Abstract (German)
Die Multiple Sklerose, auch Encephalomyelitis disseminata genannt, ist eine chronisch-entzündliche Erkrankung des Zentralnervensystems und stellt die häufigste neurologische Ursache nicht-traumatischer Invalidität junger Menschen dar. Noch ist ihre Ätiologie nicht geklärt, jedoch sind zweifellos prädisponierende genetische Faktoren und bislang unbekannte Umwelteinflüsse, die auf ein genetisch suszeptibles Individuum einwirken, im Rahmen einer multifaktoriellen Krankheitsentstehung von zentraler Bedeutung. In einer Voruntersuchung an Patienten mit isolierter Optikusneuritis, einer Erkrankung aus dem Formenkreis der MS, konnte gezeigt werden, dass Erkrankungsrisiko und Geburtsmonat statistisch assoziiert sind.
Die vorliegende Arbeit sollte prüfen, ob auch das Geburtenprofil von MS-Patienten an der Neurologischen Universitätsklinik Regensburg einer zirkannualen Periodik unterliegt. In einem zweiten Schritt sollte untersucht werden, ob statistische Zusammenhänge zwischen diesem zirkannual unterschiedlichen Erkrankungsrisiko und der frühkindlichen Ernährung der Betroffenen sowie anderen Umweltfaktoren bestehen. Die vorliegende Untersuchung verglich das jahreszeitliche Geburtenprofil einer Kohorte von 826 MS-Kranken mit Daten des Statistischen Bundesamts zum Geburtszeitpunkt der gesunden Normalbevölkerung. Um eine mögliche Assoziation des Geburtszeitpunktes von MS-Kranken mit Umweltfaktoren zu untersuchen, wurde das Probandenkollektiv zur Risikostratifizierung anhand des jahreszeitlichen Geburtenprofils in eine Hoch- und eine Niedriginzidenzgruppe aufgeteilt. Die Assoziation mit Umweltfaktoren wurde mithilfe eines Fragebogens untersucht. Kernpunkt dieses Teils der Arbeit war der Vergleich der Stillrate und -dauer dieser Subgruppen. Da neben ernährungsbedingten Faktoren auch Infektionen als Kontributoren in der Genese der Multiplen Sklerose und als Ursache des jahreszeitlichen Geburtenprofils denkbar sind, wurden zur Hypothesengenerierung außerdem einige der aktuell diskutierten Erreger berücksichtigt (Varizellen, Kinderkrankheiten in relativ hohem Alter, d. h. nach dem sechsten Lebensjahr und EBV). Außerdem wurden deskriptive Daten zum sozialen Status und der Beschaffenheit der Umgebung während der Kindheit der Probanden erhoben.
Die Stichprobe bestand aus 826 Personen, davon 65,6 % Frauen. Das mittlere Alter lag bei 46,3 Jahren. Die Gesamtrücklaufquote betrug 59,6 %. Ca. 15 % der Studienteilnehmer zeigten eine positive Familienanamnese bzgl. der MS. Die Kohorte ist bzgl. dieser Verteilung als Stichprobe der MS-Population als repräsentativ anzusehen.
Ähnlich wie in bereits publizierten Studien zu Patienten aus anderen geographischen Breiten waren bei unserer Patientenkohorte eine Häufung der normalisierten Geburtenrate im Frühjahr und ein Rückgang derselben in den Herbstmonaten zu verzeichnen. Im Rahmen der Gegenüberstellung der Stillraten der Subpopulationen ließ sich eine eindeutige Assoziation des Geburtszeitpunkts mit der frühkindlichen Ernährung nachweisen. Die Ergebnisse der vorliegenden Studie legen ein häufigeres Auftreten der Multiplen Sklerose unter nicht gestillten Individuen nahe. Darüber hinaus könnte die Tatsache, dass eine signifikante Majorität der Studienteilnehmer im ersten Herbst oder Winter ihres Lebens ohne schützende mütterliche Immunglobuline auskommen musste, auf in diesem Zeitraum wirkende Umweltfaktoren hindeuten.
Bezüglich der infektiologischen Vorgeschichte der Studienteilnehmer zeigte sich, dass eine Erhebung allein auf der Grundlage des verwendeten Fragebogens keine belastbaren Daten liefern kann. Dies gilt vor allem für Infektionen mit wenig charakteristischer klinischer Symptomatik und hoher inappararenter Infektionsrate wie Epstein-Barr-Virus. Diesbezüglich bedarf es weiterer Studien mit angepasstem Design.
Neben der aktuell intensiv diskutierten Rolle von Infektionen als wichtigem Umweltfaktor für die Entstehung der MS liefert die vorliegende Arbeit somit auch Hinweise für ernährungsbedingte Faktoren, die im Kontext mit weiteren möglichen nicht-infektiologischen Risikofaktoren (z. B. Vitamin-D-Metabolismus) diskutiert werden.

BACKGOUND and OBJECTIVES Multiple sclerosis or encephalomyelitis disseminata, is a chronic inflammatory disease of the central nervous system. It is the most frequent neurological cause of non-traumatic invalidity in young adults. While its etiology is not known, an interplay between predisposing genetic factors and yet unknown environmental factors appear to play a central role in the pathogenesis.
In a preceding study of patients with isolated optic neuritis, a disease closely related to MS, we were able to demonstrate an association between the risk of developing optic neuritis and the month of birth. The present study aimed at investigating the annual distribution of birth rates in MS-patients treated at the Department of Neurology, District Hospital, University of Regensburg. In case of a similar circannual variation of the disease risk we intended to examine whether or not this was correlated with infant nutrition and other environmental factors.
METHODS Using data from the Federal Statistical Office of Germany we compared the time of birth of 826 MS patients with corresponding data for the healthy average population. Data concerning environmental factors, in particular, the rate and duration of breastfeeding were collected utilizing a questionnaire. In addition information on the patient’s history of childhood infections was gathered. Furthermore we collected data on social status and the milieu (rural vs. town) during childhood.
RESULTS The sample consisted of 826 MS patients among which 65.6% were female. The average age was 46.3 years. 59.6% of the questionnaires were returned. Approximately 15% of study participants reported family members with MS. The cohort was considered to be a representative sample of the MS population in South Germany. Consistent with earlier studies conducted in different latitudes we found the maximal birthrate (acrophase) in spring and a corresponding minimum (nadir) in autumn. Comparing the spring and autumn subgroups there was a clear association between time of birth and infant nutrition.
DISCUSSION The present study confirms a circannual distribution of birthrates among MS patients and indicates a higher occurrence of multiple sclerosis among non-breastfed individuals. Our data suggest that a significant majority of study participants passed their first winter without protective maternal antibodies provided during breastfeeding and thereby indicate a protective role of breast-feeding (or a disease promoting role of cow milk). Regarding the previous medical history of infections we found that the utilized questionnaire did not provide plausible data. While information on occurrence and timing chicken pox seemed to be rather reliable the rate of other common infections like Epstein-Barr virus was far lower than in other surveys. Further studies with an adapted design are required to address the role of infections in the risk of acquiring MS.
CONCLUSION The present study indicates that timing of birth and the rate and/or duration of breastfeeding are factors shaping the risk of developing MS.
