Vor der Anwendung einer monoklonalen Antikörpertherapie mittels Cetuximab und Panitumumab wird prätherapeutisch der RAS-Mutationsstatus (NRAS/KRAS) von kolorektalen Karzinomen im Sinne eines prädiktiven Markers bestimmt. Vor der Anwendung einer monoklonalen Antikörpertherapie mittels Cetuximab und Panitumumab wird prätherapeutisch der RAS-Mutationsstatus (NRAS/KRAS) von kolorektalen Karzinomen im Sinne eines prädiktiven Markers bestimmt. In dieser Arbeit wurde der KRAS/BRAF-Mutationsstatus zwischen Primärtumoren (n=109) und korrespondierenden Metastasen verglichen. Es wurden 284 DNA-Proben von Tumoren und Metastasen kolorektaler Karzinome mittels Sequenzierung nach Sanger sowie der Sequenzierung mittels PyroMark Q24 untersucht. Neben Metastasen in Lymphknoten (n=51) wurden Lebermetastasen (n=55), Metastasen im Peritoneum (n=34), Lungenmetastasen (n=15), Metastasen im Ovar (n=5), Hirn (n=4), Harnblase (n=2), Ureter (n=2), Haut (n=1) und Knochen (n=1) untersucht. Wir fanden eine hohe Übereinstimmung von 98,2% (107/109) im untersuchten Mutationsstatus zwischen Primärtumoren und Metastasen. Diskrepanzen in Bezug auf den KRAS/BRAF-Mutationsstatus waren insgesamt selten (2/109). Bei fehlender Verfügbarkeit des Primärtumors stellt die Untersuchung von Metastasengewebe, abgeleitet von unseren Daten, eine zuverlässige Möglichkeit dar, den KRAS/BRAF-Mutationsstatus zu bestimmen.

Prior to monoclonal antibody therapy by means of cetuximab and panitumumab the RAS mutation status (NRAS/KRAS) of colorectal cancer is used as a predictive marker. In this study the KRAS/BRAF mutation status of primary tumors (n=109) and their corresponding metastases were investigated. 284 DNA specimens of primary tumors and matched metastases were analyzed by Sanger sequencing and Pyrosequencing (PyroMark Q24). We examined various metastatic sites, such as lymph nodes (n=51), liver (n=55), peritoneum (n=34), lung (n=15), ovary (n=5), brain (n=4), bladder (n=2), ureter (n=2), skin (n=1) and bone (n=1) and found a high concordance of 98,2% (107/109) of the KRAS/BRAF mutation status between primary colorectal tumors and related metastatic sites. Discordance of KRAS/BRAF mutation status between primary tumors and corresponding metastases did rarely occur (2/109). Based on our data, mutation testing of KRAS/BRAF can be reliably performed in tumor samples of metastatic sites if primary tumor DNA is unavailable.
