Das peritoneal metastasierte Magenkarzinom ist mit einer schlechten Überlebensprognose verbunden. Bereits in den 1980ern wurde deshalb damit begonnen, die Peritonealkarzinose mit zytoreduktiver Chirurgie (CRS) und hyperthermer intraperitonealer Chemotherapie (HIPEC) zu behandeln.
Die vorliegende Studie untersucht den Einfluss der HIPEC auf das Gesamtüberleben für 38 Patienten, welche aufgrund einer Peritonealkarzinose mit Primarius Magenkarzinom in den Zeiträumen Januar 2006 bis Juli 2012 an der Klinik und Poliklinik für Chirurgie der Universität Regensburg und Februar 2011 bis Januar 2013 an der Klinik für Allgemein- und Viszeralchirurgie am Krankenhaus Barmherzige Brüder in Regensburg mit einer HIPEC behandelt wurden. Zur Beurteilung der Wirksamkeit der HIPEC wurden diesen Patienten als Vergleichsgruppen 38 gastrektomierte Patienten mit Magenkarzinom ohne Metastasierung, 27 gastrektomierte Patienten mit Peritonealkarzinose ohne HIPEC-Behandlung und 31 Patienten mit Peritonealkarzinose ohne chirurgische Therapie anhand von Geschlecht, Tumorausdehnung, Lymphknotenstatus und Alter zugeordnet.
In der Überlebenszeitanalyse zeigte sich, dass das Überleben nach Peritonealkarzinose-Erstdiagnose der mit HIPEC behandelten Patienten im Vergleich zur Gruppe der gastrektomierten Patienten mit Peritonealkarzinose ohne HIPEC-Behandlung signifikant um 11,2 Monate im Mittel und um 6,2 Monate im Median länger ist. Vergleicht man das Überleben der HIPEC-Patienten mit der Palliativgruppe ohne chirurgische Therapie, so beträgt der Überlebensvorteil der HIPEC-Patienten im Mittel 15,3 Monate und im Median 9,5 Monate. Wird hingegen die HIPEC-Gruppe mit den Magenkarzinom-Patienten ohne Fernmetastasierung verglichen, so zeigt sich kein signifikanter Unterschied im Überleben dieser beiden Gruppen.
In Zusammenschau der Ergebnisse kann die HIPEC als wirksame Therapie für Patienten mit peritoneal metastasiertem Magenkarzinom bezeichnet werden und sollte deshalb bei passenden Patienten Anwendung finden. Die aggressive chirurgische Therapie mit CRS und HIPEC führt zu einem längeren Überleben, eine tatsächliche längerfristige Heilung konnte im betrachteten Patientenkollektiv jedoch nicht erzielt werden.

Translation of the abstract (English)

Peritoneal carcinomatosis from gastric cancer is linked to a bad survival prognosis. Because of that, some surgeons began to treat peritoneal carcinomatosis with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) in the 1980s.
This study examines the influence of HIPEC on overall survival of 38 patients who suffered from peritoneal carcinomatosis from gastric cancer and were treated with HIPEC in the period january 2008 to july 2012 at the Department of Surgery, University Medical Center Regensburg, Regensburg, Germany and february 2011 to january 2013 at the Department of Surgery, St. John of God Hospital Regensburg, Regensburg, Germany. To be able to measure the impact of HIPEC on survival, we matched patients by gender, tumor extent, degree of spread to regional lymph nodes and age to get comparison groups consisting of 38 patients with gastric cancer without metastasis treated with gastrectomy, 27 patients with gastric cancer and peritoneal carcinomatosis treated with CRS but not HIPEC and 31 patients with gastric cancer and peritoneal carcinomatosis not treated surgically.
In the survival analysis we could show that the survival after diagnosis of peritoneal carcinomatosis increased 11.2 months in mean and 6.2 months in median if a patient with peritoneal carcinomatosis from gastric cancer was treated with with HIPEC instead of only CRS. Comparing the survival of HIPEC patients with the patients of the palliative group (without any surgical treatment) the survival benefit of the HIPEC patients increased to 15.3 months in mean and 9.5 months in median. However, by comparing the survival of HIPEC patients to the survival of patients with gastric cancer without metastasis we could not find a statistically significant difference in survival between the groups.
In conclusion HIPEC can be identified as effective therapy for patients with peritoneal carcinomatosis from gastric cancer and therefore should be used with suitable patients. The aggressive surgical therapy with CRS and HIPEC leads to a higher overall survival. However, a complete long-term remission could not be observed in the examined patient population.
