Abstract (German)
Hintergrund: Ösophaguskarzinome werden meist in einem bereits fortgeschrittenem Tumorstadium diagnostiziert. Obwohl die Therapie in den letzten Jahren immer mehr optimiert wurde, ist die Prognose dennoch sehr schlecht. In der vorliegenden Arbeit wurde ein Patientenkollektiv aus der Klinik und Poliklinik für Strahlentherapie des Uniklinikums Regensburg untersucht, das dort im Zeitraum vom 09/2003 bis 01/2011 behandelt wurde. Material und Methode: Die vorliegende retrospektive Arbeit umfasst Daten von 51 Patienten. Es wurde die Zusammensetzung des Patientenkollektivs, sowie die durchgeführten Therapiemodalitäten untersucht. Weiterhin erfolgte eine Auswertung der therapiebedingten Toxizitäten sowie eine Auswertung des Gesamtüberlebens und rezidivfreien Intervalls im Kollektiv. Ergebnisse: Eingeschlossen waren 45 Männer (88,2%) und 6 Frauen (11,8%), davon 45 Patienten (88,2%) mit einem Plattenepithelkarzinom, 5 Patienten (9,8%) mit Adenokarzinom und ein Patient (2%) mit einem Siegelringkarzinom des Ösophagus. Bei 31 Patienten erfolgte eine primäre Radiochemotherapie mit einer medianen Gesamtdosis von 66,6Gy (Gruppe RCT), bei 7 Patienten wurde eine primäre Radiotherapie mit einer medianen Gesamtdosis von 65,4Gy (Gruppe RT) durchgeführt und bei 13 Patienten erfolgte eine neoadjuvante Radiochemotherapie mit anschließender operativer Resektion mit einer medianen Gesamtdosis von 45Gy (Gruppe NA). Die Bestrahlung wurde in der Mehrheit als Konformations-RT durchgeführt. Seit April 2006 erfolgte die Bestrahlung teilweise auch in IMRT-Technik. Alle wurden in konventioneller Fraktionierung mit 1,8 – 2 Gy pro Tag fünf Mal die Woche bestrahlt. Akuttoxizitäten wurden mehr in den Gruppen mit einer Kombination von Radiotherapie und Chemotherapie beobachtet. Nach 1, 2 bzw. 3 Jahren lag das Gesamtüberleben bei 64,7%, 47,1% bzw. 33,1%. Das Gesamtüberleben zeigte eine signifikante Abhängigkeit von der Therapie und dem T-Status. Bei einem medianen Nachbeobachtungszeitraum von 17,3 Monaten entwickelten insgesamt 30 Patienten ein Rezidiv. Nach 1 bzw. 2 Jahren betrug der Anteil an rezidivfrei Überlebenden 54,9% bzw. 33,4%. Ein Karnofsky-Index von 80% oder mehr zeigte einen signifikanten Vorteil in Bezug auf das rezidivfreie Intervall. Schlussfolgerung: Insgesamt stellt die Anzahl von 51 Patienten ein sehr kleines Kollektiv dar. Insbesondere die Anzahl an Adenokarzinomen war mit insgesamt 5 sehr klein, so dass für diese keine repräsentativen Aussagen getroffen werden können. Die heterogene Gabe von Chemotherapeutika erschwerte den Vergleich der untersuchten Therapieansätze. Dieser Bias stellt einen der Nachteile einer retrospektiven Studie dar. Für eine weitere Studie wäre daher ein größeres Patientenkollektiv und eine homogene Verteilung der Tumorentitäten wünschenswert, um eine bessere Aussagekraft zu erzielen.

Background: Esophageal cancer is mostly diagnosed in an already advanced tumor stage. Although the therapy was optimated more and more over the past years, however the prognosis is very bad. In this study, a patient collective of the Clinic and Policlinic for Radiotherapy of the University medical center of Regensburg was examined. It was treated there in the period of 09/2003 till 01/2011. Material and Methods: This retrospective study includes data of 51 patients. The study evaluated the composition of the collective as well as the performed therapy modalities. Furthermore the therapy induced toxicities were analyzed as well as the overall survival and the progression free interval in the collective. Results: The collective was composed of 45 men (88,2%) and 6 women (11,8%), therefrom 45 patients (88,2%) with a squamous cell carcinoma, 5 patients (9,8%) with an adenocarcinoma and one patient (2%) with a signet ring cell carcinoma. 31 patients were treated with a definitive chemoradiotherapy with a median total dose of 66,6 Gy (Group RCT), 7 patients were treated with a definitive radiotherapy and with a median total dose of 65,4 Gy (Group RT) and 13 patients were treated with a neoadjuvant radiochemotherapy and following surgical resection with a median total dose of 45 Gy (Group NA). On the majority the radiotherapy was performed as a 3-dimensional conformal radiotherapy. Since april 2006, the radiotherapy was also performed in IMRT-technique. All were irradiated in conventional fractionation with 1,8 – 2 Gy per day 5 times a week. Acut toxicities were mainly observed in the groups, who received chemotherapy combined with radiotherapy. After 1, 2 and 3 years respectively the overall survival rate was 64,7%, 47,1% and 33,1%. The overall survival showed a significant dependence on therapy and T-Status. After a median follow-up of 17,3 months, 30 patients develop a relapse. After 1 and 2 years the part of progression-free survivors was 54,9% and 33,4%. A Karnofsky-Index of 80% or more was significantly beneficial related to the progression-free interval. Conclusion: All in all, the quantity of 51 patients is a very small collective. In particular the quantity of adenocarcinoma was with a total of 5 very small. For this group the evidence is not representative. Due to the inhomogeneous CT-Regime, the comparison of the examined therapy modalities is difficult. This bias is one of the disadvantages of a retrospective study. For following studies a larger patient collective and a more homogeneous distribution of tumor entities would be desirable.
