Hintergrund:
Tuberculum majus Frakturen sind seltene Verletzungen und treten entweder isoliert oder in Kombination mit einer vorderen Schulterluxation auf. In der Literatur gibt es nur wenige Langzeitergebnisse zu den funktionellen Ergebnissen nach operativer und konservativer Behandlung von Tuberculum majus Frakturen.
Material:
Über eine prospektiv gesammelte Datenbank wurden retrospektiv die funktionellen und radiologischen Ergebnisse, sowie die Begleitverletzungen von 72 Patienten erhoben. Davon wurden 24 Patienten operativ versorgt und mit den Ergebnissen von 48 konservativ behandelten Patienten verglichen. Die Nachuntersuchung lag nach Trauma durchschnittlich 9,3 Jahre (Zeitspanne 0-15,3 Jahre) zurück. Die funktionellen Ergebnisse wurden mittels drei Schulter Scores bestimmt: Constant Score (CS), DASH-Score (Disability of the Arm, Shoulder and Hand und dem ASES-Score (American Shoulder and Elbow Surgeons Score). Die radiologischen Ergebnisse wurden anhand von Röntgenbilder in zwei Ebenen (anteroposterior und axillar) beurteilt. Die Patienten wurden entweder durch offene Reposition und Interner Fixation (n=12) oder durch geschlossene Reposition und perkutaner interner Fixation versorgt (n=12).
Ergebnisse:
Zum Zeitpunkt der Nachuntersuchung betrug der mittlere Constant Score der operativ behandelten Patienten 79,21 CS Punkte. Insgesamt erreichten 62,5 % der 24 operativ behandelten Patienten ein sehr gutes funktionelles Ergebnis im Constant Score. Im Constant-Score erzielten 62,9 % der 48 konservativ behandelten Patienten ein sehr gutes oder gutes funktionelles Ergebnis. Es fanden sich Unterschiede im funktionellen Resultat, -sowohl im Constant- als auch im DASH- und ASES-Score- in Abhängigkeit des Frakturtyps und des Patientenalters. Jüngere Patienten erzielten einen besseren funktionellen Score-Wert als ältere Patienten. In Abhängigkeit des Geschlechts konnte in beiden Therapiegruppen kein Unterschied nachgewiesen werden. Operierte Patienten zeigten mit steigender Fragmentzahl, höherem Dislokationsgrad sowie mit Luxationsfrakturen zunehmend schlechtere Ergebnisse. Die Zunahme der Fragment-dicke und der Fragmentlänge hatte in beiden Gruppen einen positiven Einfluss auf das Ergebnis der operierten Patienten.
Der mittlere Constant Score der konservativ Behandelten betrug 86,92 Punkte, der durchschnittliche DASH-Score lag bei 12,42 Punkten und der durchschnittliche ASES-Score bei 83,40 Punkten. Insgesamt erreichten die 72 Patienten, unabhängig von Art der Therapie, ein gutes funktionelles Ergebnis mit 84,26 Constant Score Punkten. Die 45 Nichtluxierer hatten ein gutes Outcome mit 87,78 Constant Punkten, die 27 Luxierer mit 78,63 CS Punkten ein befriedigendes Ergebnis.
Die Gruppe der geschlossenen Reposition mit interner perkutaner Schraubenfixation erhielt mit 74,58 CS-Punkten ein befriedigendes Ergebnis. Mit 83,83 CS-Punkten und somit mit „gut“ schnitt dagegen die Gruppe „Nahtosteosynthese“ mit offener Reposition und Interner Fixation ab. Bei Nachweis einer Läsion der Rotatorenmanschette wurde diese mitbehandelt. Im Vergleich beider operativer Verfahren schnitt somit die offene Technik mit Nahtosteosynthese deutlich, aber nicht signifikant, besser ab.
Das funktionelle Ergebnis war somit stark vom Auftreten einer Begleitverletzung nach stattgefundener Luxation geprägt.
Zusammenfassung:
Grundsätzlich empfehlen wir das operative Vorgehen bei Tuberculum majus Frakturen mit Luxation und eindeutigem Nachweis einer Begleitverletzung. Ferner ist in jedem Fall eine anatomische Rekonstruktion des Tuberculum majus sowie der Rotatorenmanschette anzustreben. Somit kann eine frühe passive Beübung durch die Physiotherapie angestrebt werden. Die Vermeidung von Sehnenretraktionen, Pseudarthrosen oder sekundären Dislokationen liefert somit vorhersehbare und gute Langzeitergebnisse.

Background:
Fractures of the greater tuberosity are rare and occur either isolated or in combination with anterior luxation. The literature gives only few long term data of functional results after operative and nonoperative treatment.
Material:
From a prospectively gathered database, we retrospectively analyzed functional and radiographic results as well as collateral injuries of 72 patients. Of these 24 patients were treated operatively . Those results were compared with the functional and radiographic outcome of 48 patients who had been treated nonoperatively. The follow up date back at an average time of 9.3 years (range 0-15.3 years). Functional results were defined by three supplementary shoulder scores: Constant Score (CS), DASH-Score (Disability of the Arm, Shoulder and Hand and the ASES-Score (American Shoulder and Elbow Surgeons Score). Radiographic results were assessed based on accurate radiographics in two planes (anteropsterior and axillary). Patients underwent either open reduction and internal fixation (n=12) or closed reduction and percutaneous internal fixation (n=12).
Results:
At the time of follow up the average Constant Score of the operatively treated patients was 79,21 CS points. 62,5 % of the patients after operative treatment achieved excellent results. 62,9 % of 48 nonoperatively treated patients reached excellent or good results. Depending on the fracture type and age of patient differences in the functional results were detected both in Constant-, DASH and ASES – Score. Young patients achieved better results in Constant Score than older patients. Depending gender no differences were proven in both groups of treatment. Patients after operative treatment exposed poorer results by increasing mount of fragments, higher degree of displacement and subsequent anterior luxation. In both groups after operative treatment enlargement of thickness and length of the greater tuberosity had positive bearing on outcome.
Mean Constant Score of nonoperative treatment was 86.92, mean DASH Score was 12.42 and mean ASES Score was 83.40 points.
Overall 72 patients achieved good functional results with 84.26 points in Constant Score independent which treatment was chosen. 45 patients without luxation reached a good outcome with 87.78 Constant Score points, 27 patients after luxation got 78.63 points in CS and a satisfactory result.
The surgical group with closed reduction and percutaneous internal fixation scored with 74.58 points in CS only a satisfactory result. A good result with 83.83 CS points achieved the group with open reduction and internal fixation. In comparison of both treatments open technique performed better, but not significantly better (p>0,05). The functional outcome was characterized by arising collateral injuries after anterior luxation.
Conclusion:
Principally we recommend operative treatment on fractures of the greater tuberosity after luxation with clear evidence of collateral injuries. Further an anatomical reconstruction of the greater tuberosity as well as the rotator cuff is aspired. Early passive motion by physiotherapy must be sought. Avoiding retraction of the tendon, malunions and loss of reduction provides foreseeable and good long term results.
