Abstract (German)
Hintergrund: In der Diagnostik von chronisch-entzündlichen Darmerkrankungen gibt es keinen einzelnen eindeutig festgelegten Goldstandard. Ziel dieser Studie ist es, die drei apparativen Säulen der CED-Diagnostik (makroskopische Befundung mittels endoskopischem Verfahren, histologische Beurteilung und radiologische Untersuchung mittels MRT) hinsichtlich ihrer Übereinstimmung zu überprüfen und eventuelle Korrelationen herauszuarbeiten. Da sowohl Daten zum Zeitpunkt der Erstdiagnose als auch im weiteren Verlauf gesammelt wurden, kann in dieser Arbeit das Befallsmuster der CED auch im Verlauf beurteilt werden.
Außerdem werden mögliche Risikofaktoren in Bezug auf die Befallsprogression untersucht.
Methoden: Alle Patienten, die in der Region Oberpfalz zwischen dem 1. Januar 2004 und dem 6. März 2010 mit der Erstdiagnose einer CED gemeldet und erfasst wurden, sind in diese Studie aufgenommen worden. Grundlage der hier zusammengetragenen Daten bilden standardisierte Fragebögen, die sowohl vom behandelnden Arzt als auch vom Patienten selbst zum Zeitpunkt der Erstdiagnose ausgefüllt wurden. Des Weiteren erfolgte die Datenerfassung der für diese Arbeit notwendigen Informationen mittels Überprüfung der Patientenakten am Universitätsklinikum Regensburg sowie der Erfassung der Daten von extern behandelten Patienten durch einen weiteren standardisierten Fragebogen. Dieser beinhaltete neben Informationen zum Befallsmuster zum Zeitpunkt der Erstdiagnose sowie im Verlauf auch Daten über mögliche Risikofaktoren und den sozialen und demografischen Hintergrund.
Ergebnisse: Von den 482 CED-Patienten litten bei Erstdiagnose 297 an Morbus Crohn, 173 an Colitis ulcerosa und bei 12 wurde eine Colitis indeterminata diagnostiziert. Bei der Beurteilung der Übereinstimmung des Befallsmusters zum Zeitpunkt der Erstdiagnose zeigt sich bei MC-Patienten bei makroskopischem Befund als Goldstandard eine Sensitivität der histologischen Untersuchungsergebnisse von 94% vs. 67% Sensitivität der MR-Tomographie. Im Bereich des terminalen Ileums liegt die Sensitivität der MRT-Untersuchung mit 88% am höchsten. Bei der Spezifität ergibt sich ein Bild von 92% bei der histologischen Untersuchung vs. 95% bei der MR-Tomographie. Bei der Frage nach der Übereinstimmung zwischen allen drei Untersuchungstechniken ergibt sich ein Kappakoeffizient κ von 0,561 (p≤0,01). Bei CU-Patienten ergibt sich ein Übereinstimmungswert κ von 1,000 (p≤0,01).
Zu einer Befallsmusteränderung im Sinne einer Progression kam es bei MC-Patienten in 16,8%, bei CU-Patienten in 9,7% der Fälle.
Für MC zeigt sich kein nennenswert erhöhtes relatives Risiko für eine Veränderung des Befallsmusters durch einen der potentiellen Einflussfaktoren. Auch bei CU ist dies der Fall. Einzige Ausnahme ist ein Trend hinsichtlich eines protektiven Einflusses eines Nikotinkonsums bei CU-Patienten (RR 0.42, [0.07-2.55]), allerdings ist dieser Faktor nicht statistisch signifikant (p=0,26).
