Abstract (German)
Die vorliegende Studie untersuchte CT-Aufnahmen von 54 Patienten mit primär irresektablen kolorektalen Lebermetastasen nach neoadjuvanter Chemotherapie auf prädiktive radiologische Parameter für die Resektabilität der Metastasen. Ebenso wurde untersucht, welchen Einfluss diese Parameter auf eine Expertengruppe aus Leberchirurgen haben. Diese sollten im Rahmen des retrospektiven Chirurgen-Reviews der CELIM-Studie, verblindet zu klinischen Daten und Therapie-Status, die Resektabilität der Patienten anhand radiologischer Aufnahmen beurteilen (Folprecht et al. 2010).
Das Patientenkollektiv der vorliegenden Studie bestand aus einer Auswahl an Patienten der CELIM-Studie, die neoadjuvant mit einer Kombination aus Cetuximab und FOLFOX6 bzw. FOLFIRI therapiert wurden, um in einen resektablen Zustand konvertiert zu werden.
Die radiologischen Parameter wurden retrospektiv, möglichst in portalvenöser Kontrastmittelphase, in Transversalschnitt-Aufnahmen der Follow-up-Untersuchungen ermittelt. Bestimmt wurden die Abstände der Metastasen zu Gefäßen, die Anzahl der Lebermetastasen und die Anzahl befallener Lebersegmente.
Im Rahmen der Abstandsbestimmungen wurden zwei Gefäßgruppen definiert. Zur Gefäßgruppe A zählten die Vena cava inferior und die Vena portae bis zu ihrer Aufzweigung. Die Gruppe B beinhaltete die Vena cava inferior, die Vena portae, die Venae hepaticae und die Hauptäste der Vena portae, die beiden letzteren bis zur ersten computertomographisch erfassbaren Aufzweigung.
Bei beiden Gruppen war der geringste ermittelbare Abstand zwischen einer Metastase und einem der definierten Gefäße entscheidend. Die Art des jeweiligen Bezugsgefäßes war von untergeordneter Bedeutung. Alle Patienten wurden hinsichtlich ihrer gemessenen Abstände mittels vier verschiedener Grenzwerte (< / ≥ 1 / 3 / 5 / 10 Millimeter) gruppiert und in separaten Analysen den Resektionsstatus und den Ergebnissen des Chirurgen-Reviews gegenübergestellt. Bezüglich der Metastasenzahlen wurden die Grenzwerte (≤ / >) 3 / 5 / 10 Metastasen auf Zusammenhänge mit Resektionsstatus und Chirurgen-Review untersucht. Hinsichtlich befallener Lebersegmente wurde der Grenzwert von < / ≥ 4 Segmenten auf gleiche Art und Weise analysiert. Abschließend wurden die Urteile des Chirurgen-Reviews noch mit den Resektionsstatus verglichen. Die Ergebnisse des Chirurgen-Reviews wurden in der vorliegenden Studie in zwei Kategorien zusammengefasst, die entweder eine Resektion befürworteten ("Resektion") oder sie ablehnten ("primär keine Resektion"). Die erzielten Resektionsstatus wurden in die Kategorien "R0 Resektion" und "Keine R0 Resektion" zusammengefasst. Die statistischen Analysen erfolgten in Vierfeldertafeln mit Hilfe des Exakten Tests nach Fisher.
Bei der Gegenüberstellung von Abständen und Resektionsstatus fanden sich für die Gefäßgruppe B signifikante Ergebnisse für die Grenzwerte (< / ≥) 1 mm (p = 0,038), 3 mm (p = 0,035) und 5 mm (p = 0,041). Die höchsten prädiktiven Werte lieferte der Abstand von < / ≥ 3 Millimeter. Nicht signifikant waren alle Abstandsanalysen zur Gefäßgruppe A in Kombination mit den Resektionsstatus. Alle Abstandsgrenzwerte beider Gefäßgruppen zeigten signifikante Zusammenhänge mit dem Chirurgen-Review. Dieses entschied sich am häufigsten für die Kategorie "primär keine Resektion" bei Abständen unter zehn Millimeter zur Gefäßgruppe A (p < 0,001). Eine Resektion befürworteten die meisten Chirurgen wenn alle Abstände zur Gefäßgruppe B mindestens zehn Millimeter betrugen (p = 0,012).
