Abstract (German)

Die knöcherne Hemithoraxaufnahme ist eine häufig angewandte radiologische Untersuchungsmethode bei klinischem Verdacht auf Rippenfrakturen, ihr Nutzen wird jedoch in mehreren Studien angezweifelt. In dieser Arbeit wurde der diagnostische Nutzen der Hemithoraxaufnahme sowie die therapeutische Relevanz der erhobenen Röntgenbefunde untersucht. Hierfür wurden alle 669 Patienten retrospektiv untersucht, die 2009 und 2010 eine knöcherne Hemithoraxaufnahme am Universitätsklinikum Regensburg erhalten hatten.
Bezüglich der diagnostischen Aussagekraft in der Erkennung von Rippenfrakturen wurden Sensitivität/Spezifität von Hemithoraxaufnahme und konventioneller Thoraxübersichtsaufnahme mit der CT verglichen. Es zeigte sich, dass die CT signifikant die meisten Rippenfrakturen entdeckte, gefolgt von der Hemithoraxaufnahme und zuletzt der konventionellen Thoraxübersichtsaufnahme.
Bezüglich der therapeutischen Relevanz wurden Behandlungshäufigkeit sowie Behandlungsart - im Falle einer Behandlung - in Abhängigkeit vom erhobenen Röntgenbefund untersucht. Es konnte gezeigt werden, dass Patienten mit Frakturnachweis nicht signifikant häufiger behandelt wurden als Patienten ohne Frakturnachweis. Im Falle einer Behandlung wurden Patienten mit Frakturnachweis und Komplikationen signifikant aufwändiger behandelt als Patienten ohne Fraktur.
Folglich wird eine strenge Indikationsstellung für die Hemithoraxaufnahme empfohlen.

Rib series are a common radiological method for the detection of rib fractures, although it´s value is questioned in many studies. In this study we investigated the diagnostic benefit of rib series as well as the therapeutic relevance of the radiological results. We retrospectively examined 669 patients, that received rib series between 2009 and 2010 at the Universitätsklinikum Regensburg.
Regarding the diagnostic benefit of the detection of rib fractures we compared the sensitivity/specifity of rib series and plain chest x-ray with computed tomography (CT). This study showes that most rib fractures were detected by CT, followed by rib series and plain chest x-ray.
With respect to therapeutic relevance we also investigated the amount of treatment and kind of treatment (in case of treatment) depending on the radiological results. We found that patients with evidenced fractures were not significantly treated more often than patients without. In case of treatment patients with evidenced fractures and complications were treated more intensively than patients without fractures.
Therefore we recommend a strict consideration of using rib series.
