Abstract (German)

In dieser Arbeit wurde die Urinausscheidung von 12-HETE, 15-HETE, Leukotrien B4 (LTB4), Leukotrien E4 (LTE4), Prostaglandin E Metaboliten (PGEM) und 6-keto-Prostaglandin F1-alpha (PGF1a) jeweils 14 Tage und 3 Monate nach erfolgter Nierentransplantation mittels Enzym-Immunoassays gemessen. Abstoßungsgruppe: n=17, Kontrollgruppe: n=17.

Bezüglich der Ausscheidung von 12-HETE, LTB4, LTE4 und PGF1a konnte kein Zusammenhang mit einer Abstoßungsreaktion ermittelt werden. Diese Marker kommen daher für die Diagnostik einer Abstoßungsreaktion nicht in Frage. Es zeigten sich etwas geringere Konzentrationen von 15-HETE während einer Abstoßung verglichen mit den Werten vor einer Abstoßung. Bei Patienten mit initialer Abstoßung kam es entsprechend zu einer leichten Erhöhung der Werte nach Abstoßung. Weitere Untersuchungen können zeigen, ob die 15-HETE-Konzentrationen im zeitlichen Verlauf zur Erkennung einer Abstoßung geeignet sind. Die Ausscheidung von PGEM war bei Patienten mit Abstoßung 14 Tage nach Transplantation gegenüber der Kontrollgruppe hochsignifikant (p=0,005) erhöht. Auch Patienten die erst im späteren Verlauf eine Abstoßung entwickelten, zeigten nach 14 Tagen signifikant erhöhte Konzentrationen von PGEM. Diesen Ergebnissen zufolge dürfte die Urinausscheidung von PGEM eine geeigneter Marker zur Diagnostik einer akuten Abstoßungsreaktion sein.

Translation of the abstract (English)

The urinary excretion of 12-HETE, 15-HETE, leucotriene B4 (LTB4), leucotriene E4 (LTE4), prostaglandin E metabolites (PGEM) and 6-keto-prostaglandin F1-alpha (PGF1a) was investigated 14 days and 3 months after kidney transplantation using enzyme immunoassays. Rejection group: n=17, Control group: n=17.

The excretion of 12-HETE, LTB4, LTE4 and PGF1a was not influenced by acute rejection. Therefore these markers can not be used as diagnostic tool for the detection of acute rejection. 15-HETE concentrations were slightly lower during rejection compared to the time before rejection. In patients with early rejection 15-HETE concentrations increased slightly afterwards. Further studies will show whether rejection events can be predicted by monitoring the time evolution of the 15-HETE concentration. The excretion of PGEM showed a highly significant (p=0,005) elevation in patients with rejection 14 days after transplantation compared to the control group. Patients with later rejection also had significantly (p<0,05) higher levels of PGEM after 14 days. These results show that the urinary excretion of PGEM could be an appropriate diagnostic tool to detect an early acute rejection.