Ein Zusammenhang zwischen Resektionsstatus und Metastasenzahl fand sich bei einer Anzahl von maximal oder mehr als ( ≤ / >) fünf Metastasen (p = 0,015). Ein signifikanter Einfluss auf das Urteil des Chirurgen-Reviews wurde für ≤ / > 5 (p = 0,001) und ≤ / > 10 Metastasen (p = 0,019) ermittelt. Auch ein Befall von weniger oder mindestens vier Lebersegmenten beeinflusste die Meinung der Chirurgen (p = 0,001). Auf die Resektionsstatus wirkte sich dies nicht aus. Von allen Patienten, die vom Chirurgen-Review als primär nicht resektabel beurteilt wurden, erreichten in 92 % der Fälle keinen R0-Status (p = 0,002). Dagegen wurden weniger als die Hälfte aller Patienten der Kategorie "Resektion" nicht R0 reseziert. Der Abstand von unter drei Millimeter zur Gefäßgruppe B eignet sich offenbar am besten um Patienten zu selektieren, die höchstwahrscheinlich keinen R0 Status erreichen.
Bei jenen Patienten sollte eine Gefäßresektion bzw. -rekonstruktion erwogen werden. Alternativ mag die Aufnahme in Studien zur Optimierung der Überlebensraten bei R1 Resektionen sinnvoll sein. Der Metastasen-Gefäß-Abstand von < / ≥ 10 Millimeter ist bei der Gefäßgruppe B offenbar ebenso wenig bedeutsam für die R0 Resektabilität, wie alle untersuchten Abstände zur Gefäßgruppe A. Dagegen scheinen bei der chirurgischen Entscheidung für oder gegen eine Resektion die Abstände zu beiden Gefäßgruppen relevant zu sein. Der Abstand von < / ≥ 10 Millimeter ist offensichtlich immer noch am bedeutsamsten.
Für primär irresektable Patienten mit neoadjuvanter Chemotherapie hat der Grenzwert von ≤ / > 3 Metastasen keinen Einfluss auf die R0 Resektabilität oder die Entscheidung zur Resektion. Dagegen ist die Anzahl von ≤ / > 5 Metastasen für beides entscheidend. Mehr als zehn Lebermetastasen gelten meist als Ausschlusskriterium für eine Resektion. Allein auf Grund eines Befalles von vier oder mehr Lebersegmenten sollte eine Resektion nicht ausgeschlossen werden, auch wenn die Segmentzahl offenbar die Entscheidung zur Resektion beeinflusst.
Die erfassten prädiktiven radiologischen Parameter erreichten annähernd die Genauigkeit des Chirurgen-Reviews in der Bestimmung der R0 Resektionen. Jedoch sollten eventuelle zukünftige Resektabilitätsscores nur ergänzend bei der Entscheidung zur Resektion sein. Diese sollte möglichst in interdisziplinären Tumorboards gefällt werden.

In this study computed tomography (CT) scans of 54 patients with colorectal liver metastases were analyzed for predictive radiologic parameters for resectability of those metastases. All included patients had unresectable liver metastases and were treated with neoadjuvant chemotherapy in the context of the CELIM-trial (Folprecht et al. 2010). Furthermore the influence of those parameters on the decision making of a panel of expert liver surgeons was analyzed. In the context of the CELIM-trial that expert panel reviewed retrospectively and blinded "radiological images at both baseline and during treatment […] to assess objectively any changes in resectability" (Folprecht et al. 2010). Patients enrolled in the present study were an assortment of the CELIM patients. In the context of the CELIM trail all of them were treated with a combination of Cetuximab and either FOLFOX 6 or FOLFIRI to convert them into a resectable situation. Radiological parameters were analyzed retrospectively in follow-up axial images preferably in portal venous phase. Distances between metastases and vessels, the total number of liver metastases and the number of affected liver segments were recorded.
