Abstract (German)
Die Adhäsion von Bakterien und Zellen auf der Oberfläche von Biomaterialien ist ein Faktor, der den klinischen Erfolg oder Misserfolg dieser Werkstoffe mitbestimmt. Die Oberflächen der in den Organismus eingebrachten Werkstoffe adsorbieren zunächst Proteine aus dem umgebenen Milieu, die nun ihrerseits das Maß der Adhäsion von Bakterien und Zellen modifizieren können. Rauigkeit und Hydrophobizität von Werkstoffoberflächen werden in der Literatur als modifizierende Parameter der Proteinadsorption diskutiert.
Ziel dieser Arbeit war es, die Adsorption von humanem Serumalbumin (HSA) als Modellprotein für Serumproteine und der Proteine des Gesamtspeichels auf klinisch relevanten Biomaterialien zu untersuchen. Außerdem wurde die Menge der adsorbierten Proteine auf die Hydrophibizität der Materialoberflächen bezogen.
Als Materialien wurden die Metalllegierungen Ti6Al4V und Co28Cr6Mo, die Kunststoffe Polytetrafluorethylen, Polyethylen, Polymethylmethacrylat (PMMA), Tetric Evo Ceram, Filtek Siloran und Silikon sowie der Glasionomerzement (Ketac Molar) verwendet. Die Prüfkörper der Werkstoffe hatten einen Durchmesser von 11,3 mm, eine Wandstärke von 2 mm und waren 2 mm hoch. Als Maß der Hydrophobizität der Oberflächen diente die Bestimmung des Netzwinkels gegen Wasser. Als stark hydrophob wurden Polytetrafluorethylen, Silikon und Polyethylen mit Netzwinkeln zwischen 118,5° und 110° eingestuft; Polymethylmethacrylat, Filtek Siloran, Tetric Evo Ceram, Titan und Cobalt-Chrom-Molybdän wurden aufgrund von Netzwinkeln zwischen 73° und 65° als schwach hydrophob bewertet und der Glasionomerzement Ketac Molar hatte mit einem Netzwinkel von 50,5° eine hydrophile Oberfläche.
Die Rauigkeitswerte der manuell und maschinell gefertigten Biomaterialien lagen zwischen Ra = 0,04 µm für das Polymethylmethacrylat und Ra = 0,53 µm für das dentale Komposit Tetric Evo Ceram sowie Polyethylen. Die Unterschiede in der Oberflächenrauigkeit wurden auf die Herstellungsmethoden und die Viskosität der einzelnen Werkstoffe zurückgeführt.
HSA und die Proteine aus Gesamtspeichel auf den kleinen Prüfkörpern wurden über den Nachweis freier Aminogruppen und über eine Biotin-Avidin-gekoppelte Reaktion detektiert. Eichkurven mit HSA und Speichelstandards erlaubten die quantitative Bestimmung adsorbieter Proteinmengen. Die als stark eingestuften hydrophoben Biomaterialien Polytetrafluorethylen und Polyethylen adsorbierten 37- (0.185 µg/cm2) beziehungsweise 12,5-mal so viel Proteine aus dem Gesamtspeichel als HSA. Außerdem adsorbierten Polytetrafluorethylen und Polyethylen deutlich weniger HSA als die weniger hydrophoben Biomaterialien Polymethylmethacrylat, Tetric Evo Ceram, Titan, Cobalt-Chrom-Molybdän und Filtek Siloran. Auch an den Werkstoffen Polymethylmethacrylat, Silikon, Ketac Molar, Titan und Filtek Siloran wurde eine deutlich höhere Speicheladsorption gefunden, wobei hier jedoch die Unterschiede nicht so groß waren wie bei Polytetrafluorethylen und Polyethylen. Nur auf Cobalt-Chrom-Molybdän konnte als einzigem der neun Biomaterialien eine höhere Proteinadsorption von HSA als von Speichel nachgewiesen werden.
Die Korrelatioinsanalysen zwischen der Größe der Netzwinkel und den adsorbierten Proteinmengen auf den unterschiedlichen Materialien zeigten mit HSA für den Bereich hydrophiler Netzwinkel (55-75°) relativ große Unterschiede. Im hydrophoben Bereich der Netzwinkel (100-120°) hingegen adsorbierten vergleichsweise ähnliche Mengen an HSA. Möglicherweise besteht im hydrophoben Bereich ein spezifischer, im hydrophilen Bereich ein unspezifischer Zusammenhang zwischen Netzwinkel und HSA-Proteinkonzentration. Für die Adsorption von Speichelproteinen erkennt man, anders als für HSA, im Bereich hydrophiler Netzwinkel (55-75°) relativ geringe Unterschiede, im hydrophoben Bereich der Netzwinkel (100-120°) waren die Unterschiede der adsorbierten Mengen an Speichelproteinen groß. So könnte diese Korrelationsanalyse einen Hinweis darauf geben, dass Speichelproteine eher auf hydrophilen Oberflächen spezifisch adsorbieren. Für den Parameter Oberflächen-rauigkeit war mit den hier geprüften Materialien kein Zusammenhang zur Proteinadsorption erkennbar.
