Abstract (German)
Einleitung:
Die häufigste traumatische Verletzung des zweiten Halswirbels stellt eine Fraktur des Os odontoid oder Dens mit einem Anteil von 9%–16% an allen Halswirbelsäulenfrakturen bzw. von 1%–2% aller Wirbelsäulenfrakturen dar. Zahlreiche Studien wurden mittlerweile publiziert, die zeigen, dass die Mehrzahl der Autoren der operativen Therapie den Vorzug vor der konservativen Therapie gibt, weil bei der operativen Versorgung v.a. die Risiken einer Pseudarthrose weitaus geringer sind als bei der konservativen Therapie. Diese Studien liefern bezüglich der direkt postoperativen und kurzzeitig poststationären (FollowUp von 6 Monaten bis 41,5 Monaten, x̄=21,1 Monate) Ergebnisse anschauliche Aussagen, doch es finden sich in der Literatur wenig Angaben über mittelfristige Ergebnisse nach ventraler Schraubenosteosynthese der traumatischen Densfraktur. Ziel der vorliegenden retrospektiven Studie war es, insbesondere im Hinblick auf Frakturheilung, Funktionalität und den allgemeinen Gesundheitszustand der Patienten, die mittelfristigen Ergebnisse zu diskutieren und den Vergleich mit anderen Behandlungsmethoden darzustellen.
Material und Methode:
60 konsekutive Patienten der Klinik für Unfallchirurgie der Universität Regensburg, die im Zeitraum von Dezember 1993 bis März 2005 eine traumatische Densfraktur erlitten und operativ mittels ventraler Schraubenosteosynthese versorgt wurden, wurden in die Studie eingeschlossen. Die klinische Nachuntersuchung erfolgte zwischen September 2007 und Oktober 2008, im Mittel 97,9 Monate (± 39,5 Monate) postoperativ. Neben der Erfassung von patientenbezogenen Daten wie Alter zum Operationszeitpunkt, Geschlecht, Zeitraum zwischen Unfall und Operation, Zeitraum zwischen Operation und Entlassung und der Unfallursache wurden auch die Frakturart bzw. Dislokation, neurologische Einschränkungen (Frankel-Einteilung), Komplikationen, Begleitverletzungen sowie das Ergebnis der Frakturheilung ausgewertet. Die Röntgendiagnostik erfolgte mittels HWS-Aufnahmen in zwei Ebenen (a.p. und lateral) sowie einer Dens-Zielaufnahme. Zur Beurteilung des psychischen Zustandes der Patienten bzw. der subjektiven Beurteilung ihres Empfindens wurden zwei Fragebögen verwendet: der SF-36 und der SSE SPINE TANGO Nacken. Die mathematische Auswertung der Statistik erfolgte nach Überprüfung auf Normalverteilung mithilfe des Kolmogorov-Smirnow-Test mittels zweiseitigem Welch-Test (t-test) bei ungleichen Varianzen. Das Signifikanzniveau betrug für alle Tests 5% bzw. p<0,05.
