Abstract (German)
Bei Keloiden handelt es sich aus klinischer Sicht um gutartige, fibroproliferative Hauttumore, welche eine progressive Form der gestörten Wundheilung darstellen und bisher nur beim Menschen beobachtet werden konnten. Sie infiltrieren durch Überschreitung der ursprünglichen Begrenzung der Wunde das gesunde Nachbargewebe und zeigen keine Tendenz zur spontanen Rückbildung. Sie stellen in vielfacher Weise eine große Belastung für den Patienten dar, oftmals in Form von Schmerzen, Jucken sowie ästhetischer oder auch funktioneller Einschränkung. Keloide stellen zudem eine große Herausforderung für den behandelnden Arzt dar. Trotz einer großen Vielfalt an Therapieoptionen existiert noch keine definitiv wirksame, nebenwirkungs- oder rezidivfreie Therapieform, was wiederum das eingeschränkte Verständnis des genauen Pathomechanismus sowie der molekularen Zusammenhänge dieser Wundheilungsstörung wiederspiegelt.
Im Rahmen dieser Dissertation sollten das Proliferationsverhalten und die Sekretion wundheilungsrelevanter Wachstumsfaktoren und Zytokine von patientenspezifischen Keloidkulturen auf zellulärer Ebene in vitro charakterisiert werden, sowohl unter Standardbedingungen als auch unter definierter Beeinflussung des Wachstumsmilieus. Zu diesem Ziel erfolgte die Aussaat von Keloidfibroblasten (KF) aus sechs verschiedenen patientenspezifischen Kulturen und normaler dermaler Fibroblasten als Kontrolle (NHF). Die Beurteilung der Zellviabilität unter Standardbedingungen erfolgte mittels Resazurin-Assay und Fluoreszenzmessung nach 72 h Inkubation sowie durch Aussaat in einem 6-Well-System und Inkubation für 96 h mittels anschließender Zellzählung. Es zeigte sich eine signifikant geringere Proliferation bei allen Keloidkulturen gegenüber NHF als Kontrolle. Auch die Ergebnisse der einzelnen Keloidkulturen zeigten im Vergleich untereinander signifikante Schwankungen. Die Analyse der Protein- und Genexpression der Keloidfibroblasten unter Standardbedingungen zeigte eine gegenüber den NHF signifikant verringerte Syntheseleistung für IL-6, aber eine um ein Vielfaches verstärkte Expression von VEGF. Es bestätigte sich, dass VEGF ein durchaus bedeutender Faktor in der Pathogenese der Keloide und letztlich auch für die Keloidtherapie sein könnte. Die Rolle von IL-6 blieb in diesem Zusammenhang aufgrund der individuellen Unterschiede unklar.
Des Weiteren wurde eine Darstellung des Einflusses mechanischer Krafteinwirkung in Form zyklischer Dehnung im Minutentakt für 72 h während der Kultivierung angestrebt. So sollte die Rolle dieses Faktors im Rahmen der Keloidpathogenese und eines komplikationsarmen Heilungsverlaufs nach Keloidexzision eruiert werden. Es ergab sich weder bei den NHF noch bei den KF eine signifikant veränderte Proliferation bei den gedehnten Proben im Vergleich zur ungedehnten Kontrolle. Auch die Proteinexpression von VEGF sowie IL-6 war weder bei den NHF noch bei den Keloidfibroblasten signifikant verändert unter dem Einfluss der Dehnung.
