Abstract (German)

Mit der vorliegenden Arbeit soll untersucht werden, in welchen klinischen Situationen heutzutage eine CMV-Erkrankung bei HIV-infizierten Patienten in Deutschland auftritt, wie die Langzeitprognose dieser Patienten ist und welche prognostische Bedeutung unterschiedliche Faktoren für die Schätzung der Überlebenszeit bei HIV-infizierten Patienten nach CMV-Retinitis bzw. CMV-Gastroenteritis haben. ...

Mit der vorliegenden Arbeit soll untersucht werden, in welchen klinischen Situationen heutzutage eine CMV-Erkrankung bei HIV-infizierten Patienten in Deutschland auftritt, wie die Langzeitprognose dieser Patienten ist und welche prognostische Bedeutung unterschiedliche Faktoren für die Schätzung der Überlebenszeit bei HIV-infizierten Patienten nach CMV-Retinitis bzw. CMV-Gastroenteritis haben. Diese Fragestellung wurde an einem Kollektiv von 170 Patienten der ClinSurv-Datenbank des Robert-Koch-Instituts untersucht.
Bei unserer retrospektiven Arbeit wurden folgende Parameter erhoben: Alter, Geschlecht, wahrscheinlicher Infektionsweg, Angaben zum Beginn des AIDS-Stadiums, Immunstatus, Angaben zu HCMV-Manifestationen und Angaben zur antiretroviralen Therapie.
Nachfolgend wurde eine statistische Analyse der Überlebenszeit mittels des Kaplan-Meier-Verfahrens und des Cox-Modells(Proportional-Hazard-Model) durchgeführt. Das Ziel der Arbeit war dabei, diejenigen Parameter nach und nach herauszuarbeiten, bei denen ein statistisch signifikanter Zusammenhang mit der Überlebenswahrscheinlichkeit besteht.
Die Zwei- bzw. Fünfjahres-Überlebensrate im Gesamtkollektiv betrug 79 bzw. 70%. Der Median der Überlebenszeit wurde im Kollektiv nicht erreicht, da ein Großteil der Patienten im Beobachtungszeitraum überlebte.
Die Parameter CD4-Zellzahl im Verlauf und Viruslastverlauf kristallisierten sich nach den einzelnen Analysen als signifikante Einflussgrößen hinsichtlich der Überlebenswahrscheinlichkeit heraus. Im multivariaten Cox-Modell konnte dies eindeutig bestätigt werden. Der Supprimierung der Viruslast im Verlauf kam dabei die größte Bedeutung zu. Im multivariaten Modell zeigte sich auch, dass das Alter - für diesen Faktor ließ sich ja weder in der Kaplan-Meier-Analyse noch in der monovariaten Cox-Analyse eine Signifikanz erkennen - im Zusammenspiel mit den anderen Faktoren ebenfalls signifikanten Einfluss auf die Schätzung der Überlebenswahrscheinlichkeit hat.

This study examines the clinical circumstances in which CMV disease currently occurs in patients infected with HIV in Germany, the long-term prognosis of these patients and the prognostic significance of diverse factors for assessing the survival time of HIV infected patients following CMV retinitis or CMV gastroenteritis. This study examines the clinical circumstances in which CMV disease currently occurs in patients infected with HIV in Germany, the long-term prognosis of these patients and the prognostic significance of diverse factors for assessing the survival time of HIV infected patients following CMV retinitis or CMV gastroenteritis. These issues were examined using the data of 170 patients via the ClinSurv database of the Robert Koch Institute.
In our retrospective study, the following parameters were set: age, gender, probable route of infection, details on the beginning of the AIDS stage, immune status, details on HCMV manifestations and details of anti-retroviral treatment.
A statistical analysis of the survival times was then carried out using the Kaplan-Meier procedure and the Cox model (proportional hazard model). The aim of the study was gradually to work out those parameters which have a statistically significant correlation to the probable survival time.
The two or five year survival rate within the entire group amounted to 79 or 70%. The median survival time of the group was not achieved because the majority of the patients survived the surveillance period.
Following individual analyses the parameters CD4 cell count curve and viral load curve crystallized as significant magnitudes of influence regarding the probable survival time. This was decisively confirmed in the multivariate Cox model. Of most significance was suppression of the viral load curve. In the multivariate model it was also clear that age - a significance could not be identified for this factor either in the Kaplan-Meier analysis or in the monovariate Cox analysis - in collaboration with the other factors also has a significant influence on the assessment of probable survival time.
