JNK also resulted in most cases in a reduction which was less significant. In contrast the inhibition of ERK and JNK enhanced the secretion of IL-12. This indicates that IL-12 is negatively regulated by ERK and JNK.
Looking at the surface antigens studied the changes caused by applying inhibitors are less prominent. Although the influence of p38 was bigger than those of the other kinases, inhibition of p38 only rarely resulted in significantly reduced expression of surface antigens. Inhibition of ERK on the hand did not resulted in any significant changes.