Concerning the distances between metastases and vessels two separated groups of vessels were defined. Group A included vena cava inferior and portal vein up to its bifurcation. Group B was defined as vena cava inferior, portal vein, hepatic veins and both branches of the portal vein up to their first branch-off captured by CT. For both groups the minimal distance between any metastasis of a patient and one of the included vessels was essential. The kind of vessel within a group did not matter in this analysis. Patients were subdivided by their measured distances with the help of four thresholds (< / ≥ 1 / 3 / 5 / 10 mm). The results were analyzed concerning their effect on the achieved resection status and their effect on the expert panel with the help of Fisher's exact test. Those analyses were done separately for each threshold. The analyses of the total number of liver metastases were done similarly with the following thresholds: ≤ / > 3 / 5 / 10 metastases. For the analysis of affected liver segments the only used threshold was defined as < / ≥ 4 segments. All those analyses were done independent of each other. One further analysis compared the decisions of the expert panel with the achieved resection status.
For all these analyses the decisions of the CELIM expert panel were summarized into two groups: "resection" and "primarily no resection". The achieved resection statuses were summarized as "R0 resection" and "No R0 resection".
As to the vessel analyses group B revealed significant results for the following thresholds: (< / ≥) 1 mm (p = 0,038), 3 mm (p = 0,035) and 5 mm (p = 0,041). A distance of < / ≥ 3 mm demonstrated the highest positive predictive value. There was no analysis of group A vessels and the resection status which was significant. However all analyses of metastasis to vessel distances revealed significant results with the decisions of the expert panel. The panel decided most frequently in favor of "primarily no resection" in case of distances of less than ten millimeters to group A vessels (p < 0,001). A distance of at least ten millimeters between any metastasis and group B vessels led most frequently to a decision in favor of "resection" (p = 0,012). A threshold of ≤ / > 5 metastases demonstrated significant results concerning the resection status (p = 0,015). A number of ≤ / > 5 (p = 0,001) and ≤ / > 10 metastases (p = 0,019) as well as the affection of < / ≥ 4 liver segments (p = 0,001) had a significant influence on the decision making of the expert panel. However the number of affected segments revealed no significant effect on the resection status. 92 % of all patients rated as "not resectable" by the expert panel did not achieve R0 status (p = 0,002). In contrast less than half of the patients rated as "resectable" achieved R0 status. Apparently a distance of less than three millimeters to any vessel of group B is the most appropriate way to select patients who have a very high probability to not achieve R0 status. Those patients should be considered to have resection and reconstruction of the affected vessel. Alternatively it could make sense to include those patients in other studies which intend to improve the survival of R1 resections. As to group B a metastasis to vessel distance of < / ≥ 10 millimeters is as unimportant for R0 resectability as all the investigated distances to group A vessels apparently. In contrast distances to both defined groups of vessels (A/B) have an influence on the decision making of liver surgeons with regard to the resectability of liver metastases. A distance of < / ≥ 10 millimeters appears to be still the most important threshold in that process. For patients with primarily unresectable liver metastases and neoadjuvant chemotherapy a number of ≤ / > 3 metastases does not have any impact neither on the decision making of liver surgeons nor on the final resection status. However a number of ≤ / > 5 metastases influences both. A total number of more than ten liver metastases can be considered as an exclusion criterion for a resection in most cases. Even if a number of four and more affected liver segments have an influence on the decision making of liver surgeons a resection should not be excluded based upon that fact alone.
The predictive radiological parameters measured in this study almost reached the correctness of a panel of expert liver surgeons concerning the prediction of a R0 status. Even if there might be a future resectability score based on these parameters such a score should only be complementary to the decision process. The decision concerning a resection should always be made in the context of a multidisciplinary tumor board.