Die in der Arbeit gefundenen Ergebnisse der Proteinadsorption auf unterschiedlichen Biomaterialien verweisen somit auf einen sehr komplexen Vorgang, der nur mit den Parametern der Hydrophobizität oder Hydrophilie und der Oberflächenrauigkeit nicht vollständig zu erklären ist. Bestimmt spielen auch die funktionellen chemischen Gruppen selbst, die hydrophobe oder hydrophile Oberflächen erzeugen eine Rolle. Daneben sollten Parameter wie Lösungsmittel, pH-Wert, Ionen und Ionenstärke die Adsorption unterschiedlicher Proteine quantitativ und selektiv verändern können.

The adhesion of bacteria and cells to biomaterial surfaces influence their clinical success. After being implanted in the human organism the biomaterial surfaces initially adsorb proteins from the surrounding environment, which are assumed to modify the add-on adhesion of different bacteria and cells. Roughness and hydrophobicity (wettability) of biomaterial surfaces are discussed as modifying parameters of protein adsorption in the current literature.
The aim of this study was to investigate the adsorption of human serum albumin (HSA) as a model protein and of human saliva proteins on clinically relevant biomaterials. Furthermore, the amounts of adsorbed proteins were related to the wettability of the biomaterial surfaces.
Nine different materials were used: the metal alloys Ti6Al4V and Co28Cr6Mo, polytetrafluoroethylene, polyethylene, polymethyl methacrylate, Tetric EvoCeram (dental composite), Filtek Silorane (dental composite), silicone as polymers and a glass ionomer cement (Ketac Molar). Specimens of the biomaterials had a diameter of 11.3 mm and were 2 mm high. Hydrophobicity [wettability] of the different biomaterial surfaces was determined by measuring the contact angle of a specimen surface against water. Polytetrafluoroethylene, silicone and polyethylene caused contact angles between 118.5° and 98.5° (medians) and were classified as highly hydrophobic; polymethyl methacrylate, Filtek Silorane, Tetric EvoCeram, Ti6Al4V and Co28Cr6Mo specimens with angles between 73° and 65° were considered less hydrophobic. The surface of a glass ionomer cement (Ketac Molar) specimen caused a 50.5° contact angle and was classified as hydrophilic.
The roughness of the biomaterial surfaces ranged between Ra = 0,04 µm (polymethyl methacrylate and Ra = 0,53 µm (Tetric EvoCeram and polyethylene). The different surface roughnesses probably originated from the modes of sample preparation and the different viscosities of the various materials. HSA and whole saliva proteins adsorbed to the material surfaces were detected by amino group-labeling (with biotin-avidin) coupled to a chemiluminescence reaction for direct determination of proteins adsorbed on material surfaces.
Standard curves derived from known amounts of HSA and saliva proteins allowed for the quantitative determination of proteins adsorbed to surfaces. Polytetrafluoro-ethylene and polyethylene classified as highly hydrophobic adsorbed about 37 times (0.185 µg/cm2) and 12.5 times more proteins from whole saliva than from HSA solutions. In addition, polytetrafluoroethylene and polyethylene adsorbed significantly less HSA than the less hydrophobic polymethyl methacrylate, Tetric EvoCeram, Ti6Al4V, Co28Cr6Mo and Filtek Silorane specimens. Polymethyl methacrylate, silicone, Ketac Molar, Ti6Al4V and Filtek Silorane also adsorbed distinctly higher amounts of saliva proteins than HSA. Only Co28Cr6Mo adsorbed higher amounts of HSA than of saliva proteins. Correlation analysis between hydrophobicity of the various materials and the amounts of adsorbed protein showed large differences with HSA for hydrophilic contact angles (55-75°). On the other hand similar amounts of HSA adsorbed to material surfaces in the hydrophobic range of contact angles (100-120°). It is possible that a specific binding of HSA is supported by hydrophobic surfaces while adsorption of HSA is unspecific on hydrophilic surfaces. Unlike with HSA, only small differences between the amounts of adsorbed saliva proteins were found in the hydrophilic range of material surfaces but big differences were detected on hydrophobic surfaces. Thus, these correlation analyses might indicate that saliva proteins preferably adsorb more specific to hydrophilic surfaces. No positive correlation was found between the roughness of material surfaces and the amounts of adsorbed proteins. The results of the protein adsorption to different biomaterials found in this study indicate a very complex process that cannot be fully explained by surface hydrophobicity and surface roughness. Functional chemical groups that create hydrophobic and hydrophilic surfaces might play a important role for protein adsorption as well. Besides these groups other parameters like solvent, pH value, ions and ionic strength should modify the adsorption of different proteins to surfaces quantitatively and selectively.