Schlussfolgerung: Durch die gute Vergleichbarkeit sowohl der Patientencharakteristika als auch der Befallsmusterverteilung mit vielen anderen Studien eignet sich die Oberpfalzkohorte sehr gut, um die diagnostischen Methoden miteinander zu vergleichen und Befallsmusteränderungen im Verlauf und deren potentiellen Einflussfaktoren zu untersuchen. Die Daten dieser Studie zeigen, dass bei der Lokalisation des Befallsmusters die Übereinstimmung des makroskopischen und histologischen Untersuchungsergebnisses am größten ist. Die MRT-Untersuchung weist im Vergleich eine niedrige Sensitivität auf. Bei Befall im Bereich des terminalen Ileums kann sie jedoch eine sinnvolle Alternative darstellen, in anderen Abschnitten sind die makroskopische und histologische Diagnostik aber unerlässlich. Deshalb ist eine MRT-Untersuchung als alleinige diagnostische Methode bei der Erstdiagnose einer CED nicht geeignet, dient aber sicherlich als gute Ergänzung in einzelnen Segmenten wie dem terminalen Ileum. Zudem bleibt sie der Goldstandard bei der Bewertung endoskopisch nicht erreichbarer Regionen im Dünndarm. Risikofaktoren für die Progression einer CED scheinen sich durch unterschiedliche Studienergebnisse nicht eindeutig herauszustellen. Dies zeigt sich auch in dieser Arbeit. Eine Ausnahme scheint ein Trend hinsichtlich eines protektiven Einflusses von Nikotin bei der Progression der CU zu sein.

Background: There is no clearly defined gold standard in the diagnosis of chronic inflammatory bowel disease. The aim of this study is to review the three pillars of apparatus IBD diagnosis (macroscopic findings by endoscopic procedures, histological evaluation and radiological examination by MRI) for accordance and to identify any correlations. Since both data at the time of diagnosis as well as in the course were collected, a possible disease progression of IBD can also be assessed.
In addition, possible risk factors are examined in relation to the disease progression.
Methods: All patients with the diagnosis of IBD, who were reported and recorded in the Upper Palatinate region (Oberpfalz) between 1 January 2004 and 6 March 2010 have been included in this study. The basis of the collected data is standardized questionnaires that were filled out by the attending physician and also the patient himself at the time of initial diagnosis. Furthermore, additional data collection was necessary for this study by checking the medical records at Regensburg University Hospital or in form of an additional standardized questionnaire from patients treated externally. This included information of the pattern of disease at the time of initial diagnosis and in the course as well information about possible risk factors and the social and demographic background.
Results: Of the 482 IBD patients 297 suffered from Crohn's disease, 173 from ulcerative colitis and 12 from an indeterminate colitis. Taking macroscopic findings as the gold standard, there was a sensitivity of histological examination results of 94% vs. 67% sensitivity for MR imaging at the time of initial diagnosis. In the area of the terminal ileum the sensitivity of MRI is higher than in other regions (88%). The specificity was 92% on histological examination vs. 95% in MR imaging. In the issue of consistency between all three examination techniques Kappa coefficient κ is 0.561 (p≤0,01). In UC patients the matching value κ is 1.000 (p≤0,01).
A disease modification in the sense of a progression occurred in 16.8% of CD patients and in 9.7% of the CU cases.
For IBD there is no significantly increased relative risk affecting the disease progression. The only exception is a protective influence of nicotine use among CU patients (RR 0:42, [0:07 to 2:55]), however, this factor is not statistically significant (p = 0.26).
Conclusion: Due to the good comparability of patient characteristics and the pattern of disease with many other studies, the Upper Palatinate cohort is well suited to compare the diagnostic methods and to investigate pattern of disease changes in the course and their potential influencing factors. Data from this study show that macroscopic and histological results matching very good with regard to the localization of the infestation pattern. In comparison the MRI examination has a low sensitivity. However, in cases of infestation in the area of the terminal ileum, it can be a useful alternative. In other segments macroscopic and histological diagnostics are essential. Therefore, an MRI scan is not suitable as the only diagnostic method in the diagnosis of IBD, but it is certainly a good addition in single segments as the terminal ileum. Furthermore, it remains the gold standard in endoscopically unreachable regions in the small intestine. Because of different study results, risk factors for the progression of IBD do not clearly become apparent. This is also shown in this work. An exception seems to be the protective effect of nicotine in the progression of the CU.
