Bis heute sind die invasiven Mykosen eine schwerwiegende Komplikation im Rahmen einer aggressiven Chemotherapie. Schon bei dem ersten Verdacht auf eine invasive Mykose, wird eine empirische antimykotische Therapie eingeleitet.
Amphotericin B desoxycholate (AmB) war im Zeitraum der Datenerfassung in den Jahren 2000-2002 der Goldstandard der systemischen antimykotischen Therapie. Allerdings musste eine Reihe teils schwerer Nebenwirkungen unter der Therapie in Kauf genommen werden. An erster Stelle ist hier die Nephrotoxizität zu nennen, die am Häufigsten zu einem Wechsel der Therapie geführt hat. Als Reserveantimykotikum stand eine liposomale Amphotericin-Lösung (Ambisome®) zur Verfügung. Diese erhöhte aber die Therapiekosten um ein vielfaches. Daher wurde versucht, durch eine modifiziertes Verabreichungsregime die Verträglichkeit von AmB zu verbessern. Die Infusionszeit wurde von 6 auf 24h verlängert. Die im 6h-Regime übliche prophylaktische Co-Medikation mit Paracetamol, Dimetinden und Pentoxifyllin wurde bei der 24h-Gabe nur bei Bedarf verabreicht.
Anhand der Krankenakten haben wir bei 56 Patienten retrospektiv untersucht, ob diese Therapieregime-Modifizierung zu einer Senkung der Nebenwirkungen geführt hat. Die einbezogenen Patienten wurden im Rahmen der Chemotherapie einer malignen Grunderkrankung im Zeitraum Juli 2000 bis Juni 2002 einer antimykotischen Therapie unterzogen. 20 Patienten nach dem bisherigen Schema einer AmB-Gabe über 6 Stunden und 36 Patienten nach dem modifizierten Schema über 24 Stunden.

Es zeigten sich folgende Ergebnisse:

Bezüglich der Nephrotoxozität zeigt sich zwischen den Gruppen kein signifikanter Unterschied. Die mittlere Kreatininwertanstieg unterscheidet sich mit 0,38μg bei der 6h-Gruppe nicht signifikant von 0,31μg bei der 24h-Gruppe.

In beiden Gruppen sind keine akuten, schweren Nebenwirkungen aufgetreten. Lediglich in der 24h-Gruppe kam es in drei Fällen zu Schüttelfrost. Dies lässt sich durch den Verzicht auf die Paracetamolprophylaxe in dieser Gruppe erklären.

Schlußfolgerung:

Durch die Umstellung des Infusionsregimes von 6 auf 24 Stunden bei der AmB-Gabe lässt sich keine signifikante Reduzierung der Nephrotoxizität und anderer Nebenwirkungen erreichen.

Translation of the abstract (English)

During the period of data collection in the years 2000-2002 Amphotericin B Deoxycholate (AmB) was the ‘gold standard’ of systemic antimycotic therapy. However, a series of toxic adverse effects were known to occur. Leading amongst them was nephrotoxicity, which made a shift of the therapy essential. As a substitute liposomal Amphotericin, a lipid formulation of Amphotericin B (Ambisome), was made available, but the costs of therapy increased manifold. In order to improve the tolerance of AmB, a modified regime of administration was introduced. The time of infusion was extended from 6 to 24h. The usual co-medication of Paracetamol, Dimetinden and Pentoxifyllin, typical for the 6h infusion time, was administered in the 24h regime only on demand.

Patients and Methods:
On the basis of the medical records, we have investigated 56 patients retrospectively to determine if modifying the frequency of infusion regime from 6h to 24h decreases the adverse effects of AmB.
This study was conducted between July 2000 to June 2002 and included patients undergoing chemotherapy for malignancies (mainly leukemia), requiring an antimycotic therapy. The 6h protocol of AmB infusion was administered to 20 patients and the modified 24h protocol to 36 patients.

Results:
Both groups of patients showed no difference in respect to the average age, laboratory tests, daily dose and overall dose of AmB as well as switch to second-line antimycotics and therefore were eligible to compare the infusion protocols.

In respect to nephrotoxicity, there was no significant difference between the two groups.
The mean increase of creatinine with a value of 0.31 mg/dl (creatinine ratio 1.41) in the 6h group did not differ significantly from the 24h group value of 0.38 mg/dl (creatinine ratio 1.48).
In both groups acute severe side effects were not detected, except in the 24h infusion group where three cases of chills occurred. This could be explained by lack of paracetamol prophylaxis.

Conclusion:
Switching the infusion time from 6 to 24h does not significantly reduce the nephrotoxicity and the side effects of AmB.