Als drittes Ziel sollte ermittelt werden, ob der therapeutische Effekt von Botulinumtoxin A in der Keloidbehandlung anhand patientenspezifischer Keloidgewebeproben auf zellulärer Ebene reproduzierbar ist und sich so eine positive Wirkung von Botulinumtoxin A in der Therapie begründen ließe. Ferner sollte analysiert werden, ob für die Wundheilung relevante Zytokine und Wachstumsfaktoren unter dem Einfluss von Botulinumtoxin A entscheidend moduliert werden. Es wurden Konzentrationen von 1 IU, 2.5 IU, 5 IU und 10 IU Botulinumtoxin A im Resazurin-Assay eingesetzt sowie 1 IU und 2.5 IU bei Inkubation im 6-Well-System für 72 h. In beiden Fällen gab es zusätzlich einen Versuchsansatz ohne Botulinumtoxin A als Kontrolle. Es zeigte sich weder bei den NHF noch bei den KF eine signifikante Veränderung der Zellproliferation bei einer der Konzentrationen von Botulinumtoxin A im Vergleich zur Kontrolle. Auch die Untersuchung der Proteinexpression von IL-6, VEGF und TGF-ß1 ergab keine signifikante Veränderung unter dem Einfluss der Inkubation mit Botulinumtoxin A im Vergleich zur Kontrolle.

Keloids are benign, fibro-proliferative skin tumors, which represent a progressive type of impaired wound healing and have only been observed in humans. They expand beyond the boundaries of the initiating skin wound by infiltrating the surrounding healthy tissue and show no tendency for spontaneous regression. By causing pain, pruritus, aesthetic and functionaI restrictions they mean in most cases a massive weight bearing for the patients. Despite many and various treatment options in the therapy of keloids, by now no strategy has been found which is definitely efficient, free of undesirable side effects or recurrence. This fact shows the constrained understanding of the pathomechanism and the molecular mechanisms underlying keloid formation which are only partially understood.
In this dissertation several patient-customized cultures of keloid fibroblasts should be characterized at cellular level in vitro in terms of proliferation and protein expression of important cytokines and growth factors in wound healing. This should be examined at standard conditions and during defined manipulation of the growth milieu and conditions during incubation. Therefore fibroblasts isolated from six different patient-customized keloid cultures (KF) and normal dermal fibroblasts (NHF) as control culture were disseminated. Cell viability at standard conditions was investigated by resazurin-assay with fluorescence measurement after 72 h of incubation and by incubation in a 6-well-system for 96 h and cell counting afterwards. The fibroblasts of each keloid culture showed a significant reduced cell proliferation in comparison with NHF as control. The results of protein and gene expression on standard conditions included a significant lower expression for IL-6 in KF towards NHF, but a significant increased synthesis for VEGF in keloid fibroblasts in comparison to NHF. These results confirm the hypothesis of VEGF as an important factor in the pathogenesis of keloids and regarding their therapy. The role of IL-6 in this context couldn`t get evolved by the results of this study and stayed obscure.
A further target was to describe which influence mechanical forces take in terms of cyclical stretching tension at minute intervals for 72 h during incubation, in order to analyze mechanotransduction pathways and to develop new strategies for keloid prevention and treatment after surgical excision. Neither NHF nor KF showed a significant altered cell proliferation after stretching compared with the unstretched control. As well the protein expression of VEGF and IL-6 was not influenced by the mechanotransduction. NHF and KF didn`t show a changed synthesis for both proteins after stretching in comparison to their unstretched control.
Finally the therapeutic effect of botulinum toxin A as an option in treating keloids should be determined at patient-customized cultures at cellular level to find an explanation for its positive impacts in the therapy of keloids. For the same reasons the expression of important cytokines and growth factors was analyzed addicted to the influence of botulinum toxin A. Concentrations of 1 IU, 2.5 IU, 5 IU und 10 IU botulinum toxin A were employed for incubation (72 h) at resazurin assay. During incubation (72 h) at 6-well-system 1 IU and 2.5 IU were used. For both experiments a control without botulinum toxin A was applied. No effects on the cell proliferation under the influence of incubation with botulinum toxin A could be detected. Neither NHF nor KF showed differences in proliferation comparing the control to the fibroblast cultures after incubation with botulinum toxin A. The results for the protein expression of IL-6, VEGF and TGF-ß1 were similar. No significant change could be determined influenced by botulinum toxin A.