Ergebnisse:
An der klinischen Nachuntersuchung beteiligten sich 16 von 60 Patienten (26,7%), dies ergibt ein lost to follow up von 44 Patienten oder 73,3%. Vom Gesamtkollektiv waren 36 (60%) Patienten männlich und 24 (40%) Patienten weiblich. Das Durchschnittsalter aller 60 Patienten betrug 55,3 Jahre (± 17,2 Jahre), wobei Männer im Durchschnitt mit 45,8 Jahren im Vergleich zu Frauen mit 68,3 Jahren deutlich jünger waren. Der Zeitraum zwischen Unfall und Operation betrug bei 34 Patienten (56,7%) 1-5 Tage und im Mittel 7,3 Tage, der Zeitraum zwischen Operation und Entlassung bei 46 Patienten (76,7%) 1-10 Tage und im Mittel 12,2 Tage. Von den 60 Patienten erlitten 34 (60%) einen Sturz, 21 (35%) einen Verkehrsunfall und 3 (5%) eine sonstige Verletzung. Der Vergleich von Unfallursache und Geschlecht zeigte auf, dass von 36 Stürzen als Unfallursache 20 Frauen (55,6%) und 16 Männer (44,4%) betroffen waren und von 21 Verkehrsunfällen als Ursache 19 Männer (90,5%) und zwei Frauen (9,5%). Die Densfraktur vom Typ II nach Anderson und D`Alonso war mit 51 Fällen (85%) am häufigsten vertreten. Bei 24 Patienten (40%) lag präoperativ keine, bei 18 Patienten (30%) eine Dislokation nach dorsal, bei elf Patienten (18,3%) eine Dislokation nach ventral vor. Bei sieben Patienten (11,7%) war keine Zuteilung möglich. Die häufigste neurologische Einschränkung war bei 9 Patienten (15%) Grad D, 48 (80%) hatten keinerlei neurologische Symptomatik. Komplikationen waren v.a. Einschränkungen in der Beweglichkeit und muskuläre Verspannungen. Die Gesamtmortalität betrug 31,6%, was 19 Patienten entspricht. Begleitverletzungen kamen insgesamt bei 51 Patienten (85%) vor, wobei die Atlasbogenfraktur mit 26,6% und andere Frakturen mit 31,6% am häufigsten vertreten waren. Bei 15 von 16 Patienten (93,8%) zeigte sich zum Zeitpunkt der Nachuntersuchung eine knöcherne Konsolidierung der Densfraktur mit regelrechter Schraubenlage. Bei einem Patienten (6,2%) wurde eine straffe Pseudarthrose nachgewiesen. Von 21 Patienten litten sieben Patienten (33,3%) an Schmerzen im Nacken, zwei Patienten (9,5%) an Schmerzen in Armen/Schultern und zwölf Patienten (57,1%) wiesen keinerlei Beschwerden auf. Insgesamt 19 von 21 Patienten (90,5%) gaben im SSE SPINE TANGO Nacken an, dass sie mit der Behandlung „sehr zufrieden“ oder „zufrieden“ waren und 18 Patienten (85,7%), dass Ihnen die Therapie „sehr geholfen“ oder „geholfen“ hat. Die Ergebnisse des Gesundheitszustandes des nachuntersuchten Patentenkollektivs mittels SF-36 zeigten im Vergleich zu einer gesunden Normpopulation in den Bereichen „körperliche Funktionsfähigkeit“, „körperliche Schmerzen“ und „emotionale Rollenfunktion“ schlechtere Ergebnisse. In den beiden Hauptgruppen „körperliches Wohlbefinden“ und „psychisches Wohlbefinden“ ergibt sich sowohl für das körperliche (p=0,11) als auch für das psychische Wohlbefinden (p=0,32) kein signifikanter Unterschied.
Zusammenfassung:
Die ventrale Schraubenosteosynthese stellt bei Densfrakturen eine bewährte Behandlungsmethode dar, vor allem um die problematische Fraktur vom Typ II nach Anderson und D`Alonzo sicher zu versorgen. Mit Hilfe der ventralen Schraubenosteosynthese können gute bis sehr gute operative Ergebnisse erzielt werden. Dennoch bedeutet eine Densfraktur trotz knöcherner Konsolidierung nach ventraler Schraubenosteosynthese mittelfristig eine Minderung der allgemeinen Lebensqualität für den Betroffenen, bedingt durch Bewegungseinschränkungen und anhaltende Beschwerden v.a. im Nacken und in der Schulterregion.

