Abstract (German)
Das Hepatozelluläre Karzinom (HCC) ist weltweit der fünfthäufigste Tumor. In Deutschland liegt die Inzidenz bei 4,6/100 000 pro Jahr. Das HCC wird häufig erst spät erkannt, da es lange asymptomatisch bleibt. Je früher es entdeckt wird, desto besser ist die Langzeitprognose. Die Lebertransplantation (LTx) oder eine Resektion stellen dabei die besten Therapieverfahren dar. Eine Leberzirrhose und chronische Virushepatitiden gelten als Hauptrisikofaktor für die Entstehung eines HCCs. Deshalb sollen gemäß den Richtlinien der European Association for the Study of the Liver (EASL) betroffene Patienten alle 6 Monate mittels Abdomensonographie und Alphafetoprotein-(AFP)-Bestimmung überwacht werden. Trotz dieses Surveillance-Programms werden viele HCC-Erkrankungen jedoch zu spät erkannt.
Ziel der vorliegenden, retrospektiven Studie war es, herauszufinden, durch welche Symptome oder Faktoren es zur Erstdiagnose des HCCs gekommen ist. Insgesamt wurden Daten von 308 Patienten, die in den Jahren 1993 bis 2011 mit der Diagnose eines HCCs am Universitätsklinikum Regensburg in Behandlung waren, ausgewertet. Des Weiteren wurden die Einflüsse der therapeutischen, patientenspezifischen und klinischen Parameter auf das Überleben analysiert. Daraus sollte abgeleitet werden, welche Personen besonders gefährdet sind, ein HCC zu entwickeln und um welche Diagnoseparameter das bisher durchgeführte Surveillance-Programm sinnvoll erweitert werden könnte.
Das vorwiegend aus dem Süddeutschen Raum stammende Kollektiv unserer repräsentativen Studie setzte sich aus 262 Männern (85,6%) und 46 Frauen (14,4%) zusammen. Die jüngste Patientin war zum Zeitpunkt der Erstdiagnose 25 Jahre alt, der älteste Patient 86 Jahre. 89,6% der Erstdiagnosen wurden ab einem Alter von ≥51LJ gestellt. Der Altersmedian bei Erstdiagnose lag bei 64 Jahren.
Bezüglich der Erstdiagnose (ED) konnte in unserer Studie erstmals aufgezeigt werden, dass lediglich 20,8% der HCC-Erkrankungen durch das bisher gängige Screening erkannt werden. Die anderen 79,2% der HCC-Patienten wurden entweder als Zufallsbefund im Rahmen anderer Beschwerden (21,4%), im Zusammenhang mit einer dekompensierten Leberzirrhose (11,7%) oder bei Verschlechterung des Allgemeinzustandes (32,5%) entdeckt. Bei einem kleineren Teil der Patienten wurde das HCC als inzidentielles HCC im Explantat bei Lebertransplantation (2,3%) oder als extrahepatische Manifestation (0,9%) diagnostiziert. Bei 10,4% Patienten blieb die Art der ED unbekannt.
Um weitere Hinweise darauf zu bekommen, wie ein optimales HCC-Screening aussehen sollte, wurde nach negativ prädiktiven Faktoren gesucht. Folgende negative prädiktive Werte bezüglich des kumulativen Überlebens konnten ausgemacht werden: Tumoranzahl >2, Tumordurchmesser >5cm, Vorhandensein von Aszites und Pfortaderthrombose, Alkalische Phosphatase und Gesamtbilirubin >2facher Norm, Alphafetoprotein >Norm, sowie eine hohe Stadienzugehörigkeit (>Child A, >Okuda I, >CLIP 2, >BCLC A). Diese Ergebnisse stehen im Einklang mit früheren Publikationen.
Betrachtet man die Art der ED und der Primärtherapie zeigten diejenigen Patienten ein signifikant besseres kumulatives Überleben (p<0,05), deren Erkrankung als Zufallsbefund oder als inzidentelles HCC im Explantat im Rahmen der LTx entdeckt worden waren. Hinsichtlich der Primärtherapie hatten die Patienten die beste Prognose, die mittels LTx oder Perkutaner-Ethanol-Injektion therapiert worden waren.
Kein signifikant (p>0,05) besseres kumulatives Überleben ergab sich bezüglich Geschlecht, Alter und der Genese der Leberzirrhose (z.B. Virushepatitis oder Alkoholabusus).
