Eine Vielzahl an Ursachen kann dazu führen, dass ein Organ so stark geschädigt wird, dass das Überleben nicht möglich ist. Eine Transplantation ist für den Patienten die einzige langfristige Möglichkeit, ein annähernd normales Leben zu führen. Mesenchymale Stammzellen, vor über 30 Jahren im Knochenmark entdeckt, können einen starken immunsuppressiven Effekt ausüben. Wenn die Wirkmechanismen dieser Zellen ausreichend verstanden sind, kann die zelluläre Immuntherapie zur Reduktion der Dosen von klassischen Immunsuppressiva führen, die mit vielfältigen Nebenwirkungen vergesellschaftet sind. Es wurde eine Charakterisierung muriner mesenchymaler Stammzellen erarbeitet, die durch molekulargenetischen Knockout entweder keinen Interferon-γ-Rezeptor oder keine funktionstüchtige Cyclooxygenase-2 besitzen. Damit sollen die Mechanismen, die zur Immunsuppression dieser Zellreihe führen, besser verstanden werden. Die Knockout-Zellen wurden im Hinblick auf ihr Wachstum in Zellkultur, ihren
Immunphänotyp, ihr Differenzierungsverhalten in verschiedene Stützgewebe und ihr immunsuppressives Verhalten mit Stammzellen aus Wildtyp-Mäusen verglichen. Die genetische Veränderung wurde mittels PCR kontrolliert.
Nach Präparation aus dem Fettgewebe konnte bei allen untersuchten Zellen ein zuverlässiges Zellwachstum unter normalen Kulturbedingungen beobachtet werden. Die Interferon-γ- und die Cyclooxygenase-2-Knockout-ASC (ASC = Adipose tissue derived mesencymal stem cells) präsentierten denselben Immunphänotyp wie die Wildtyp-Stammzellen, mit dem Unterschied, dass sie eine deutlichere Expression des CD 34 aufzeigen. Unter verschiedenen Differenzierungsstimuli entwickelten alle untersuchten Zellen
eine Einlagerung von Fettvakuolen und zeigten Mineralisationen von Kalkkristallen im Sinne einer Differenzierung zu Osteoblasten und Adipozyten. Unter Kultur mit Mitogen-aktivierten Splenozyten konnten die ASC des Wildtyp-Stammes eine dosisabhängige Immunsuppression auslösen. Eine gleichwertige Hemmung der Proliferation der Lymphozyten konnte durch die IFNγ-Knockout-ASC erreicht werden, so dass davon ausgegangen werden muss, dass auch ohne funktionstüchtigen Rezeptor für dieses Zytokin eine Aktivierung der mesenchymalen Stammzellen möglich sein kann. Ohne Cyclooxygenase-2 ließ sich nur im Verhältnis 1:1 eine Hemmung der Zellteilung der Responder-Zellen zeigen. Alle anderen Proben waren im Vergleich zu den Wildtyp-Stammzellen nicht signifikant immunsuppressiv. Eine essentielle Rolle der Prostaglandine für die hemmende Wirkung von mesenchymalen Stammzellen auf die Proliferation von Lymphozyten und anderen Zellen des Immunsystems wird durch die Ergebnisse dieser Arbeit deutlich gemacht.

Translation of the abstract (English)

There are many reasons for serious organ damage that makes longterm survival impossible for the patient. Solid organ transplantation is in that case the only possibility for patients to survive. Mesenchymal stem cells (MSCs) have been discovered in the bone marrow over 30 years ago and can have a strong immunosuppressive effect. Once the immunological pathways are fully understood, cellular immunotherapy can cut back the dosages of classical immunosuppressants, which are accompanied by many side effects. In this study murine mesenchymal stem cells with genetic Knockout of either the interferon-γ-receptor or the cyclooxygenase-2 where investigated to better understand the pathway of the immunosuppressive effect the stem cells exert. The cells were compared to wildtype stem cells regarding their growth in cell culture, their phenotype, their ability to differentiate into various subtypes of connective tissue and their immunosuppressive potential. The correct genetic modification was verified by polymerase chain reaction. After separation out of adipose tissue all cells (Adipose-tissue derived mesenchymal stem cells, ASC) showed constant growth under normal cell culture conditions. The knockout cells without interferon-γ-receptor or cyclooxygenase-2 presented the same phenotype as the wildtype cells with the only difference being the presence of CD 34, an antigen of the hematopoietic cell line. Exposed to diverse stimuli, the stem cells differentiated into adipocytes and osteoblasts. In coculture with Mitogen-activated splenocytes wildtype ASCs were capable to suppress the proliferation in a dosage-dependent way. The ASCs without Interferon-γ-receptor suppressed proliferation of splenocytes to the same level as the wildtype cells. This is an indicator of a way to activate MSCs apart from the interferon-γ-receptor. Cyclooxygenase-2-negative MSCs showed immunosuppression exclusively at a ratio of 1:1. There was no significant immunosuppression in the other samples. Our results make an essential role for prostaglandins in the reduction of lymphocyte proliferation likely. Further studies will have to elucidate the molecular pathways which lead to the immunosuppressive effect of MSCs.