Introduction:
The most frequent traumatic injury of the second cervical vertebra represents a fracture of the os odontoid or “dens” with a percentage of 9%-16% of all cervical spine fractures and of 1%-2% of all spinal column fractures. By now, numerous studies were published which show that most authors give preference to the operative therapy because especially the risks of a pseudarthrosis are by far less than with the conservative therapy. These studies supply descriptive statements concerning the directly postoperative and briefly post-hospital (follow up from 6 months to 41.5 months, x̄=21.1 months) outcome, but few information concerning medium-term outcome by anterior odontoid screw fixation of the traumatic dens fracture. The aim of the present retrospective study was to discuss this medium-term outcome, in particular in view of fracture healing, functionality and the general state of health of the patients and compare the results with other treatment methods.
Materials and methods:
60 consecutive patients of the Department of Trauma Surgery at the University of Regensburg, who suffered a traumatic dens fracture between December 1993 and March 2005 and underwent surgical treatment by anterior odontoid screw fixation, were included in the study. The clinical follow-up took place between September 2007 and October 2008, on average 97.9 months (± 39.5 months) postoperatively. Apart from patient-related data such as age on the date of operation, sex, period between accident and operation, period between operation and hospital discharge and the cause of accident also the type of fracture and dislocation, neurological restrictions (Frankel classification), complications, combined injuries as well as the result of the fracture healing were evaluated. The radiological diagnosis was made with radiographic examinations, including lateral view, AP view and open-mouth view of plain X-ray films. For the evaluation of the psychological condition of the patients and the subjective evaluation of their feeling two questionnaires were used: the SF-36 and SSE SPINE TANGO. The statistics were made with the Kolmogorov Smirnow test and a bilateral t-test with unequal variances. The level of significance amounted to 5% or rather p<0,05 for all tests.
Results
In the clinical re-examination 16 of 60 patients (26.7%) participated, meaning a lost to follow up of 44 patients or 73.3%. From the overall collective 36 (60%) patients were male and 24 (40%) patients were female. The mean age of all 60 patients averaged 55.3 years (± 17.2 years), whereby men were significantly younger with 45.8 years compared with women with 68.3 years on average. The period between accident and operation amounted to 1-5 days for 34 (56.7%) patients with an average of 7.3 days, the period between operation and hospital discharge to 1-10 days for 46 (76.7%) patients with an average of 12.2 days. From the 60 patients 34 (60%) had a fall, 21 (35%) a traffic accident and a 3 (5%) another injury. The comparison of cause of accident and sex pointed out that from 36 falls 20 (55.6%) were female and from 21 traffic accidents 19 (90.5%) were male. The dens fracture Type II classified by Anderson and D`Alonso was most frequent with 51 cases (85%). No preoperative dislocation was present for 24 patients (40%), 18 patients (30%) had a dorsal dislocation and 11 patients (18.3%) a ventral one. The most frequent neurological restriction was level D with 9 patients (15%), 48 (80%) patients had no neurological symptomatology. Most complications were restrictions in terms of mobility and muscular tension. Overall mortality amounted to 31.6%, which corresponds to 19 patients. 51 patients (85%) suffered combined injuries, whereby a fracture of the atlas arch with 26.6% and other fractures with 31.6% were most frequently represented. 15 of 16 patients (93.8%) showed a bony consolidation with proper screw position at the time of the clinical follow-up. One patient (6.2%) showed a pseudarthrosis. Altogether 19 of 21 patients (90.5%) indicated with the SSE SPINE TANGO that they were “very content“ or “content“ with the treatment and 18 patients (85.7%) that the therapy was “very useful” or “useful“. The results of the health status of the patient by means of the SF-36 did not show significant differences in both main categories “physical well-being” and “mental well-being” compared with a healthy standard population (p=0.11 and p=0.32 respectively).
Summary
The anterior screw fixation represents a proven working method concerning dens fractures, in particular tending the problematic fracture from Type II classified by Anderson and D`Alonso. By means of anterior screw fixation good to very good surgical results can be achieved. Nevertheless a dens fracture – despite a bony consolidation – means a medium-term restriction of the general quality of life due to restrictions in terms of mobility and persistent pain especially in the neck and shoulder region.
