Abstract (German)
Die vorliegende Arbeit soll zum besseren Verständnis der molekularen Ursachen und der Genregulation bei rheumatoider Arthritis und Osteoarthritis beitragen. Bei beiden Erkrankungen kommt es zur Zerstörung des Gelenkknorpels und dadurch zu einer funktionellen Einschränkung des Gelenks. Dabei spielen aktivierte Synovialfibroblasten (SF) eine entscheidende Rolle. Diese Zellen wandern in den Knorpel ein und tragen durch die verstärkte Expression von Matrix-degradierenden Enzymen zum Abbau des Knorpels bei. In der Literatur wurde bereits beschrieben, dass aktivierte SF von Patienten mit rheumatoider Arthritis (RASF) im Vergleich mit SF von gesunden Spendern eine höhere Expression von Rezeptoren aufweisen, die ursprünglich auf Neuronen während der Entwicklung des zentralen Nervensystems entdeckt wurden. Dazu gehören unc-5 homolog B und C (Unc5B und Unc5C), sowie ein Rezeptor der Roundabout, Axon Guidance Receptor Rezeptorfamilie, Robo3. Durch Bindung des Liganden Slit3 an den Robo3-Rezeptor werden RASF inhibiert und migrieren weniger. Neben diesen Rezeptoren wird auch ein aus dem Nervensystem bekanntes Cadherin, N Cadherin, in aktivierten SF gegenüber SF von gesunden Spendern verstärkt exprimiert. N Cadherin kann mit Robo3 interagieren und trägt damit ebenfalls zum aggressiven Phänotyp von RASF und SF von Patienten mit Osteoarthritis (OASF) bei.
Bisher war nicht bekannt, welche der Robo3-Isoformen in RASF exprimiert werden und welche Transkriptionsfaktoren an der Signaltransduktion nach Slit3-Bindung beteiligt sind. Deshalb war das Ziel des ersten Projektteils die Identifikation der exprimierten Robo3-Variante und der beteiligten Transkriptionsfaktoren. Im zweiten Projektteil wurde die Regulation der Expression der überexprimierten Rezeptoren durch micro RNAs (miRs) untersucht. Dabei lag der Fokus auf miRs, die in RASF weniger exprimiert werden als in SF von gesunden Spendern, was eine stärkere Expression ihrer Zielgene ermöglicht. Ziel des zweiten Projektteils war somit die Identifikation einer in RASF fehlregulierten miR sowie die Aufklärung ihrer Rolle für die Expression von Unc5B, Unc5C und Robo3. Im dritten Projektteil wurde die Bedeutung von N Cadherin für die Entstehung und Progression von Osteoarthritis untersucht. Ziel dabei war die Assoziation von SNPs mit der Erkrankung. Außerdem wurde nach einem Transkriptionsfaktor gesucht, der die Expression von N Cadherin aufgrund des gefundenen SNPs beeinflussen kann.
In der vorliegenden Arbeit konnte für rheumatoide Arthritis im ersten Projektteil gezeigt werden, dass RASF die Slit-bindende Robo3B Isoform exprimieren und der Transkriptionsfaktor AP 1 in das Robo3-Slit3 signalling involviert ist. Durch die Beteiligung von AP 1 an der Signaltransduktion lässt sich aufgrund der bisherigen Literaturdaten schließen, dass Rho-GTPasen das Signal nach Slit3-Bindung an den Robo3-Rezeptor intrazellulär weiterleiten und dadurch die Aktin-Polymerisation beeinflusst wird. Außerdem wurde im zweiten Projektteil gezeigt, dass hsa-miR-188-5p in RASF in frühen Passagen weniger exprimiert wird als in RASF in späten Passagen. Durch diese fehlregulierte miR konnte ein ebenfalls fehlreguliertes Zielgen, KIAA1199, identifiziert werden. Da KIAA1199 ein Hyaluronsäure-depolymerisierendes Enzym ist und bereits als in RASF fehlreguliert beschrieben wurde, stellt hsa-miR-188-5p eine Möglichkeit der Regulation dieses Proteins dar. Im dritten Teil der Arbeit konnte gezeigt werden, dass das seltene Allel von rs11564299 bei Osteoarthritis protektiv ist und mit einer erhöhten N Cadherin Expression in OASF korreliert. Damit konnte zum ersten Mal eine Beteiligung von N Cadherin an der Ausprägung von Osteoarthritis gezeigt werden. Zudem konnte ein Transkriptionsfaktor, hnRNP K, identifiziert werden, der an das seltene Allel von rs11564299 bindet und somit zur erhöhten N Cadherin Expression beitragen kann.
