Der menschliche Organismus ist täglich einer Vielzahl von schädlichen und krankheitserregenden Einflüssen durch Pilze, Viren, Bakterien oder Toxinen ausgesetzt. Um sich dieser zu erwehren hat sich im Laufe der Evolution das Immunsystem ausgebildet. Bei der Abwehr von potentiellen Krankheitserregern spielen neben den Zellen des Immunsystems die hoch konservierten antimikrobiellen Proteine der angeborenen Immunität eine essentielle Rolle. In dieser Arbeit sollten sowohl Funktion, als auch Expressionsregulation des antimikrobiellen Abwehrproteins Bactericidal/permeability increasing protein (BPI) anhand von neu generierten Mausmodellen charakterisiert werden. BPI des Menschen wird im Knochenmark gebildet und hauptsächlich in primären Granula von neutrophilen Granulozyten gespeichert. Eine Expression von humanem BPI konnte in mukosalen Epithelzellen des Verdauungstraktes, der Atemwege und des Urogenitaltraktes nachgewiesen werden. In erster Line ist BPI für seine bakterizide und bakteriostatische Wirkung gegenüber gramnegativen Bakterien bekannt, neuere Arbeiten belegen eine Wirkung auf grampositive Bakterien. Das murine Ortholog wurde erst vor wenigen Jahren entdeckt und wird nur in geringen Mengen konstitutiv exprimiert. Die antiinflammatorische Wirkung der beiden Proteine ist ähnlich, murines BPI zeigt jedoch eine abgeschwächte bakterizide Wirkung. Um die Funktion von BPI detailliert untersuchen zu können wurden BPI-defiziente, für BPI und für BPI und LBP humanisierte Tiere verwendet. Nachdem sowohl in vitro als auch in vivo abgesichert war, dass die basalen Expressionslevel für BPI und dessen funktionellen Antagonisten LBP in der transgenen Maus dem menschlichen System entsprechen, wurden Versuche zur Expressionsregulation nach Infektion mit gramnegativen und grampositiven Erregern durchgeführt. Hierbei zeigte sich eine zeitlich und örtlich divergierende Regulation der beiden Proteine, wodurch BPI (lokal) und LBP (systemisch) als Biomarker für Infektions- bzw. Entzündungsgeschehen eingesetzt werden könnten. Um langwierige, kostenintensive und ethisch schwer zu rechtfertigende Tierversuche zu vermeiden, wurden im Verlauf dieser Arbeit erfolgreich ER-Hoxb8-basierte immortalisierte hämatopoetische Stammzelllinien generiert und zu Neutrophilen differenziert, die ein neues geeignetes System für detaillierte Untersuchungen der Expressionsregulation und Funktion von BPI darstellen.
Eine besondere Bedeutung kommt BPI bei der chronischen Atemwegserkrankung Mukoviszidose oder Zystische Fibrose (CF, cystic fibrosis) zu. Die Infektion mit P. aeruginosa ist die häufigste Todesursache bei diesen Patienten. Charakteristisch für das Infektionsgeschehen ist eine massive Einwanderung von neutrophilen Granulozyten, wodurch eine erhöhte Ausschüttung von BPI am Entzündungsherd stattfindet. Folglich ist die Rolle von BPI auch für bakterielle Lungeninfektionen mit Streptococcus pneumoniae (S. pneumoniae) von besonderem medizinischem Interesse. Daher wurde die Wirkung von BPI auf den P. aeruginosa- und S. pneumoniae-Infektionsverlauf, sowie die Bedeutung bei der Modulation der Entzündungssituation in der Lunge im Modell der pulmonalen Infektion untersucht, um neue Ansatzpunkte für die Behandlung dieser Entzündungs- und Infektionsprozesse aufzudecken. Humanes BPI führte in den transgenen Tieren zu einem Vorteil bei der Bekämpfung der Bakterien. Murines BPI zeigte in Übereinstimmung mit vorhandener Literatur eine geringere bakterizide Wirkung, spielte jedoch eine wichtige Rolle bei der Immunmodulation nach Erregerkontakt. Die durchgeführten Versuche zeigten erstmalig eine essentielle, nicht redundante Funktion von BPI für die Entzündungsregulation und Erregereliminierung bei einer Pneumonie mit grampositiven Bakterien, was dieses Protein zu einem möglichen Kandidaten für die Therapie von chronischen infektiösen Lungenerkrankungen macht.