In unserer Studie konnte kein signifikant schlechteres Überleben (p>0,05) für Patienten mit niedrigen Quick-Werten (<70%), sowie mit Leberzirrhose festgestellt werden. Dies weicht von anderen Publikationen ab, kann jedoch damit in Zusammenhang gebracht werde, dass 46,8% unserer Patienten eine prognostisch günstigere Leberzirrhose Child A hatten.
Schlussfolgernd lässt sich aus unseren Daten ableiten, dass nur 20,8% der HCC-Patienten im Rahmen des Surveillance-Programms diagnostiziert werden. Anhand unserer Daten wäre eine Abdomensonographie ab dem 50. Lebensjahr unabhängig vom Geschlecht sinnvoll. Zusätzlich könnte eine Bestimmung der Serummarker Alkalische Phosphatase und Gesamtbilirubin hilfreich sein. In einer prospektiven Studie sollten die von uns vorgeschlagenen HCC-Screening-Parameter weiter evaluiert werden.

Hepatocellular carcinoma (HCC) is the fifth most common cancer worldwide. In Germany the incidence is about 4,6/100 000 each year. Frequently the HCC is diagnosed late, due to the fact that it is asymptomatic for a long time. The earlier it’s diagnosed the better is the long-range prognosis. Liver-transplantation (LTx) or resection represent the best therapies. Liver cirrhosis and chronic virus hepatitis are classified as the main risk factors for HCC. Corresponding to the guidelines of the European Association for the Study of the Liver (EASL) affected patients should be monitored with sonography of the abdomen and alpha-fetoprotein (AFP) determination every half year. Despite this surveillance a lot of HCCs get diagnosed too late.
The aim of this retrospective study was to determine symptoms and factors which led to the diagnosis of HCC. Overall data of 308 patients with the diagnoses of HCC got evaluated, who have been treated at the Universitätsklinikum Regensburg between 1993 and 2011. The influence of therapeutic, patient-specific and clinical parameters on the overall-survival got analysed. The aim was to derive from this data which patients are at high risk to develop a HCC and which diagnostic parameters could be used to extend the current surveillance program reasonable.
The collective of our representative study consisted of 262 male (85,6%) and 46 female (14,4%) patients, who mainly originated from the Southern part of Germany. The youngest (female) patient was 25 years, the oldest (male) patient 86 years old at the time of diagnosis. 89,6% of the HCCs have been diagnosed in the age of ≥51years. The median at the time of diagnosis was 64years.
Our study demonstrated for the first time that only 20,8% of the HCCs got diagnosed with the help of the current screening program. 79,2% of the HCCs got diagnosed per incidence in the context of other disorders (21,4%), related to decompensated liver cirrhoses (11,7%) or due to a decline of the general condition (32,5%). Only a small amount of patients got diagnosed per incidence in the histology of the explants after liver-transplantation (2,3%) or due to an extrahepatic manifestation of the HCC (0,9%). The kind of the first diagnose of 10,4% of the patients staid unknown.
To get further information how an optimal screening program could be designed, we searched for further negative predictive values. The following negative predictive values respective the cumulative overall survival could be determined: quantity of tumors >2, diameter of the tumor >5cm, presence of ascites or portal vein thrombosis, alkaline phosphatase or total bilirubin risen above two times of the standard value, alpha-fetoprotein risen above the standard value and a affiliation to a high score (>Child A, >Okuda I, >CLIP 2, >BCLC A). These results are consistent with previous publications.
Considering the type of the first diagnosis patients whose HCCs have been determined by incidence in the explants after LTx had a significant better overall survival (p<0,05). Regarding the first therapy patients who have been treated by LTx or percutaneous ethanol injection had the best prognosis.
No significant better cumulative survival (p>0,05) arises from sex, age or genesis of the liver-cirrhosis (e.g. virus hepatitis or alcohol abuse).
With our study we were not able to determine a significantly worse survival (p>0,05) of patients with low Quick (<70%) or with liver cirrhosis. These results differ from those of other publications and can be associated with the fact that 46,8% of the patients of our study suffered from prognostically positive CHILD A liver cirrhosis.
In summary from our data can be derived that only 20,8% of the patients suffering from HCC get diagnosed in the context of the surveillance program. According to our results a sonography of the abdomen is wise independent from the sex starting at the age of 50. In addition the determination of alkaline phosphatase and total bilirubin in the serum could be helpful. Our recommended HCC screening parameters should be evaluated in a prospective study.
