Abstract (German)

Etablierte Materialien zur Stimmlippenaugmentation sind Kollagen, Hyaluronsäure, Goretex, Silikon und autologes Fett. Nachteile dieser Substanzen umfassen Migration, Diffusion, Allergisierung und Resorption, so dass nur eine temporäre Verbesserung der Dysphonie möglich ist. Ein optimiertes, langzeitstabiles Fibringel steht als Alternative zur Augmentation paretischer oder resezierter Stimmlippen zur Verfügung.

Explantierte porcine Kehlköpfe wurden mit erwärmter und befeuchteter Druckluft insuffliert und die passive Stimmlippenbewegung mittels Hochgeschwindigkeitsglottographie digital dokumentiert. Die Kehlköpfe wurden vor und nach glottischer Injektion von Kollagen (Zyplast), Hyaluronsäure (Surgiderm) oder Fibringel aufgenommen und das Schwingungsverhalten mit einem rechnergestützten System ausgewertet.

In allen untersuchten Parametern konnten durch den Einsatz von Fibringel vergleichbare Ergebnisse erzielt werden. Fibringel stellt durch seine Langzeitstabilität und seine Biokompatibilität eine viel versprechende Alternative zu den konventionellen Augmentationsmaterialien dar.

Als Zellträger für autologe Chondrozyten oder Fibroblasten ermöglicht es die zelluläre Rehabilitation pathologisch veränderter Stimmlippen. Weitere Untersuchungen in vivo werden unternommen.

Translation of the abstract (English)

Collagen, hyaluronic acid, Goretex, silicone and autografted fat are established materials for the augmentation of vocal folds. Among the disadvantages of these substances migration, diffusion, allergizatisation and resorption have to be mentioned, so that in consequence only a temporary improvement of the dysphony can be made possible. An optimized, long-term stable fibrin gel is available as an alternative for the augmentation of parethic or resected vocal folds.

Compressed air was blown into explanted porcine larynxes and the passive movement of the vocal folds was documented digitally with a high-speed glottographe. The larynxes were filmed before and after the injection of collagen (Zyplast), hyaluronic acid (Surgiderm) or fibrin gel and the oscillation was evaluated with a computer-based system.

Similar results could be proven for all tested parameters when fibrin gel was used. Fibrin gel can be seen as a highly promising alternative for conventional means of augmentation. Being a matrix for autografted chondrocytes or fibroplasts, it makes the cellular rehabilitation of pathologically mutated vocal folds possible. There will be further examinations in vivo.