Die Ergebnisse dieser Arbeit liefern erste Hinweise für neue Therapie-Optionen und Prognose-Möglichkeiten von rheumatoider Arthritis und Osteoarthritis. Da RASF Robo3B stärker exprimieren als normale SF, bietet dieser Rezeptor und dessen Inhibition durch Slit einen Mechanismus für die Therapie von rheumatoider Arthritis. Ebenso könnte die Regulation von KIAA1199 durch hsa-miR-188-5p in Zukunft für die Inhibition der Aggressivität von RASF genutzt werden. Rs11564299 stellt zwar keine Therapie-Option für Osteoarthritis dar, könnte jedoch dazu beitragen, das Risiko, Osteoarthritis zu entwickeln, genauer zu beurteilen.

The goal of this work was to elucidate molecular reasons and genetic regulation mechanisms in rheumatoid arthritis and osteoarthritis for a better understanding of the diseases. Both diseases have in common that patients suffer from cartilage loss in synovial joints, leading to pain and joint stiffness. Contributors to cartilage degradation are synovial fibroblasts. These cells are activated in rheumatoid arthritis and osteoarthritis. The aggressive synovial fibroblasts migrate into the cartilage and release high amounts of matrix degrading enzymes, thereby destroying the cartilage. It has been shown that receptors of repellent factors, like Robo3, Unc5B and Unc5C play crucial roles in the activation of these cells. Repellent factors and their receptors have originally been identified in the developing nervous system and regulate the direction of neurite outgrowth. In activated synovial fibroblasts, the repellent receptors are up-regulated and, therefore, the migration of the cells can be inhibited via treatment with the ligands. Another molecule, up-regulated in rheumatoid arthritis and osteoarthritis, is N-cadherin. This cell adhesion molecule has also originally been identified in the nervous system and can interact with the Robo receptor. N-cadherin has been shown to influence migration ability of cells, therefore being a possible regulator of the aggressiveness of synovial fibroblasts as well.
Goal of the first part of this work was to identify the Robo3 isoform expressed in synovial fibroblasts and to rule out signaling pathways after binding of the ligand Slit3 to the Robo3 receptor. In the second part, the involvement of miRNAs in the regulation of the altered gene expression in activated synovial fibroblasts as analyzed. The third part addressed the question if polymorphisms in the N-Cadherin gene could be responsible for the elevated N-cadherin expression in synovial fibroblasts from osteoarthritis patients compared to those obtained from healthy donors.
Herein, we were able to show that activated synovial fibroblasts obtained from patients with rheumatoid arthritis up-regulate the expression of the Robo3B isoform and not Robo3A. Robo3B is the isoform which is capable of binding the ligand Slit. Analyzing differences in transcription factor activity after Slit treatment revealed AP-1 as a potential mediator of gene expression triggered by Slit-binding to the Robo receptor. Another de-regulated RNA could be identified in activated synovial fibroblasts obtained from patients with rheumatoid arthritis, hsa-miR-188-5p. This miRNA is down-regulated in these cells compared to those of healthy donors. The down-regulation of hsa-miR-188-5p leads to the up-regulation of its target genes, e.g. KIAA1199. KIAA1199 is a hyaluronan-degrading enzyme and has already been shown to be involved in cartilage degradation caused by synovial fibroblast. We also identified a single nucleotide polymorphism, rs11564299, in the N-cadherin promoter leading to increased levels of the protein in individuals carrying the minor allele. This allele was protective in osteoarthritis. Moreover, a transcription factor, hnRNP K, was found to bind the minor allele exclusively. HnRNP K is therefore a possible contributor to the increased N-cadherin expression.
The results of this thesis may provide new targets for diagnostic or treatment of rheumatoid arthritis and osteoarthritis. For example, inhibition of Robo3 with Slit or treatment with miR-188-5p might be possible treatment approaches for the future. Further, determining the status of rs11564299 may be a useful tool to determine the risk of developing osteoarthritis.
