Seit der Entdeckung der Röntgenstrahlen ist die nichtinvasive Bildgebung in der Medizin nicht mehr wegzudenken. Neue Bildgebungsverfahren wie PET (Positronen-Emissions-Tomographie), SPECT (single photon emission computed tomography) oder Kernspinspektroskopie (functional magnetic resonance imaging, fMRI) liefern dreidimensional aufgelöste Informationen über biologische Vorgänge und die chemische Zusammensetzung im Körper. In der Strahlentherapie wird diese biologische Bildgebung verwendet um die Dosis an die Physiologie der Tumorzellen anzupassen und somit eine bessere Tumorkontrolle - bei gleichzeitiger Schonung der gesunden Organe - zu erreichen. Diese als Dose Painting bezeichneten, lokalen Dosismodulationen erfordern von einem Bestrahlungsplanungssystem die Fähigkeit, eine gezielt inhomogene Dosisverteilung zu realisieren.
In dieser Arbeit wird das von M. Hartmann in Regensburg entwickelte inverse Monte-Carlo-Planungssystem IKO auf seine Anwendbarkeit zur gezielten Dosiseskalation hin untersucht. Einfache zwei- und dreidimensionale Strukturen in einem Phantom sollen Aufschluss darüber geben, bis zu welcher Strukturgröße IKO scharfe Dosisgradienten realisieren kann. Anhand zweier Prostata-Pläne und eines HNO-Plans wird die Fähigkeit untersucht, Pläne mit mehreren Zielvolumina zu optimieren.
Da die endgültigen Feldgrenzen erst nach der Optimierung durch IKO, während der so genannten Segmentierung festgelegt werden, kann der Dosisberechnungsalgorithmus von IKO die durch die Strahlenfeldbegrenzung verursachten Effekte nicht berücksichtigen. Die Segmentierungssoftware IMFAST(R) (Siemens) nähert die von IKO optimierten Intensitätsverteilungen durch eine Überlagerung bestrahlbarer Felder (Segmente) und zugehöriger Strahlgewichte. Der durch die Näherung unvermeidbare Qualitätsverlust soll durch eine Reoptimierung der Segmentgewichte minimiert werden. Ein entsprechendes Programm wurde entwickelt und anhand eines Prostata-Plans und eines HNO-Plans untersucht. Im Zusammenhang mit der Reoptimierung der Segmentgewichte wurde von Z. Morávek eine Optimierungsbedingung in IKO implementiert die Intensitätsgradienten und -extrema vermeiden soll (Glättung). Diese wurde ebenfalls an einem Prostata- und einem HNO-Plan untersucht.
Ein großer Teil dieser Arbeit beschäftigt sich mit der Einführung von IKO in die klinische Routine. Dazu war es notwendig, die mit IKO erstellten Bestrahlungspläne in ein, im klinischen Einsatz verwendetes, Bestrahlungsplanungssystem zu importieren und dort zu verifizieren. An einem HNO- und einem Lungen-Tumor wird der Ablauf der Bestrahlungsplanung demonstriert.
Als dosimetrische Verifikation hat sich die Filmdosimetrie in einem Phantom bewährt. Bei der Verifikation eines IKO-optimierten Plans wurde ein neuer selbstentwickelnder Filmtyp (Gafchromic(R)) in Verbindung mit einem Durchlichtscanner (Microtek 9800XL mit TMA 1600) getestet und mit der bisherigen Kombination aus Dunkelkammerfilm (Kodak EDR2) und Videodensitometer (Wellhöfer WD700i) verglichen.

Translation of the abstract (English)

Since the discovery of x-rays it's impossible to imagine medicine without noninvasive imaging. New imaging methods like PET (positron emission tomography), SPECT (single photon emission tomography) or magnetic resonance spectroscopy (MRS) provide three dimensional information on biological activities and the chemical composition inside the body. In radiotherapy this biological imaging is used to adapt the dose to the physiology of the tumor cells and therewith to achieve a better tumor control, while sparing the healthy organs as much as possible. These local dose modulations, called dose painting, challenge a treatment planning system to realize an inhomogeneous dose prescription.
In this work, the inverse Monte Carlo treatment planning system IKO, developed at Regensburg by M. Hartmann, is studied with regard to its feasibility to create a systematic dose escalation. Simple two- and three dimensional structures inside a phantom shall give information down to which size of structure IKO is able to realize sharp dose gradients. Using two prostate cases and a head and neck case, the ability to optimize treatment plans with several target volumes is investigated.
As the final apertures are defined by the segmentation process after the optimization with IKO, the dose calculation algorithm of IKO can't take into account those effects caused by the aperture boundary. The segmentation software IMFAST(R) (Siemens) approximates the fluence distribution by superposing realizable fields (segments) and the corresponding segment weights. The loss in quality caused by the approximation shall be minimized by a reoptimization of the segment weights. An appropriate software was developed and evaluated using a prostate plan and a head and neck plan. In combination with the reoptimization of the segment weights, an additional term to the objective function was implemented by Z. Moràvek in order to avoid steep intensity gradients and extrema (smoothing). This was evaluated again by using a prostate case and a head and neck case.
A part of this work is dedicated to the introduction of IKO into clinical routine. Therefore it was necessary to import the treatment plans created by IKO into a clinically used treatment planning system and verify them. This process is demonstrated by using a head and neck case and a lung case.
As a means for dosimetric verification the dosimetry with radiochromic films is established. For the verification of an IKO optimized plan a new self developing type of film (Gafchromic(R)) was tested in combination with a transmission scanner (Microtek 9800XL and TMA 1600) and was compared to the combination of conventional film (Kodak EDR2) and video densitometer (Wellhöfer WD700i).