Die Rolle von BPI bei akuter und chronischer Entzündung des Darms und die Auswirkung auf das intestinalen Immunsystems waren ein weiterer Schwerpunktpunkt dieser Arbeit und wurden im Modell der DSS-induzierten Kolitis untersucht. Bei chronischen Entzündungsprozessen, wie Morbus Crohn (MC) oder Colitis ulcerosa (CU), scheinen nach heutigem Kenntnisstand komplexe Defekte in der Homöostase vorzuliegen, die aus einer gestörten Interaktion zwischen dem mukosalen Immunsystem, genetischer Prädisposition und Umweltfaktoren resultieren. Die erhobenen Daten sprechen dafür, dass sich BPI als möglicher Kandidat zur Therapie von chronisch entzündlichen Darmerkrankungen eignet, da sich dessen An- bzw. Abwesenheit auf den Verlauf der Entzündung auswirkt. Während eine BPI-Defizienz zum frühen Zeitpunkt (akut) noch keine Auswirkung hat, zeigt sich in der Anschlussphase (Regeneration) eine positive Wirkung auf den Entzündungsverlauf. Diese hängt vermutlich mit einer abgeschwächten Immunantwort zusammen, welche zu geringerer Entzündung, bzw. Schädigung des Darms führt. Langfristig ist eine effektive Immunantwort zur vollständigen Regeneration des Darmgewebes und zur Erreichung der Homöostase erforderlich, wobei BPI eine wichtige Rolle spielt. Mögliche verantwortliche Ursachen wurden in einer Mikrobiomanalyse mittels NGS ermittelt. Die erhobenen Daten unterstützen die Hypothese, wonach die Komposition der bakteriellen Besiedlung des Verdauungstraktes eine entscheidende Rolle bei der Entstehung intestinaler Entzündungen spielt. BPI-defiziente Tiere wiesen durch den fehlenden Selektionsdruck bei Fehlen dieses antibakteriellen Proteins eine deutlich höhere Diversität auf. Die Mikrobiomanalyse bestätigte dadurch unsere Hypothese, dass BPI die Zusammensetzung des Darmmikrobioms und infolge dessen den Verlauf von entzündlichen Darmerkrankungen beeinflusst. Basierend auf diesen Befunden ist BPI somit ein hochinteressanter neuer Therapiekandidat für die Behandlung chronisch entzündlicher Darmerkrankungen.
In einer weiteren Serie von Experimenten wurde die Bedeutung der Mausgenetik für die Ausprägung der Infektions- und Entzündungsmodellen untersucht. Der Mausstamm C57BL/6 hat sich seit der Weitergabe von Zuchttieren von The Jackson Laboratory (JAX) an die National Institutes of Health (NIH) in zwei Unterstämme aufgeteilt, C57BL/6J (JAX) und C57BL/6N (NIH). Seit längerem war bekannt, dass sich beide Stämme in einer Vielzahl von Eigenschaften unterscheiden. Auch in den in dieser Arbeit getesteten Modellen waren drastische Unterschiede zwischen den Unterstämmen zu verzeichnen. BL/6N-Mäuse waren generell stärker von Infektionen oder Entzündungen betroffen. Vor allem bei der P. aeruginosa-induzierten Lungeninfektion wiesen diese Inzuchtmäuse eine überschießende Immunantwort bei gelichzeitig schlechterer Erregerkontrolle auf. Im Modell der akuten DSS-induzierten Kolitis wurde ebenfalls ein signifikant schwererer Verlauf bei BL/6N-Tieren festgestellt. Für detaillierte Einblicke wurden Mikrobiomanalysen mittels NGS durchgeführt, die eine unterschiedliche Zusammensetzung des Darmmikrobioms bei den BL/6-Stämmen zeigte. Anhand von sogenannten co-housing Experimenten konnte gezeigt werden, dass diese Unterschiede im Darmmikrobiom ursächlich für den divergierenden Verlauf der induzierten Kolitis sind und eine Angleichung der Mikrobiomkomposition in einem identischen Kolitisverlauf resultierte.

