Abstract (German)

Die frühe zelluläre Immuntwort nach Hepatits-B-Impfung ist bislang weitestgehend unerforscht. In einer prospektiven, kontrollierten Pilotstudie wurde die frühe zelluläre Immunantwort bei 16 Probanden während einer Hepatitis-B-Grundimmunisierung im Elispot-Assay untersucht. Hierzu wurde den Spendern im Verlauf von 9 Monaten zehnmal Blut entnommen und auf eine HBsAg-spezifische IFN γ- und iL 2-Aktivität untersucht. Diese wurden mit der Entwicklung von Anti-HBs korreliert. Bei 43,8% konnte innerhalb der ersten 48 Stunden und bei über 80% innerhalb der ersten 7 Tage nach Erstimpfung eine spezifische zelluläre Immunantwort nachgewiesen werden. Das dominierende Zytokin in der Frühphase war IFN γ. Überraschenderweise zeigte sich bei 6 der 16 Spender bereits vor Impfung eine Zytokinantwort nach Stimulation mit HBsAg, deren Natur unklar ist. Somit konnte erstmals gezeigt werden, dass die zelluläre Immunantwort nach HBV-Impfung der Entwicklung von Anti-HBs Wochen, in Einzelfällen Monaten vorausging.

In this controlled, prospective pilot study 16 subjects were monitored during a basic Hepatitis-B-vaccination for an early cellular immune response. Therefore, blood was drawn ten times in a course of nine months and tested in an Elispot-Assay for a HBsAg-specific IFNy and iL 2 activity. This was compared to the development of anti-HBs.
48.5% developed a specific cellular immune response within 48 hours and over 80% developed within 7 days after the first vaccination shot. The dominating cytokine in the early phase was IFN y. Surprisingly, 6 out of 16 subject showed a HBsAg-specific INF y response before the first vaccination shot. This study showed, for the first time, that the cellular immune response preceeds the development of anti-HBs weeks, and in some cases even months.
