Stroma-targeted palliative tumor therapy with biomodulators

Abstract

In search of new strategies for the therapy of advanced tumors stroma-targeted approaches have been discussed recently, especially antiangiogenic therapies. It has turned out that some biomodulating drugs exhibit also effects against tumors beyond their original non-oncologic indication. Among these drugs are for example thalidomide,COX-2 inhibitors, PPARγ agonists (thiazolidindiones) and mTOR antagonists (rapamycin).The antitumor efficacy of these therapeutics is fundamentally based on indirect effects on the stroma of the tumors. Combination therapies of these therapeutics seem to be superior to the use of single agents. These new biomodulating therapy approaches represent an interesting option for the palliative treatment of advanced tumors, especially as a supplement or extension of established chemo- and immunotherapeutic therapies, since they can be performed in an outpatient setting, the observed side effects are usually mild, and the therapies are therefore well accepted by the patients.

Auf der Suche nach neuen Strategien zur Therapie von Tumoren in fortgeschrittenen Stadien wurden in jüngster Zeit vermehrt Stroma-orientierte Ansätze diskutiert, insbesondere antiangiogenetische Therapien. Es hat sich gezeigt, dass einige biomodulierende Medikamente über ihre ursprüngliche nichtonkologische Indikation hinaus
auch antitumoral wirksam sind. Zu diesen Medikamenten gehören z. B. Thalidomid, COX-2 Inhibitoren, PPARγ Agonisten (Thiazolidindione) und mTOR Antagonisten
(Rapamycin). Ein wesentlicher Teil der antitumoralen Wirksamkeit dieser Therapeutika beruht dabei auf indirekten Effekten auf das Tumorstroma. Dabei scheinen Kombinationstherapien dieser Pharmaka den Monotherapien überlegen zu sein. Aufgrund der ambulanten Durchführbarkeit dieser Therapien, der meist geringen Nebenwirkungen und der dadurch guten Akzeptanz bei den Patienten stellen diese neuen biomodulierenden Therapieansätze eine interessante Option in der palliativen Therapie fortgeschrittener Tumoren dar, insbesondere auch als Ergänzung und Erweiterung
der klassischen chemo- und immuntherapeutischen Möglichkeiten.
