Parvovirus B19 (B19V) ist ausschließlich humanpathogen und die Ursache der Kinderkrankheit Ringelröteln (Erythema infectiosum). Die Infektion und Vermehrung von B19V ist auf CD36+ erythroide Vorläuferzellen beschränkt, deren Zerstörung zur Ausbildung einer Anämie, bis hin zur aplastischen Krise, führen kann. Bei akuten Infektionen in der Schwangerschaft ist der diaplazentare Übertritt des Virus auf den Fötus mit Auslösung des anämiebedingten Hydrops fetalis möglich. Im Rahmen der vorliegenden Arbeit wurde ein Zellkultursystem etabliert, mit dem die Replikation von Parvovirus B19 und Messung der Menge infektiöser B19V-Partikel möglich war. Aus peripheren, humanen CD34+ hämatopoetischen Stammzellen wurden die CD36+ erythroiden Vorläuferzellen gewonnen, mit B19V infiziert und die Infektionsraten durchflusszytometrisch mit einem fluoreszenzfarbstoffgekoppelten, monoklonalen Antikörper (MAb) gemessen, der spezifisch gegen das intrazelluläre, virale NS1-Protein gerichtet war. Mit diesem Infektions- und Nachweissystem konnten die vorhandenen, aber unterschiedlich ausgeprägten, neutralisierenden Eigenschaften der VP2- und VP1-spezifischen MAbs und ihrer Fab-Fragmente gezeigt werden. Der VP2-spezifische MAb und seine Fab-Fragmente waren stärker inhibierend als die VP1-spezifischen Immunglobuline. Dieses weist darauf hin, dass als Neutralisationsmechanismus die Blockade bzw. Beeinflussung wichtiger Virus-Zellrezeptor-Strukturen vorlag. Weiterhin wurden die neutralisierenden Eigenschaften der B19V-spezifischen Antikörper in Seren von gesunden Probanden mit abgelaufener B19V-Infektion und von chronisch-virämischen Patienten gemessen. Der Bezug zu erhobenen Laborwerten (IgG/IgM-Antikörperstatus, Virämie) und den klinischen Verläufen zeigte die erweiterten Möglichkeiten dieses Infektionssystems. Es konnte die Neutralisationskapazität der Immunglobuline in humanen Seren direkt darstellen und deckte wesentliche Unterschiede in der Beurteilung des, auf herkömmlichen Labormethoden (ELISA, Immunblot) basierenden, Antikörperstatus auf. Zudem ließ sich kein kontinuierlicher Neutralisationseffekt B19V-spezifischer, IgM-haltiger Seren nachweisen, was auf eine eingeschränkte IgM-Neutralisationskapazität hindeutete.

Parvovirus B19 (B19V) is only pathogenic in humans and the causative agent of the childhood disease Erythema infectiosum. The infection and replication of B19V is restricted to CD36+ erythroid progenitor cells. Their destruction leads to the development of anemia and to aplastic crisis in some case. In acute infections during pregnancy the transplacental transmission of the virus to the fetus may be associated with the development of anemia-related hydrops fetalis. In the present work a cell culture system was established, enabling the replication of parvovirus B19 and measurement of the amount of infectious B19V-particles. From peripheral, human CD34+ hematopoietic stem cells CD36+ erythroid progenitor cells were differentiated, infected with B19V, and the infection rates were measured flow cytometrically using fluorescence-labelled, monoclonal antibodies (MAbs) specifically directed against the intracellular, viral NS1-protein. Based on this infection and detection system the existing, but differently pronounced, neutralizing activities of the VP2- and VP1-specific MAbs and their Fab-fragments could be shown. VP2-specific MAbs and the corresponding Fab-fragments displayed a more pronounced inhibitory activity as compared to the VP1-specific immunoglobulins. This indicates that the mechanism of neutralization was the blockade or influence of crucial virus-cell receptor structures. Furthermore, the neutralizing activities of B19V-specific antibodies in sera of healthy individuals after B19V-infection and of chronic-viremic patients were measured. The correlation to obtained laboratory parameters (e.g., IgG/IgM-antibody status, viremia) and clinical courses showed the extended possibilities of this infection system. It could directly display the neutralizing capacity of immunoglobulins in human sera and revealed substantial differences in the evaluation of the antibody status which based on conventional laboratory methods (e.g., ELISA, immunoblot). Besides, no continuous neutralizing effect of B19V-specific, IgM-containing sera could be demonstrated indicating a limited IgM-neutralizing activity.
