Im Zeitraum zwischen 2008 und 2013 wurden in der Klinik und Poliklinik für Orthopädie der Unversitätsklinik Regensburg, Bad Abbach, 37 Patienten wegen einer kongentialen Hüftdysplasie mit einer Bandage nach Pavlik behandelt und nachuntersucht. Die Therapie mit dieser dynamischen Hüftorthese begann im Alter von durchschnittlich sechs Wochen (min. 4. Lebenstag – max. 26. Lebenswoche). Nach einer Therapiedauer von ebenfalls durchschnittlich sechs Wochen waren 97,3 % der bei Therapiebeginn dysplastischen Hüften gesund. Der Alpha-Winkel verbesserte sich unter der Therapie von durchschnittlich 48,2° auf 62,8° linkseitig und von 54,5° auf 64,5° rechtseitig. Der AC-Winkel verbesserte sich von durchschnittlich 23,4° auf 20° links und von 23,1° auf 20,6° rechts. Die Winkelverbesserung unter der Pavlik-Bandage war bei gleicher Therapiedauer und mit schlechterem Dysplasiegrad signifikant erhöht. Bei einer Hüfte schloss sich eine operative Pfannenkorrektur an. Bei keinen Hüften trat eine Nekrose auf. Der Anteil der Restdysplasien bei der letzten röntgenologischen Nachuntersuchung im durchschnittlichen Lebensalter von 3 Jahren belief sich auf 32,4 %. Die Restdysplasien standen aber in keinem Zusammenhang mit der initialen Beurteilung der Hüften. Alle Dysplasien vom Typ IIc und schlechter wurden von Beginn an mit der Pavlik Bandage suffizient behandelt und innerhalb weniger Wochen in den Typ I nach Graf überführt (Ausnahme eine Hüfte Typ IIb).
Die Pavlik-Bandage ist die am meisten gebräuchliche Abduktionshüftorthese. Die Therapie mit dieser Bandage verlangt zwar von allen Beteiligten einen hohen Aufwand, ist jedoch ein effektives und zugleich relativ schonendes Verfahren. Die vorliegende Arbeit liefert Daten, die in Zukunft bei der Überlegung der Erstversorgung von kleinen Patienten mit einer dysplastischen Hüfte hilfreich sein können. Die Ergebnisse dieser Studie belegen den großen therapeutischen Nutzen der dynamischen Orthese nach Pavlik bei der Behandlung der kongenitalen Hüftdysplasie im Säuglingsalter und helfen symptomatische (Rest-)Dysplasien bei Jugendlichen und Erwachsenen in Zukunft zu verringern.

In the period from 2008 to 2013, 37 patients with developmental dysplasia of the hip were treated with a Pavlik harness and followed up in the Department of Othopedics of the University of Regensburg, Bad Abbach. Therapy with the dynamic hip orthosis started at the average age of six weeks (min 4 days - max 26 weeks). After a treatment period of six weeks on average, 97.3% of the dysplastic hips were healthy. The alpha-angle improved under the treatment by an average of 48.2° to 62.8° on the left hip joint, and by 54.5° to 64.5° on the right hip joint. The AC-angle improved on average from 23.4° to 20° on the left and from 23.1° to 20.6° on the right. The angle improvement under the Pavlik harness was significantly increased with the same duration of treatment and with worse grades of hip dysplasia. A surgical pan correction was needed in one case. No hip necrosis occurred in any patients. The proportion of patients with residual hip dysplasia amounted to 32.4% at the final radiographic follow-up at approximately three years. These remaining hip dysplasias stood in no relation to the initial assessment of the hips. Dysplasias that were type IIc or worse were treated sufficiently from the beginning with the Pavlik harness and within a few weeks became a type I by Graf (except for one hip type IIb).
The Pavlik harness is the most common hip orthosis. Even though treatment with this orthosis requires a lot of effort for everyone involved, it is an effective yet relatively gentle method. The present study provides data that can be helpful in the consideration of the initial treatment of young patients with dysplastic hips in the future. The results of this study demonstrate the great therapeutic benefit of the Pavlik harness in the treatment of congenital hip dysplasia in infancy, and help to decrease symptomatic (residual) dysplasia in adolescence and adulthood.