The human organism is exposed to a variety of harmful and disease-causing influences by fungi, viruses, bacteria or toxins daily. In order to defend against this, the immune system has formed in the course of evolution. In the defense of potential pathogens the highly conserved antimicrobial proteins of innate immunity play an essential role in addition to the cells of the immune system. In this study both function and expression of the antimicrobial defense protein Bactericidal/Permeability Increasing Protein (BPI) should be characterized based on newly generated mouse models. Human BPI is formed in the bone marrow and mainly stored in primary granules of neutrophils. Expression of human BPI was detected in mucosal epithelial cells of the digestive tract, respiratory tract and urogenital tract. First Line BPI is known for its bactericidal and bacteriostatic activity against Gram-negative bacteria, more recent studies show an effect on Gram-positive bacteria. The murine ortholog was discovered only a few years ago and is only in small amounts constitutively expressed. The anti-inflammatory activity of the two proteins is similar, but murine BPI shows an attenuated bactericidal effect. To investigate the function of BPI in detail BPI deficient, BPI and BPI and LBP humanized animals were used. After it was secured both in vitro and in vivo that the basal expression levels for BPI and its functional antagonists LBP in the transgenic mouse are corresponding to the human system, experiments were conducted to examine expression regulation by infection with Gram-negative and Gram-positive pathogens. A time and place divergent regulation of two proteins could be detected, thus BPI (locally) and LBP (systemic) could be used as biomarkers for infectious or inflammatory process. To avoid tedious and expansive animal experiments, ER HOXB8-based immortalized hematopoietic stem cell lines were successfully generated and differentiated into neutrophils, which represent a new system suitable for detailed studies of the regulation of expression and function of BPI.
Particular importance is attached to BPI in chronic respiratory disease as cystic fibrosis Cystic fibrosis (CF, cystic fibrosis). Infection with Pseudomonas aeruginosa (P. aeruginosa) is the most common cause of death in these patients. Characteristic of the infection process is a massive influx of neutrophils, causing an increased secretion of BPI. Consequently, the role of BPI during bacterial lung infections with Streptococcus pneumoniae (S. pneumoniae) is of particular medical interest. Therefore, the effect of BPI to the P. aeruginosa and S. pneumoniae infection course, and the importance in the modulation of inflammatory situation in the lung was examined in the model of pulmonary infection in order to uncover new targets for the treatment of these inflammatory and infectious processes. Human BPI resulted in an advantage for transgenic mice to combat bacteria. Murine BPI has in accordance with existing literature a lower bactericidal effect, however played an important role in immunomodulation. The tests carried out showed the first time an essential, non-redundant function of BPI for the inflammation regulation and elimination of pathogens in pneumonia with Gram-positive bacteria, which makes this protein a possible candidate for the treatment of chronic infectious lung diseases.
The role of BPI in acute and chronic inflammation of the intestine and the effect on the intestinal immune system were another point of this work and was studied in the model of DSS-induced colitis. Chronic inflammation, such as Crohn's disease (MC) or ulcerative colitis (UC), appear to be based on complex defects in the homeostasis resulting from a disturbed interaction between the mucosal immune system, genetic predisposition and environmental factors. The data collected indicate that BPI is a possible candidate for the treatment of inflammatory bowel disease, because its presence or absence affects the course of inflammation. While BPI deficiency in the early state (acute) has no effect, a beneficial effect on the inflammatory process could be detected in the following phase (regeneration). This is presumably due to a weakened immune response, which results in less inflammation, or damage to the intestine. In the long term an effective immune response is needed for complete regeneration of intestinal tissue homeostasis, wherein BPI plays an important role. Possible causes responsible have been identified via microbiome analyses by NGS. The collected data support the hypothesis that the composition of the bacterial colonization of the digestive tract plays a crucial role in the pathogenesis of intestinal inflammation. BPI-deficient animals showed a much higher diversity through the lack of selection pressure in the absence of this anti-bacterial protein. This confirmed our hypothesis that BPI influences the composition of gut microbiota and consequently the course of inflammatory bowel disease. Based on these findings BPI is a very interesting novel therapeutic candidate for the treatment of this disease.
In another series of experiments, the significance of mouse genetics in infection and inflammation models was examined. The mouse strain C57BL/6 has been divided into two sub-strains, C57BL/6J (JAX) and C57BL/6N (NIH), since the transfer of breeding animals from The Jackson Laboratory (JAX) at the National Institutes of Health (NIH). It has been known that both strains differ in a variety of properties. In this work drastic differences between sub-strains were recorded. BL/6N mice were generally more affected by infection or inflammation. Especially with the P. aeruginosa-induced lung infection this inbred mice exhibited an excessive immune response and worse pathogen control. In the model of acute DSS-induced colitis a significantly heavier course within BL/6N animals was found, too. For detailed insights microbiome analyses were performed, which showed a different composition of the gut microbiota among BL/6 strains. Co-housing experiments resulted in an approximation of microbiome composition and in an identical course of colitis, which showed that microbiome differences are the cause of the divergent profile of the induced colitis.
