Abstract (German)

Die Zahl der zirkulierenden Endothelzellen und die apoptotische Aktivität des Serums von Patienten die mit/ohne HLM eine aortokoronare Bypassoperation bekamen, wurden verglichen. Blut von 30 Patienten (15 im OPCAB-Verfahren operiert und 15 ohne HLM) wurde präoperativ, am Ende der OP, dann 1, 6, 12, 24 und 48h postoperativ abgenommen. Die CECs wurden immunomagnetisch (Dynabeads mit Anti-CDC-146 Antikörper beschichtet) isoliert und in einer Nageotte-Zählkammer gezählt. Die apoptotische Serumaktivität wurde in einer Kultur von humanen Endothelzellen gemessen.
Die Zahl der CECs und die apoptotische Serumaktivität sind signifikant während der OP in beiden Gruppen angestiegen (6h postoperativ). Im Vergleich zur HLM-Gruppe waren die CECs-Zahl und die apoptotische Serumaktivität in OPCAB-Gruppe signifikant niedriger.
Die Zahl der CECs und die Endothelapoptose sind Marker für Endothelaktivierung/Schädigung im Rahmen einer ACVB-OP. Es wurde gezeigt, dass der Einsatz der HLM mit einer signifikanten Endothelschädigung in unmittelbaren postoperativen Verlauf einhergeht.

Translation of the abstract (English)

We compared the numbers of circulating endothelial cells and the apoptosis-inducing capacity of serum samples in on-pump and off-pump coronary artery bypass grafting patients.
Blood samples from 30 patients undergoing CABG (15 patients off-pump and 15 on-pump) were collected after induction of anesthesia (preoperatively), at weaning from CPB/end of bypass grafting (0 h), and 1, 6, 12, 24, and 48 h afterwards. CEC were isolated with immunomagnetic anti-CD146-coated Dynabeads, and counted in a Nageotte chamber. The apoptosis-inducing activity of serum samples on hEC was examined by a tissue culture assay system.
CEC numbers and proportions of apoptotic hEC were significantly elevated during and after surgery in both groups (at 6h postoperatively). Compared with the on-pump group, CEC and proportions of apoptotic hEC were significantly lower in patients having CABG performed off-pump.
The number of circulating endothelial cells and apoptotic endothelial cell death are markers of endothelial activation and damage during CABG. This study provides evidence that the use of CPB is associated with a significantly more pronounced endothelial response in the immediate postoperative period.
